Before discussing how to exchange your timeshare with II cruises, it’s important to note that you must  own a timeshare  to get started, and the resort or brand must affiliate with Interval International. Interval International is meant for owners to deposit their points or weeks on the platform and then exchange it for something of equal or lesser “value.” There are a few different tiers of II membership, but only Interval Gold and Interval Platinum members can exchange towards cruises.

Interval International Tiers

There are three II tiers in total. Folks who sign up for II start at the  standard tier . You do not qualify to exchange your weeks or points for Interval International cruises at the standard tier. So, let’s focus on the two II tiers that allow you to do so. 

Interval International Gold

You can achieve amazing vacations with just the base-level membership of Interval International. However, Interval Gold is a step above regular membership. If you’re looking for Interval International cruises to exchange towards, ask your II representative about Interval Gold. Exchange your timeshare for shorter stays, rather than traditional week-long timeshare stays. Interval Gold offers Interval Options while allowing II owners to trade their points or week towards cruises, tours, golf, spas, and even hotels.

Interval Gold also comes with VIP Concierge, a 24/7 personal assistant for travel planning. When you book an II cruise with Interval Travel, members can even receive cash back on select vacations.

Interval International Platinum

Furthermore, when you upgrade from an Interval International Gold membership to an Interval Platinum membership, there are new perks that you now have access to. With Interval Platinum, you get all the benefits of the Gold membership plus more!

In addition, you can trade your weeks/points for a hotel stay or condo-style-accommodations with “Hotel Exchange.” Use “Interval Experience” to exchange your weeks or points towards different adventures. Does anyone have any experience working directly with Interval International in purchasing a cruise? Do they offer any great deals. I know they offer great prices on get aways. I just purchased a time share (Cape Cod) and will be a member of Interval International in a few weeks.

Not II but I booked a cruise with RCI (II's competition- not the cruiseline).

The price was good. It was a special in thir magazine. I'd recommend comparing their price with the online cruise discounters. It needs to be much cheaper if you are using points or transferring your timeshare for the cruise.

For our very first cruise, I booked through II. We too own on the Cape! There rate was good. However, they are just "Order Takers". If you need help, information, changes etc., then you are dealing with just a customer service representative. They may find the right answer or they may not. When I have subsequentially given them an opportunity to quote a cruise, they are not the best or elast expensive. Major Travel Agencies whether on-line or around the corner, offer much more personalized service for the best rate.

Since we are almost neighbors, if you have any questions, please don't hesitate to ask.

We work with RCI for our exchanges and every time we've checked cruise prices through them, the rates were higher or about the same as if we were booking through a TA. In most cases, we've found it would not be worth it to do an exchange for a cruise. However, we did get a very good price for a Panama Canal Cruise on RCCL by booking with the RCI travel department and not involving our timeshare in any way.

Always do your homework on prices with other agencies before contacting Interval. If you plan to exchange your week for the cruise, you have to do it before depositing it. They also only give you a day or so to make up your mind about accepting the rate they quote you. We have priced a cruise with them as an exchange before, and the price was not less than other agencies and not worth giving up the week.

Thanks for the advice, it seems as if II offers great land vacations, but if you want to go be on the water, online sites seems to do the trick.

Just an an aside, anyone know a good place to find last minute discounts.

Question --if you exchange a timeshare wouldn't that subtract the timeshare vacation value from the initial cruise price?

The process for exchanging with Interval International is a relatively straightforward and simple: To use the Interval International timeshare program, the timeshare you own must be affiliated with Interval International.

In fact, many of the world’s top timeshare developers, including Hyatt and Marriott, include Interval International timesharing opportunities in their membership fees.

With an Interval International timeshare membership, each member is given a timeshare week. This week can then be deposited in order to take advantage of Interval’s incredible timeshare network and exchanged for time spent at another resort, allowing members the unique opportunity of spending time in a new resort without purchasing multiple weeks.

To extend your membership on a week-by-week basis, it will cost you $39 per week. However, your expiration date does not change. If you do not renew your membership with 120 days of your expiration date, you may be required to pay a $200 reinstatement fee.

How it Works: Exchanging with Interval International

In addition to your membership fee, regardless of level, you must pay an exchange fee of $189 per week. Members using IntervalWorld.com to book their exchange are eligible for a $10 discount.

Knowing how to successfully and deftly work an exchange with any timeshare exchange program is vital to maximizing the potential of your timeshare ownership.

Think of exchanging skills as a toolset that you can use to either bargain for vacations in various, high-demand locations or during sought-after high-season times. You have several options you can use to get the location, week and size of unit you desire.

  • Request First

This feature offered by Interval International enables you to request high-demand weeks without taking your points until a reservation is confirmed. If you cancel your request, your points are returned to you.

  • Deposit First

This option requires you to deposit your points before an exchange request can be made. When using request first or deposit first you only have 24 hours to cancel the trade, or you will incur additional fees.

This program enables owners to exchange properties before the general population can make reservations. Hyatt owners cannot use the Interval International exchange program. Instead, they get preference through the Hyatt system.

For a small fee of $54, this new tool enables Interval International members to exchange their confirmed week up to three additional times in order to get the best week for their circumstances. The unit must be the same size, or you may incur an additional fee.

