Every Cruise Line's Requirements for COVID Vaccines, Testing

A guide to every major cruise line's health requirements for safe sailing.

Cruise lines have shifted their requirements for passengers, making cruising accessible to practically everyone. Many popular cruise lines have eliminated the vaccine and pre-embarkation test as the industry is beginning to shift away from mandatory vaccines and testing.

The changes began shortly after the CDC ended a pandemic-era policy of publicly displaying COVID-19 cases onboard, different cruise lines to the public in July 2022. For those looking to check current COVID-19 trends onboard the CDC urges direct contact with the cruise line, and the organization will continue to monitor and offer guidelines to ships.

Below, we break down each line's list of regulations to sail as well as what travelers need to know to plan.

Avalon Waterways

Where they sail: These small-ship river cruises sail throughout Europe, Asia, South America, and down the Nile River in Egypt.

Who needs the vaccine: All international travelers from the U.S. must be fully vaccinated before boarding. Beginning March 2023 the vaccine is no longer required (but strongly encouraged).

What other safety measures are in place: All guests will undergo a health screening upon arrival and luggage will be disinfected. Avalon will provide COVID-19 testing for travelers who need to show proof of a negative test to return home at no extra cost. Depending on the location of the cruise a pre-departure COVID-19 test may or may not be required.

Where they sail : Azamara sails mid-size ships all over the world, to all seven continents.

Who needs the vaccine : All guests and crew 12 and older will be required to be fully vaccinated at least two weeks before boarding a ship. Beginning Dec. 1, 2022, the company plans to drop the vaccine requirement for departures from the U.S. and Europe.

What other safety measures are in place : Depending on the port of embarkation guests may or may not to pre-test to travel. For a full list of ports and whether they require a test guests can check online .

Azamara's ships have been upgraded with new HVAC filtration systems, and EPA-certified disinfectants are used to clean the ship.

Carnival Cruises

Where they sail: Carnival sails large ships around the world, including popular trips to the Caribbean. The company sailed its maiden voyage on the Mardi Gras out of Florida in July.

Who needs the vaccine: All guests are encouraged to be vaccinated but unvaccinated travelers, or vaccinated guests without proof of vaccination, will have to present the negative results of a PCR or antigen test taken no earlier than 3 days before sailing.

What other safety measures are in place: For cruises five days or less, there will no longer be pre-cruise testing for vaccinated passengers unless a specific port requires it. Itineraries including Bahamas, Bermuda, or Grand Cayman will still be required to test. All guests will also be required to fill out a health screening 72 hours before embarkation and undergo health screenings prior to boarding. Unvaccinated travelers on a cruise to Bermuda will be required to purchase travel insurance (children under 12 are exempt as long as they are traveling with vaccinated parents).

Both vaccinated and unvaccinated guests who have recovered from COVID-19 within three months of their sailing date, do not need the required pre-cruise COVID test before embarkation, only if they are at least 10 days past their COVID-19 infection, have no symptoms and present documentation of recovery from COVID-19 from their healthcare provider.

Celebrity Cruises

Where they sail: Celebrity Cruises sails all over the world.

Who needs the vaccine: The vaccine is no longer required to sail on cruises from the U.S. and Europe. A vaccine is required for sailings visiting Canada.

What other safety measures are in place: A pre-embarkation test is no longer required for vaccinated guests on sailings nine days or less. Unvaccinated passengers will still be required to test three days prior to sailing. Self-tests are acceptable from European and U.S. ports that don't stop in Bermuda or Canada.

Where they sail: Cunard, which sails all over the world, is known for its transatlantic journeys aboard the Queen Mary 2.

Who needs the vaccine: All guests 18 and older must be fully vaccinated. Unvaccinated passengers under 18 will be required to undergo a PCR test within 72 hours of their departure.

What other safety measures are in place: Cunard has "enhanced" their onboard ventilation systems and implemented mandatory mask-wearing policies indoors. All guests aged 4 and older are required to have a negative COVID-19 test before boarding. Crew will be tested regularly while on board. Guests who have certain pre-existing medical conditions, including those who are on supplementary oxygen, will not be allowed to board.

Disney Cruise Line

Where they sail: Disney sails family-friendly journeys including to the Caribbean, Europe, and Alaska.

Who needs the vaccine: Vaccination is no longer required by highly suggested.

What other safety measures are in place: Unvaccinated guests must provide results of a COVID-19 test taken 1 to 2 days before sail date, at home tests are not accepted, this requirement will end Nov. 14. The Cruise line also has a full list of enhanced cleaning protocols.

Grand Circle Cruise Line

Where they sail: Grand Circle Cruise Line sails small ships and river cruises all over the world, including an extensive list of European itineraries. The company plans to start sailing again in August, including to Greece, Turkey, and Italy.

Who needs the vaccine: All guests and crew are required to be fully vaccinated with the booster shot.

What other safety measures are in place: All ships have been equipped with High Efficiency Particular Air (HEPA) filters and buffets have been eliminated. The cruise line states some itineraries will require proof of a negative COVID-19 test prior to travel , and that instructors will be sent out 30 days prior to travel.

