Afghanistan

Afghanistan remains one of the most dangerous and most violent, crisis-ridden countries in the world., jump to country, relief efforts in afghanistan.

Poverty is deeply entrenched in Afghanistan. This can be attributed in part to the country’s physical isolation, rugged terrain and often har s h climate; to underlying cultural norms, especially with regard to the rights of women; and to its recurring history of conflict, insecurity and outside interference.   

Afghanistan’s poverty profile is further shaped by marked disparities between rural and urban areas , sustained population growth, extremely low levels of female participation in the work force, and the overwhelming prevalence of vulnerable forms of employment in informal and low-productivity jobs.   

Afghanistan remains one of the most dangerous and most v i olent, crisis-ridden countrie s in the world. There is persistent deepening and geographic spread of conflict. There are also regular violations of International Law and Human Rights. The security situation in Afghanistan continued to deteriorate in August 2021 when the country experienced a change in power and many feared that hard-won development gains, particularly for women and girls, would disappear.

CARE first established its mission in Afghanistan in 1961, but suspended activities after the Russian invasion of 1979. Resuming activities in 1989 f rom  a new base in Peshawar, Pakistan, CARE delivered assistance to Afghanistan from across the border until 2002 when it shifted its main office back to Kabul.   

CARE Afghanistan has developed three programs:  

  • Women’s & girls’ empowerment  
  • Enhanced resilience  
  • Humanitarian action  

Each program sets out to contribute to significant and lasting change for socially and economically vulnerable  women and girls who are lacking access to essential services. These programs focus largely on Sexual, Reproductive and Maternal Health, Food Security and Liv eli hoods , Women’s Economic Empowerment, Inclusive Governance, Education and Emergency Response.

The need in Afghanistan is overwhelming and continues to grow. As conflict escalated in 2021 and families fled their homes for safety, millions are running out of food.

The Community Care Travel Team is an innovative, cost effective, chronic disease management program that provides a range of support services to patients in their Med Net One primary care physician’s office rather than an offsite facility or hospital.

Our Community Care Travel Team (CCTT) Services: Our CCTT is comprised of registered nurses, registered dietitians, certified diabetes educators, certified pre-diabetes program educators, exercise specialists, behavioral health specialists and lifestyle coaches that provide a multitude of services within the comforts of a physician’s practice:

  • Education on self-management practices
  • Healthy lifestyle education
  • Individual or group sessions
  • Telephonic support between appointments

The patients work with the team to:

  • Manage their condition through diet and exercise
  • Better understand their medications
  • Manage emotional and physical health
  • Improve motivation to ensure lasting changes
  • Maximize quality of life

Community Care travel Team and Primary Care: The CCTT concept is based on extensive research showing that self-management, group visits and integration of the telephone into the care program work to:

  • Diminish patient symptoms
  • Enhance patient activity
  • Increase patient independence
  • Encourage patients to take a more proactive role in their care

In addition to the benefits experienced by chronically ill patients, offering the CCTT in a physician’s practice provides the capability to:

  • Decrease healthcare costs
  • Reduce redundancy of services
  • Reduce the number of prescriptions filled
  • Reduce the annual number of office visits
  • Reduce the number of unique providers involved in patient care
  • Decrease hospital stays
  • Decrease frequent emergency department and urgent care visits

CCTT and the Patient-Centered Medical Home: The CCTT program offers chronic illness management services in a cost-effective manner and lays the foundation for team-based care a key component of a “Patient-Centered Medical Home (PCMH).” A PCMH is a healthcare setting that facilitates partnerships between individual patients, their personal physicians and when appropriate, the patient’s family.

I started with Fresenius Travel Jan 2018, and it has been the best decision I’ve ever made. My first assignment was in Atlanta, GA where I was actually cross-trained into Acutes. I was in Atlanta for 5 months and learned a lot and met so many people!

My next assignment was in Hawaii, which I was at the Hilo clinic for 3 months, and then went to the Lanai clinic for 3 months. Hawaii was an amazing experience and I can’t wait to go back! I’m on an 11 month float assignment in WA now and continue to learn and grow in my nursing every day. I enjoy helping each clinic I go to and learning the different cultures of each area.

Orprn team members travel to napcrg pbrn conference.

Several ORPRN team members flew to Reston, VA in June to present at the annual North American Primary Care Research Group (NAPCRG) Practice-Based Research Network (PBRN) Conference. The Conference is an opportunity to network and share learning with PBRNs from around the world. Conference planning is supported by ORPRN Director, Melinda Davis, who serves on the NAPCRG PBRN Steering Committee.  

ORPRN presented posters and plenary sessions and participated in panel discussions over the two day conference. Nine conference sessions highlighted work occurring at ORPRN, including the following:

Notably this year, the PBRN conference was attended by funding partners at the Agency for Healthcare Research and Quality (AHRQ) who sponsor the annual meeting as well as multiple leaders from the National Institute of Health who are advancing initiatives to create a national primary care research network titled: Communities Advancing Research Equity for Health (CARE for Health TM ).