Holland America

Where they sail: Holland America sails large ships all over the world, including to the Mediterranean, Caribbean, Europe, and more.

Who needs the vaccine: Unvaccinated guests are welcome but will be required to self-test three days before cruising.

What other safety measures are in place: Vaccinated passengers traveling on ships for less than 16 days excluding itineraries with the Panama Canal, trans-ocean, and other selected itineraries will no longer be required to test.

Lindblad Expeditions

Where they sail: Lindblad Expeditions is known for its adventurous trips to hard-to-reach destinations like Antarctica and the Russian Far East, partnering with National Geographic to add to the experience.

Who needs the vaccine: All guests 12 and older need to be vaccinated before boarding. Boosters are recommended but not required.

What other safety measures are in place: Pre-departure testing will not be required unless for a specific destination.

Margaritaville at Sea

Where they sail: Margaritaville at Sea offers 3-day cruises to Grand Bahama Island departing from the port of Palm Beach. The cruise line also offers the opportunity to take the ship one way to the Bahamas to allow guests to stay at the Margaritaville resort, and then take the ship back to the Bahamas at a later date.

Who needs the vaccine: All guests are welcome regardless of vaccination status, according to the cruise line.

What other safety measures are in place: All crew members must be vaccinated for COVID-19 and the cruise line states there are vaccination requirements on board (but does not specify).

MSC Cruises

Where they sail: MSC Cruises sails large ocean cruises around the world, including throughout Europe, South America, the Persian Gulf, and the Caribbean. In August, the company started sailing to the Bahamas with a stop at its private island, Ocean Cay.

Who needs the vaccine: Vaccinations are recommended but no longer required.

What other safety measures are in place: Fully-vaccinated passengers are no longer required to show proof of a negative test unless it is required of the destination. Unvaccinated children must show proof of a negative PCR test.

Norwegian Cruise Line

Where they sail: Norwegian sails big ocean cruises all around the world, including popular itineraries all throughout the Caribbean and Greece.

Who needs the vaccine: All passengers may sail regardless of vaccination status.

What other safety measures are in place: Testing is no longer required for any passenger regardless of vaccination status with the exception of local government mandates.

Oceania Cruises

Where they sail: Oceania Cruises sails all over the world, including offering several World Cruise itineraries.

Who needs the vaccine: All passengers regardless of vaccination status are allowed to sail.

What other safety measures are in place: Only unvaccinated guests will be required to show proof of a negative PCR or antigen test taken 72 hours prior to boarding. Unvaccinated children aged 12 years and younger are exempt.

P&O Cruises

Where they sail: The UK-based cruise line sails around Europe, including to the Canary Islands, as well as to the Caribbean from Southampton in the UK.

Who needs the vaccine: Although P&O Cruises' policy states that passengers over the age of 15 are required to be vaccinated, the latest update on its website says guests will be contacted directly in regards to vaccination protocol.

What other safety measures are in place: All guests are required to undergo a COVID-19 test at the terminal. Masks will be required on board indoors, and all shore excursions will be with vetted operators.

Princess Cruises

Where they sail: The global cruise line sails large ocean ships all around the world, from Australia to Alaska.

Who needs the vaccine: All guests are welcome on board, but unvaccinated passengers will be required to test and may need an additional medical exemption at some ports.

Regent Seven Seas Cruises

Where they sail: Regent Seven Seas offers all-inclusive luxury cruises around the globe, which include perks like free airfare, free excursions, and complimentary unlimited drinks on board.

Who needs the vaccine: All guests are now welcome. Unvaccinated passengers will be required to test 3-days before embarkation.

What other safety measures are in place: Pre-departure testing may not be necessary depending on the port. Currently cruises leaving from Bermuda, Canada, and Greece will require a test.

Royal Caribbean International

Where they sail: Royal Caribbean sails large ocean ships all over the world, including many popular itineraries throughout the Caribbean and Asia.

Who needs the vaccine: Everyone is welcome regardless of vaccination status, passengers traveling to Bermuda or Canada are required to be vaccinated.

W hat other safety measures are in place: On Royal Caribbean, unvaccinated travelers 12 and older leaving from a U.S. or Caribbean port still have to get tested within three days of embarkation,  according to the cruise line , vaccinated guests are exempt.

Royal Caribbean will accept a variety of tests as proof to board, including simply taking a photo of a self-administered home test.

Where they sail: The British cruise line, exclusively for guests 50 and older, sails both ocean and river cruises around the globe.

Who needs the vaccine: All guests must be fully vaccinated, including a booster shot at least 14 days before boarding a ship. Saga was the first major cruise line to implement this vaccination policy.

What other safety measures are in place: Ocean Cruise guests will be required to get tested for COVID-19 in the terminal. Masks are required on bus transfers, in hotels, and on excursions.

Where they sail: Seabourn's mid-size luxury cruises head all around the world, from Alaska to the Caribbean.

Who needs the vaccine: Vaccinations are not required on most cruise itineraries, unvaccinated guests will be required to present a negative test taken within three days of embarkation.

What other safety measures are in place: Masks are recommended onboard in most indoor venues and the casinos are frequently sanitized.

Where they sail: Silversea brings guests all around the world in luxury and style, including to the Galapagos and Antarctica.

Who needs the vaccine: All guests are welcome, however, unvaccinated guests may not be welcome on all sailings depending on local regulations.

What other safety measures are in place: Vaccinated guests do not need a pre-departure test unless local regulations such as cruises leaving from Australia, Bermuda, Canada, or Greece require it. Unvaccinated travelers will need a negative COVID-19 test within 72 hours of embarkation.

UnCruise Adventures

Where they sail: This small-ship company is known for its Alaska journeys as well as adventures in places like the Galapagos and island hopping in Hawaii.

Who needs the vaccine: As of April 7, all guests are welcome on board regardless of vaccination status.

What other safety measures are in place: Guests will no longer be required to present proof of vaccination or a COVID-19 test prior to, or during any point of sailing and the cruise line will no longer be running at limited capacity. These measures go into effect on April 7, until then guests 12 and older must provide proof of vaccination, and itineraries may sail at less then full capacity.

Masks are required in public spaces, according to the company "bandana" and "buff" face masks will not be accepted.

Victory Cruise Lines

Where they sail: Victory Cruise Lines is known for its Great Lakes cruises, providing all-inclusive sailings to see breathtaking sights from Niagara Falls to the iconic architecture of Chicago from Lake Michigan. Beyond the U.S., Victory Cruise Lines sails to Mexico's Yucatán Peninsula.

Who needs the vaccine: All guests and crew will be required to be vaccinated before boarding, and will be required to show physical proof prior to boarding.

What other safety measures are in place: Testing is no longer required on any river cruise.

Luggage is then disinfected before being brought onto the ship and staterooms cleaned with electrostatic fogging. Masks will be required during the embarkation process and while riding a shore excursion bus, but will not be required on board.

Viking Cruises

Where they sail: Viking sails ocean and river cruises all over the world, including throughout Europe. This summer, Viking will sail several voyages, including around England for UK residents, to Bermuda, and to Iceland in June.

Who needs the vaccine: All guests must be fully vaccinated to board a ship. Guests who are eligible must have a booster.

What other safety measures are in place: In addition to vaccines, all guests may be required to undergo a saliva PCR test at embarkation as well as "frequent" testing throughout the journey. All staterooms are equipped with independent air handling units.

Virgin Voyages

Where they sail: The brand-new cruise line is launching mini sailings from England for UK residents, and has cruises throughout the Caribbean, and transatlantic options.

Who needs the vaccine: All are welcome regardless of vaccination status, the cruise line removed all vaccine and testing requirements for all its ships in late October, according to the company.

What other safety measures are in place: Virgin Voyages was the first cruise line to eliminate pre-departure testing for vaccinated guests. The cruise line will continue to work with an advisory board to help ensure health and safety on board at all times, including the use of air filtration systems and sanitization of high touch surface areas.

Windstar Cruises

Where they sail: Windstar operates small-ship cruises and several sailing vessels all around the globe, including to places like Central America .

Who needs the vaccine: All guests will be required to be fully vaccinated at least 14 days before boarding one of Windstar's yachts. Boosters are highly encouraged and are recommended to be administered at least one week prior to travel.

What other safety measures are in place: Guests no longer need a pre-embarkation test unless a specific destination requires it.

Costa cruises: new simplified protocols to better enjoy the upcoming fall and winter vacations.

As of October 8th, on Costa cruises in the Mediterranean, the Caribbean and the United Arab Emirates, fully vaccinated guests will be able to embark without any further tests.

Genoa, September 19, 2022 – Costa Cruises is updating its Covid-19-related health protocols, making its vacations even more accessible, starting with those scheduled for next fall and winter.

Indeed, as of October 8th, new procedures will enter into force, leading to a further simplification of measures , especially the pre-embarkation ones, required on board the company's ships. From last spring, it is already possible to fully enjoy, without restrictions, on-board and ashore services, such as excursions, restaurants, bars, theaters, gyms, swimming pools, and wellness areas.

The main changes concern Mediterranean, the Caribbean and the United Arab Emirates , where fully vaccinated guests will no longer have to undergo pre-embarkation testing . In addition, in the Mediterranean and the Caribbean , unvaccinated guests with negative pre-embarkation antigen tests will also be able to embark.

Here below the details of the requirements in each destination.

MEDITERRANEAN AND THE CARIBBEAN On Mediterranean (excluding Greece) and the Caribbean cruises lasting less than 14 nights, guests who have completed the Covid-19 vaccination (2 doses + booster; 1 dose J&J + booster; 2 doses +recovery) will be able to embark the ships without having to undergo any more pre-embarkation tests. People who have not been vaccinated or cured from Covid-19 (without 2 doses) will also be able to travel with Costa in these destinations, simply showing at the embarkation an official certification of the negative result of an antigenic swab (or also RT-PCR molecular swab, at guest's choice) performed within 48 hours of embarkation time.