Here’s what to do if you need to visit an international hospital during your travels

ACT budget invests almost $58 million in children's health services, including for new paediatric critical care team

Megan Beit's son Jon has spinal muscular atrophy – a condition which sees the muscles involved in movement progressively weaken and become wasted over time.

Because the specialists he needs to see regularly aren't available in Canberra, the family moved from the ACT to Bywong – near Lake George – to be closer to Sydney.

"We have to travel quite a lot," Ms Beit said.

"We've got two other children, so having to make sure they're cared for, the house is cared for, the animals are looked after whenever we have to go and travel – and it costs money every time we go to Sydney.

"Quite frankly it uproots our life every time.

"We've got to plan who's staying, if someone's staying at the hospital with him or in a hotel nearby, food — all of those things."

The Beit family have stayed at Ronald McDonald House in Sydney, where they say they often meet other families from the Canberra region.

"We all have the same story really: that we have to travel, that we have to do all of these things because we can't do it in our home city," Ms Beit said.

"We didn't realise how much we'd needed to be in Sydney, and we had to have the conversation about whether we move to Sydney or not.

"Ultimately we made the decision not to, because all of our roots are [in Canberra].

"But being a capital city we expected that some of that 'gold standard' treatment would be available here – and it's just not."

The ACT government argues Canberra's population simply isn't big enough to provide enough patients to make it a viable location for many paediatric specialists to practice.

A liaison service has already been established to help coordinate services and supports for families who need to travel interstate for treatment . 

'Starting to build more of those services here': Health Minister

ACT Health Minister Rachel Stephen-Smith speaks at a press conference, she has short red hair and black glasses.

Health Minister Rachel Stephen-Smith said the ACT budget, released today, includes an additional $58 million for paediatric inpatient care and community-based services.

Along with eight new paediatric beds at the Canberra Hospital, the funding will also go towards establishing a new paediatric critical care team.

"[The team will] be a bit of a bridge between what our teams are currently able to do and those children who need to be transferred to Sydney to provide that higher level of care for critically ill children," Ms Stephen-Smith said.

"We simply don't have the population here in the ACT and surrounding region to support a full paediatric intensive care unit.

"But we do know that we need to be able to support children who are deteriorating or in need of critical care at a higher level than we have been able to up until this point."

About $2 million will go towards expanding rehabilitation services for children with complex and chronic conditions, where care is shared between services in Canberra and interstate hospitals.

"We often see that children are delayed in their return from Sydney when they've had to receive that critical care for an illness or major trauma because there haven't been paediatric rehabilitation services in the ACT," Ms Stephen-Smith said.

"So this is the step towards starting to build more of those services here."

The Neonatal Intensive Care Unit and Special Care Nursery will receive $18 million for additional cots and the government also intends to set up a new location for community based paediatric services so families don't need to go into a hospital setting for treatment.

"That's part of listening to the community including through the kids interstate care project we did with the Health Care Consumers' Association some years ago, and we've continued to invest in the recommendations from that review," Ms Stephen Smith said.

While these measures mean Jon will still need to go to Sydney for treatment at least some of the time, Ms Beit said even small improvements would count.

"Even one trip not going to Sydney is a good thing for us," Ms Beit said.

"I think that other families as well will find that even that small amount of support can go a long way."

837 miles. That is, officially at least, the distance the Lib Dem bus will travel on its final campaign push from John O'Groats in the North East of Scotland, to Land's End on the tip of Cornwall.

Add in several daily campaign stops (and, insists our producer Katy, the fact that she and a hardcore crew rode the bus all the way up from London), and the total distance will be considerably more.

Nor is it simply another Lib Dem gimmick. Both John O'Groats and Land's End are in constituencies the party are very much targeting at this election.

What's instructive, though, are the stops they have planned in-between.

It's been a busy start to the trip, with four stops in Scotland alone. A mixture of seats the party is defending, and those it hopes to take from the SNP.

But once we leave Edinburgh tonight, we drive for several hours without stopping, until we reach the South West of England.

And not by accident. Even the most generous polls only have the Lib Dems gaining 6 seats across the whole of the North of England and the Midlands.

This is a highly targeted campaign. The strategy is to focus on areas where the party has historical strength, and where it has recently come second, primarily to the Conservatives.

And of approximately 85 such seats, 69 are in the South West, South East and London.

It is there that the Lib Dem campaign will succeed or fail. And it is there that we will spend the last precious few days left to campaign.

No doubt with the occasional stunt thrown in for good measure.

The Welsh electorate has travelled in one direction for the last 100 years.

Labour always win more votes and seats than their rivals in Wales but, floating our parliamentary bench on a barge across the Pontcysyllte Aqueduct, we discover not everything is as tranquil as it seems in this stunning beauty spot.