All cruises calling in Greece will continue to be available only to fully vaccinated, partially vaccinated and recovered guests (2 doses + booster; J&J + booster; 2 doses + recovery; 1 dose + recovery; recovery only), and who have undergone antigen testing with a negative result within 48 hours of embarkation.  

On cruises visiting Morocco, guests without full vaccination will be required to take an additional RT-PCR molecular test on board 48 hours prior to entry into the country. As for Caribbean cruises, shore leave of unvaccinated guests in Barbados, St. Vincent and St. Marteen is permitted only with a negative result of an antigen test performed 24 or 48 hours prior to the ship's arrival. These tests, as well as those scheduled prior to enter Morocco, can be done on board Costa ships at a special price.

UNITED ARAB EMIRATES Cruises in the UAE, Oman and Qatar are currently available only to fully vaccinated guests (2 doses + booster; 1 dose J&J + booster; 2 doses +recovery), but with no pre-embarkation testing required.

AROUND-THE-WORLD AND TRANSATLANTIC CRUISES Around-the-World*, transatlantic cruises, and generally all cruises longer than 14 nights, require guests to be fully vaccinated and take an antigen test with a negative result within 48 hours of embarkation time.  In addition, mask use on board will be mandatory in public areas during the first 7 days of the cruise, while in other destinations (Mediterranean, Caribbean, UAE) mask use is simply recommended.

In all destinations mentioned, where applicable, the full vaccination requirement applies only to guests aged 12 and over (except on Around-the-World and transatlantic cruises, from age 18 and over), and the swab requirement from age 5 and over.

The new version of the protocols will continue to ensure high safety standards on board . In case of need, Costa ships are still equipped to be able to perform Covid swab tests, both antigenic and molecular. Reinforcement of cleaning and sanitization of public areas, cabins, terminals, and vehicles used for excursions and guest transportation will also be maintained. The crew undergoes Covid testing prior to embarkation and periodically while on board. 

Costa protocols are consistent with measures currently in place in the destinations visited by the company's ships. If local authorities implement new measures in the future, Costa protocols will adjust accordingly. In such a case, the company will promptly inform its guests. Vaccination Policies Across Different Cruise Brands

As the pandemic situation continues to evolve, cruise lines are constantly adapting their health and safety protocols. Following recommendations from government and health authorities, several brands are also updating their vaccine policy requirements.

Cruise Industry News has studied some of those policies of a selection of cruise lines.

Carnival Cruise Line

Sailing from several U.S. homeports, Carnival Cruise Line is currently requiring most of its guests to be fully vaccinated against COVID-19.

“Carnival Cruise Line’s policy is that everyone must be vaccinated, with a very small number of exceptions for children under 12 and those unable to be vaccinated,” the company said on its website.

According to Carnival, the approach “exceeds the requirements for vaccinated cruises under the CDC’s guidance, and meets requirements set by the destinations” visited during the cruises.

Guests are considered to be fully vaccinated when they have received their final dose of an approved COVID-19 vaccine at least 14 days prior to sailing and have proof of vaccination.

Royal Caribbean International

According to the company’s website, Royal Caribbean International’s vaccination requirements vary by port of departure and age group.

For cruises departing from all U.S. homeports, all guests aged 12 and older must present proof of COVID-19 vaccination, with the final dose of their vaccine administered at least 14 days before sailing.

For cruises departing European ports, such as Barcelona (Spain) and Southampton (England), guests aged 18 and older are required to be fully vaccinated for COVID-19 at least 14 days before sailing.

Vaccination requirements may vary by itinerary, too. According to Royal Caribbean, information regarding the latest requirements for each departure port will be sent to all booked guests within 30 days before setting sail.

Norwegian Cruise Line

Norwegian Cruise Line is sailing 100-percent vaccinated cruises through the end of the year.

Therefore, all guests (including children) and crew embarking through Dec. 31 will be required to be fully vaccinated against COVID-19 to sail in the period.

MSC Cruises

MSC Cruises’ vaccination policy varies by region and country of origin.

Through Oct. 31, U.S. passengers cruising from a U.S. homeport are required to be fully vaccinated 14 days before the cruise. “If you are unable or unwilling to show proof of vaccine, you will be denied boarding,” MSC stated on its website.

Similarly, U.S. passengers aged 12 and older are required to be fully vaccinated to sail on MSC’s Mediterranean and Northern Europe cruises.

European guests, however, may be subjected to different vaccination policies. Italian residents who cruise in the Mediterranean and Northern Europe, for instance, can embark with a negative molecular COVID-19 test taken 48 hours before the cruise.

Princess Cruises

Currently sailing from the United States and the United Kingdom, Princess Cruises is operating vaccinated cruises. According to the company, the requirements are in accordance with health authority directives and follow the CDC definitions, with guests and crew vaccination rates close to 100 percent.

All cruises through Dec. 31 are currently only available to guests who have received their final dose of an approved COVID-19 vaccine at least 14 days prior to the beginning of the cruise and have proof of vaccination.