The 20mph speed limit, wind farms and waiting times on the NHS are all matters raised by people who are not happy with Labour's record in the Senedd.

The first person we meet is Reuben Jones, who works at the local barge hire company adjacent to the aqueduct.

"I'm a transgender person," says Reuben. "I'm very unhappy with the state of the Israeli-Palestinian conflicts at the moment. There are a lot of issues with the education system, a lot of problems with the health care system."

With under a week to go, the Tories and Labour have taken a drop, while support for Reform UK and the Liberal Democrats is on the rise.

First Minister of Scotland John Swinney has joined criticism of "racist and homophobic comments" allegedly made by Reform UK campaigners.

Despite this, he said they are "not a surprise to me".

Mr Swinney, also the leader of the SNP, said: "I think this is where the politics of Reform ends up and I deplore it. I deprecate it. I want nothing to do with it. 

"We will always rail against racist and homophobic comments, and I can't believe that there's a single thing Nigel Farage can do to control a problem that he himself has started."

The SNP leader goes on to claim that this is not a case of bad apples, but "an ingrained problem of Reform".

He says: "Nigel Farage has set this all up. He has stoked it all.

"With every word over all these years, he has incited all that intolerance and prejudice in our society. 

"I want to have nothing to do with it. 

"And I don't think there's anything Nigel Farage can do to stop it, because he created it."

Reform UK has reported Channel 4 to the elections watchdog for alleged "scandalous... interference" over what the party claims was a fake rant planted by the broadcaster.

Footage from an undercover Channel 4 reporter showed Reform campaigner Andrew Parker using a discriminatory term about the prime minister, as well as saying the army should "just shoot" migrants crossing the Channel.

Police are now assessing the comments to establish if an offence has been committed, while Mr Sunak  said the insult directed at him "hurts and it makes me angry".

Mr Farage, the leader of Reform UK, has since suggested that Channel 4 may have paid an actor to make racist comments about the prime minister, posing as a canvasser for Reform.

In a letter to the Electoral Commission, party secretary and leading barrister Adam Richardson claimed that "Mr Parker is not a property developer as he has alleged but a jobbing actor".

He added it was "entirely evident that Mr Parker was a plant within the Channel 4 news piece". 

The letter said: "It is wholly unbelievable that by complete coincidence Channel 4 were performing an undercover investigation and by chance were paired up to go canvassing with a man who was pretending to be someone else, using a false voice and saying almost exclusively racist and bigoted remarks."

Nigel Farage, the leader of Reform UK, has said he is refusing to appear on Laura Kuenssberg's Sunday morning politics show until the BBC apologise for a "dishonest" audience.  

He claimed this audience existed during his appearance on Question Time.

Mr Farage says: "I have just been invited to appear on Laura Kuenssberg. 

"I'm refusing until the BBC apologises for their dishonest QT audience. 

"Our state broadcaster has behaved like a political actor throughout this election. 

"Reform will be campaigning vigorously to abolish the license fee."

Sir Ed Davey, leader of the Liberal Democrats, has insisted that his party shares "no values with Nigel Farage" in the wake of a new racism row.

It comes after Mr Farage, the leader of Reform UK, suggested that Channel 4 may have paid an actor to make racist comments about the prime minister, posing as a canvasser for Reform.

Sir Ed says: "Liberal Democrats share no values with Mr. Farage. He can sort himself out. My job as a Liberal Democrat leader is to tell you what we're about. 

"I want to fight a positive campaign about how we rescue our NHS and make sure we get the investment in health and care. Make sure we sort out the cost of living problems that people are really suffering under, and deal with things like the environmental issues, like sewage. 

"You know, and I think people get fed up in this campaign when there's all these diversions and nastiness. They want to know what you're going to do for them. 

"And that's what the Liberal Democrats are going to say."

Reform UK has withdrawn support from three of its candidates, the party has confirmed.

Nigel Farage's party is no longer backing Edward Oakenfull, Robert Lomas, and Leslie Lilley - who were all previously candidates for Reform.

Mr Oakenfull reportedly penned social media posts about the IQ of sub-Saharan Africans - which he told the BBC were "taken out of context".

Meanwhile, Mr Lomas allegedly said black people should "get off [their] lazy arses" and stop acting "like savages".

Ms Lilley is accused of describing people arriving on small boats as "scum".

The candidates will remain on the ballot paper, but are no longer endorsed by Reform UK.

It comes as Sky News identified two additional Reform UK parliamentary candidates who shared material deemed "vile" and in breach of the internationally-recognised definition of antisemitism.

The Archbishop of Canterbury urged people to avoid "personalised abuse" in the closing stages of the election campaign as he encouraged voters to go to the ballot box.

The Most Reverend Justin Welby said: "In these last few days before the election, let us pray for all candidates taking part in this most essential act of democracy.

"Let us encourage courteous and kind debate and not use personalised abuse.

"Let us carefully consider issues and the common good, and above all vote."