According to Princess, passengers will be asked to confirm they’re fully vaccinated by providing proof of vaccination before boarding.

Celebrity Cruises

According to the Celebrity Cruises website, all crew and guests who are eligible to be vaccinated must be fully vaccinated with all doses administered at least 14 days prior to sailing.

The company will also “continue to abide by CDC guidance and require all guests 12 years and older to be fully vaccinated with all COVID-19 vaccine doses administered at least 14 days prior to sailing.” 

Holland America Line

While Holland America Line’s health protocol varies by region, it requires guests to be fully vaccinated for every destination the brand is sailing in 2021.

According to Holland America’s website, cruises sailing through Dec. 31 are only available to passengers who have received their final dose of an approved COVID-19 vaccine at least 14 days prior to the beginning of the cruise and have proof of vaccination.

Saga Cruises

Saga Cruises is asking all guests to have had both doses of a COVID-19 vaccine prior to boarding.

After resuming service with domestic sailings in the UK in June, the two-ship British cruise line has resumed international operations, offering cruises to the Baltic and the Mediterranean.  

Bahamas Paradise Cruise Line

Bahamas Paradise Cruise Line reviewed its COVID-19 vaccination policy in August, following an order implemented by the Bahamas Customs & Excise Department.

According to a statement, the company is now requiring all guests 12 years and older to be fully vaccinated against COVID-19. The new rules are in place until Oct. 31, 2021.

TUI Cruises

TUI Cruises’ guests are currently required to show at least one negative COVID-19 test before cruise departure. Vaccination requirements, in the meantime, vary by country and region.

When sailing to the Caribbean, for instance, passengers must also show proof that they are fully vaccinated against COVID-19.

According to the company, guests will receive all the important information and requirements for their trip via email before the start of their journey.

What Cruise Lines Allow Unvaccinated Guests to Sail?

At long last, cruise lines are finally starting to drop their vaccine mandates and allow all passengers — inoculated or not — to sail on their ships.

The big question is, what cruise lines allow unvaccinated guests to sail?

In this article, we’ve compiled all cruise lines that allow unvaccinated guests to sail.

(Note: This information is accurate as of February 7, 2023. Check the cruise lines’ websites for the most up-to-date information.)

American Queen Voyages

Unvaccinated guests are welcome to sail on American Queen Voyages .

Vaccination for sailings in the U.S. is not required. Beginning March 11, 2023, proof of vaccination for sailings to or from Europe will no longer be required.

Carnival Cruise Line

Unvaccinated guests are welcome to sail on all voyages except those in Australia and people traveling 16 nights or longer. Check with Carnival for specific cruise requirements.

Celebrity Cruises

Unvaccinated passengers are welcome to sail on all cruises departing from Europe and the U.S. Vaccination continues to be required on voyages to or from Australia, New Zealand, and the Galapagos Islands.

Cunard Line

All guests 18 and older must be vaccinated to sail. On most sailings, kids and teens aged 17 and under do not have a vaccine requirement. Check with Cunard for information on your specific sailing.

Disney Cruise Line

Unvaccinated guests are welcome to sail on all voyages.

Holland America Line

Unvaccinated guests are welcome on most voyages. This doesn’t apply to select destinations where local regulations vary, including Australia.

MSC Cruises

Unvaccinated guests are permitted to sail on most MSC voyages.

Norwegian Cruise Line

Oceania cruises.

Unvaccinated guests are welcome to sail on all voyages except for itineraries sailing in destinations that require vaccinations.

Princess Cruises

Regent seven seas cruises.

Unvaccinated guests are welcome to sail on all voyages except for destinations that require vaccinations.

Royal Caribbean

Unvaccinated guests are welcome to sail on all voyages except those from Australia and on transatlantic and transpacific cruises.

Unvaccinated guests are welcome on most voyages, except those visiting destinations where local regulations vary, including Australia. Check with Seabourn for protocols on your specific sailing.

Silversea Cruises

Unvaccinated guests are welcome to sail on most voyages, except for itineraries that feature destinations with local regulations that may continue to require vaccination, including Australia.

All guests must be vaccinated to sail with Viking . Booster shots are not required.

Virgin Voyages

Travelers to risk areas should prevent mosquito bites. Routine vaccines

Recommendations.

Make sure you are up-to-date on all routine vaccines before every trip. Some of these vaccines include

• Chickenpox (Varicella)
• Diphtheria-Tetanus-Pertussis
• Flu (influenza)
• Measles-Mumps-Rubella (MMR)

Immunization schedules

COVID-19 vaccine

Chikungunya

There has been evidence of chikungunya virus transmission in Costa Rica within the last 5 years. Chikungunya vaccination may be considered for the following travelers:

• People aged 65 years or older, especially those with underlying medical conditions, who may spend at least 2 weeks (cumulative time) in indoor or outdoor areas where mosquitoes are present in Costa Rica, OR
• People planning to stay in Costa Rica for a cumulative period of 6 months or more

Chikungunya - CDC Yellow Book

Hepatitis A

Recommended for unvaccinated travelers one year old or older going to Costa Rica.

Infants 6 to 11 months old should also be vaccinated against Hepatitis A. The dose does not count toward the routine 2-dose series.

Travelers allergic to a vaccine component or who are younger than 6 months should receive a single dose of immune globulin, which provides effective protection for up to 2 months depending on dosage given.

Unvaccinated travelers who are over 40 years old, immunocompromised, or have chronic medical conditions planning to depart to a risk area in less than 2 weeks should get the initial dose of vaccine and at the same appointment receive immune globulin.

Hepatitis B

Recommended for unvaccinated travelers younger than 60 years old traveling to Costa Rica. Unvaccinated travelers 60 years and older may get vaccinated before traveling to Costa Rica.

CDC recommends that travelers going to certain areas of Costa Rica take prescription medicine to prevent malaria. Depending on the medicine you take, you will need to start taking this medicine multiple days before your trip, as well as during and after your trip. Talk to your doctor about which malaria medication you should take.

Cases of measles are on the rise worldwide. Travelers are at risk of measles if they have not been fully vaccinated at least two weeks prior to departure, or have not had measles in the past, and travel internationally to areas where measles is spreading.

All international travelers should be fully vaccinated against measles with the measles-mumps-rubella (MMR) vaccine, including an early dose for infants 6–11 months, according to  CDC’s measles vaccination recommendations for international travel .

Dogs infected with rabies are not commonly found in Costa Rica.

Rabies is present in bats.

If rabies exposures occur while in Costa Rica, rabies vaccines may only be available in larger suburban/urban medical facilities.

Rabies pre-exposure vaccination considerations include whether travelers 1) will be performing occupational or recreational activities that increase risk for exposure to potentially rabid animals and 2) might have difficulty getting prompt access to safe post-exposure prophylaxis.

Please consult with a healthcare provider to determine whether you should receive pre-exposure vaccination before travel.

Recommended for most travelers, especially those staying with friends or relatives or visiting smaller cities or rural areas.

Yellow Fever

Required for travelers ≥9 months old arriving from countries with risk for YF virus transmission. 1 Included in this requirement are travelers arriving from Tanzania and Zambia, and designated areas of: Colombia (the entire country, except the cities of Barranquilla, Bogotá, Cali, Cartagena, and Medellín, and the archipelago department, San Andrés and Providencia); Ecuador (the provinces of Morona-Santiago, Napo, Orellana, Pastaza, Sucumbíos, and Zamora-Chinchipe, and excluding the rest of the country); Paraguay (the entire country, except the city of Asunción); Peru (the entire country, except the cities of Cusco and Lima, the regions of Cajamarca, Lambayeque, Piura, and Tumbes, and the highland tourist areas of Machu Picchu and the Inca Trail); Trinidad & Tobago (the entire country, except the urban areas of Port of Spain; travelers with itineraries limited to the island of Tobago, and travelers with airport transits or layovers are also exempt from this requirement). Travelers arriving from Argentina and Panama are exempt from this requirement.

• Touching urine or other body fluids from an animal infected with leptospirosis
• Swimming or wading in urine-contaminated fresh water, or contact with urine-contaminated mud
• Drinking water or eating food contaminated with animal urine
• Avoid contaminated water and soil
• Avoid floodwater

Avoid bug bites, chagas disease (american trypanosomiasis).

• Accidentally rub feces (poop) of the triatomine bug into the bug bite, other breaks in the skin, your eyes, or mouth
• From pregnant woman to her baby, contaminated blood products (transfusions), or contaminated food or drink.

• An infected pregnant woman can spread it to her unborn baby

Airborne & droplet

• Breathing in air or accidentally eating food contaminated with the urine, droppings, or saliva of infected rodents

• Less commonly, being around someone sick with hantavirus (only occurs with Andes virus)
• Avoid rodents and areas where they live
• Avoid sick people

Tuberculosis (TB)

• Breathe in TB bacteria that is in the air from an infected and contagious person coughing, speaking, or singing.

Learn actions you can take to stay healthy and safe on your trip. Vaccines cannot protect you from many diseases in Costa Rica, so your behaviors are important.

Eat and drink safely

Food and water standards around the world vary based on the destination. Standards may also differ within a country and risk may change depending on activity type (e.g., hiking versus business trip). Prevent bug bites

Bugs (like mosquitoes, ticks, and fleas) can spread a number of diseases in Costa Rica. Many of these diseases cannot be prevented with a vaccine or medicine. You can reduce your risk by taking steps to prevent bug bites.

What can I do to prevent bug bites?

• Cover exposed skin by wearing long-sleeved shirts, long pants, and hats.
• Use an appropriate insect repellent (see below).
• Use permethrin-treated clothing and gear (such as boots, pants, socks, and tents). Do not use permethrin directly on skin.
• Stay and sleep in air-conditioned or screened rooms.
• Use a bed net if the area where you are sleeping is exposed to the outdoors.

What type of insect repellent should I use?

• FOR PROTECTION AGAINST TICKS AND MOSQUITOES: Use a repellent that contains 20% or more DEET for protection that lasts up to several hours.
• Picaridin (also known as KBR 3023, Bayrepel, and icaridin)
• Oil of lemon eucalyptus (OLE) or para-menthane-diol (PMD)
• 2-undecanone
• Always use insect repellent as directed.

What should I do if I am bitten by bugs?

• Avoid scratching bug bites, and apply hydrocortisone cream or calamine lotion to reduce the itching.
• Check your entire body for ticks after outdoor activity. Be sure to remove ticks properly.

What can I do to avoid bed bugs?

Although bed bugs do not carry disease, they are an annoyance. See our information page about avoiding bug bites for some easy tips to avoid them. For more information on bed bugs, see Bed Bugs .

Stay safe outdoors

If your travel plans in Costa Rica include outdoor activities, take these steps to stay safe and healthy during your trip.

• Stay alert to changing weather conditions and adjust your plans if conditions become unsafe.
• Prepare for activities by wearing the right clothes and packing protective items, such as bug spray, sunscreen, and a basic first aid kit.
• Consider learning basic first aid and CPR before travel. Bring a travel health kit with items appropriate for your activities.
• If you are outside for many hours in heat, eat salty snacks and drink water to stay hydrated and replace salt lost through sweating.
• Protect yourself from UV radiation : use sunscreen with an SPF of at least 15, wear protective clothing, and seek shade during the hottest time of day (10 a.m.–4 p.m.).
• Be especially careful during summer months and at high elevation. Because sunlight reflects off snow, sand, and water, sun exposure may be increased during activities like skiing, swimming, and sailing.
• Very cold temperatures can be dangerous. Dress in layers and cover heads, hands, and feet properly if you are visiting a cold location.

Stay safe around water

• Swim only in designated swimming areas. Obey lifeguards and warning flags on beaches.
• Practice safe boating—follow all boating safety laws, do not drink alcohol if driving a boat, and always wear a life jacket.
• Do not dive into shallow water.
• Do not swim in freshwater in developing areas or where sanitation is poor.
• Avoid swallowing water when swimming. Untreated water can carry germs that make you sick.
• To prevent infections, wear shoes on beaches where there may be animal waste.

Keep away from animals

Most animals avoid people, but they may attack if they feel threatened, are protecting their young or territory, or if they are injured or ill. Animal bites and scratches can lead to serious diseases such as rabies.

Follow these tips to protect yourself:

• Do not touch or feed any animals you do not know.
• Do not allow animals to lick open wounds, and do not get animal saliva in your eyes or mouth.
• Avoid rodents and their urine and feces.
• Traveling pets should be supervised closely and not allowed to come in contact with local animals.
• If you wake in a room with a bat, seek medical care immediately. Bat bites may be hard to see.

All animals can pose a threat, but be extra careful around dogs, bats, monkeys, sea animals such as jellyfish, and snakes. If you are bitten or scratched by an animal, immediately:

• Wash the wound with soap and clean water.
• Go to a doctor right away.
• Tell your doctor about your injury when you get back to the United States.

Consider buying medical evacuation insurance. Rabies is a deadly disease that must be treated quickly, and treatment may not be available in some countries.

Reduce your exposure to germs

Follow these tips to avoid getting sick or spreading illness to others while traveling:

• Wash your hands often, especially before eating.
• If soap and water aren’t available, clean hands with hand sanitizer (containing at least 60% alcohol).
• Don’t touch your eyes, nose, or mouth. If you need to touch your face, make sure your hands are clean.
• Cover your mouth and nose with a tissue or your sleeve (not your hands) when coughing or sneezing.
• Try to avoid contact with people who are sick.
• If you are sick, stay home or in your hotel room, unless you need medical care.

Avoid sharing body fluids

Diseases can be spread through body fluids, such as saliva, blood, vomit, and semen.

Protect yourself:

• Use latex condoms correctly.
• Do not inject drugs.
• Limit alcohol consumption. People take more risks when intoxicated.
• Do not share needles or any devices that can break the skin. That includes needles for tattoos, piercings, and acupuncture.
• If you receive medical or dental care, make sure the equipment is disinfected or sanitized.

Know how to get medical care while traveling

Plan for how you will get health care during your trip, should the need arise:

• Carry a list of local doctors and hospitals at your destination.
• Review your health insurance plan to determine what medical services it would cover during your trip. Consider purchasing travel health and medical evacuation insurance.
• Carry a card that identifies, in the local language, your blood type, chronic conditions or serious allergies, and the generic names of any medications you take.
• Some prescription drugs may be illegal in other countries. Call Costa Rica’s embassy to verify that all of your prescription(s) are legal to bring with you.
• Bring all the medicines (including over-the-counter medicines) you think you might need during your trip, including extra in case of travel delays. Ask your doctor to help you get prescriptions filled early if you need to.

Many foreign hospitals and clinics are accredited by the Joint Commission International. A list of accredited facilities is available at their website ( www.jointcommissioninternational.org ).

In some countries, medicine (prescription and over-the-counter) may be substandard or counterfeit. Bring the medicines you will need from the United States to avoid having to buy them at your destination.

Select safe transportation

Motor vehicle crashes are the #1 killer of healthy US citizens in foreign countries.

In many places cars, buses, large trucks, rickshaws, bikes, people on foot, and even animals share the same lanes of traffic, increasing the risk for crashes.

Be smart when you are traveling on foot.

• Use sidewalks and marked crosswalks.
• Pay attention to the traffic around you, especially in crowded areas.
• Remember, people on foot do not always have the right of way in other countries.

Riding/Driving

Choose a safe vehicle.

• Choose official taxis or public transportation, such as trains and buses.
• Ride only in cars that have seatbelts.
• Avoid overcrowded, overloaded, top-heavy buses and minivans.
• Avoid riding on motorcycles or motorbikes, especially motorbike taxis. (Many crashes are caused by inexperienced motorbike drivers.)
• Choose newer vehicles—they may have more safety features, such as airbags, and be more reliable.
• Choose larger vehicles, which may provide more protection in crashes.

Think about the driver.

• Do not drive after drinking alcohol or ride with someone who has been drinking.
• Consider hiring a licensed, trained driver familiar with the area.
• Arrange payment before departing.

Follow basic safety tips.

• Wear a seatbelt at all times.
• Sit in the back seat of cars and taxis.
• When on motorbikes or bicycles, always wear a helmet. (Bring a helmet from home, if needed.)
• Avoid driving at night; street lighting in certain parts of Costa Rica may be poor.
• Do not use a cell phone or text while driving (illegal in many countries).
• Travel during daylight hours only, especially in rural areas.
• If you choose to drive a vehicle in Costa Rica, learn the local traffic laws and have the proper paperwork.
• Get any driving permits and insurance you may need. Get an International Driving Permit (IDP). Carry the IDP and a US-issued driver's license at all times.
• Check with your auto insurance policy's international coverage, and get more coverage if needed. Make sure you have liability insurance.
• Avoid using local, unscheduled aircraft.
• If possible, fly on larger planes (more than 30 seats); larger airplanes are more likely to have regular safety inspections.
• Try to schedule flights during daylight hours and in good weather.

Medical Evacuation Insurance

If you are seriously injured, emergency care may not be available or may not meet US standards. Trauma care centers are uncommon outside urban areas. Having medical evacuation insurance can be helpful for these reasons.

Maintain personal security

Use the same common sense traveling overseas that you would at home, and always stay alert and aware of your surroundings.

Before you leave

• Research your destination(s), including local laws, customs, and culture.
• Monitor travel advisories and alerts and read travel tips from the US Department of State.
• Enroll in the Smart Traveler Enrollment Program (STEP) .
• Leave a copy of your itinerary, contact information, credit cards, and passport with someone at home.
• Pack as light as possible, and leave at home any item you could not replace.

While at your destination(s)

• Carry contact information for the nearest US embassy or consulate .
• Carry a photocopy of your passport and entry stamp; leave the actual passport securely in your hotel.
• Follow all local laws and social customs.
• Do not wear expensive clothing or jewelry.
• Always keep hotel doors locked, and store valuables in secure areas.
• If possible, choose hotel rooms between the 2nd and 6th floors.

Use the Healthy Travel Packing List for Costa Rica for a list of health-related items to consider packing for your trip. Talk to your doctor about which items are most important for you.

Why does CDC recommend packing these health-related items?

It’s best to be prepared to prevent and treat common illnesses and injuries. Some supplies and medicines may be difficult to find at your destination, may have different names, or may have different ingredients than what you normally use.

If you are not feeling well after your trip, you may need to see a doctor. If you need help finding a travel medicine specialist, see Find a Clinic . Be sure to tell your doctor about your travel, including where you went and what you did on your trip. Also tell your doctor if you were bitten or scratched by an animal while traveling.

If your doctor prescribed antimalarial medicine for your trip, keep taking the rest of your pills after you return home. If you stop taking your medicine too soon, you could still get sick.

Malaria is always a serious disease and may be a deadly illness. If you become ill with a fever either while traveling in a malaria-risk area or after you return home (for up to 1 year), you should seek immediate medical attention and should tell the doctor about your travel history.

Your sailing is fast approaching, and we're looking forward to welcoming you onboard. We would appreciate if you could take a moment to provide your COVID-19 vaccination status ahead of your cruise. Please complete the short form below.

NOTE: All Royal Caribbean guests aged 12 and older must present proof of COVID-19 vaccination with the final dose of their vaccine administered at least 14 days before sailing. While vaccination is not required for guests under 12, we kindly ask that you fill out their status below. Feel free to visit our  FAQs  for full details on vaccination requirements.

How do you count the 14 days for full vaccination, exactly? The 14 day time period starts the day you get your last dose. That’s Day 1. After 14 days pass, you can get onboard on Day 15. For example, if you received your final dose on October 1st, your 14 day countdown starts on October 1st, and you would be permitted to board the ship on October 15th or later.