The Choose2 program allows you to gain an extra week of vacation after you exchange into a week. Only some resorts and destinations participate, so the availability is limited and confined to low season destinations, but if you have some flexibility, this is a lucrative opportunity for additional vacation time.

Things to Remember

  • Interval International Check-In

With the exception of ShortStay exchanges, check-in is typically Thursday, Friday, Saturday or Sunday.

  • Upgrade Fees

To reserve a larger unit than the one you have deposited, you will have to pay an upgrade fee of $99 per upgrade step, so a studio to a 1-bedroom unit is $99. Gold members pay $79, and Platinum members pay $59.

  • Exchanging Rental Weeks

Renting your exchange week is strictly forbidden, and your membership can be terminated if you are caught. You can use a guest certificate to gift your exchange week to another individual.

Interval International Launches Inaugural Member Cruise

Miami, FL, January 22, 2020 – Interval International, a prominent worldwide provider of vacation services, recently hosted its first-ever Member Cruise aboard Symphony of the Seas , one of Royal Caribbean’s newest ships. The seven-night eastern Caribbean sailing catered to nearly 200 participants, who enjoyed a variety of curated activities that ranged from members-only shore excursions to special giveaways. A team of Interval Ambassadors was onboard to share the experience with members and host the private events.

“This wonderful cruise event was an opportunity to build member affinity and a more connected Interval community,” said Sharon Freed. “We know how much our members love to cruise, and it was very rewarding to see how much they also valued the chance to connect with fellow members.”

As part of their booking, members were able to participate in various exclusive events and activities, including two private cocktail receptions, member games and nightly group dining, as well as choose from one of three members-only shore excursions to St. Maarten; San Juan, Puerto Rico; and Stingray City in Bahamas. In addition, they received goodie bags, special giveaways, and an onboard credit per stateroom. Upon arrival, members were greeted by the Interval Ambassadors, who were available to assist with questions throughout the week.

Interval International Membership: A 101 Guide from a Happy Timeshare Owner

Are you a timeshare owner at a resort that qualifies for an Interval International Membership, but you haven’t joined it to trade your timeshare for other vacations? Well, this introductory article is for you! 

Interval International & The Timeshare World

Without getting into the whole history of the timeshare industry, I think it is important to know about the biggest timeshare corporations and how these conglomerates create a separation of church and state for owners and users of timeshares and an exchange network.

Personally, I think it is crucial to know all you can about this world before buying a timeshare or spending money on an Interval International Membership. But, unfortunately, I find that many disappointed owners are learning key pieces of information after the fact, and I’m on a mission to help people learn more before opening their wallets. For example, Did you know that timeshare resort chains actually own the two largest timeshare exchange companies? 

RCI is owned by Wyndham Destinations, now named Travel + Leisure

Travel +Leisure (yes, the magazine brand!) also owns

  • Margaritaville Vacation Club
  • Shell Vacation Club
  • 7 Across (the exchange company formerly known as DAE)

Why This Is Important

As a timeshare owner who wants to trade her timeshare for vacations in other destinations knowing that Wyndham Vacation Resorts will never be available in Interval International and Marriott Vacation Club resorts will never be available in RCI is a really critical piece of information. See, those large corporations will never deposit weeks from their owned brands into the other’s exchange network. 

So as a timeshare owner who currently owns a resort that is only affiliated with RCI and no other timeshare exchange company, if I want to stay at a Marriott Timeshare resort, I will have to rent it.

When we recently traveled to Fort Lauderdale for a family event and stayed at Marriott Beachplace Towers , I didn’t have access to the exchange network that Marriott Vacation Club uses.  And since the resort was sold out, the only way for me to secure a room for our specific dates was by renting from another owner on Redweek.com .

But if I had specifically bought a vacation ownership to trade it into Marriott ones but later discovered that the resort only plays with RCI and never Interval International, boy would I be disappointed. That’s just another reason to take your time buying a timeshare.

Think about your travel bucket list, travel preference, and lifestyle before you buy! It’s a buyer’s world in timeshares, so those timeshare deals will still be there when you are ready! Don’t let any timeshare salesperson rush you so they can make their sales quota. 

Now that my mini-rant is over let’s get to learning about Interval International!

Who is Interval International?

Interval International is the second-largest timeshare exchange company, with RCI being the largest. Created in 1976, they now have 2 million members with 3,200 affiliated resorts in 80+ countries.

How Much Does a Membership Cost?

If you own a timeshare that is affiliated with Interval International, you are eligible for membership. While some timeshare exchange companies out there let anyone join and rent their leftover rooms at the last minute, Interval International is not one of them. So if you hear otherwise from someone, that’s a major red flag that they don’t know what they are talking about.

It is a Week for Week Exchange, Unlike RCI

For those familiar with RCI, Interval International also has two programs –  One for Points Members and One for Weeks Members. However, there is one MAJOR difference between RCI and Interval International, which has both its pros and cons. You see, for people in the Interval International week’s program, it’s a week for week exchange.

That’s right! So if you can work the system, there are ways to take a “cheap” vacation ownership week and trade it up for a week at a premium resort, like Marriott. That’s why we personally are interested in finding that unicorn timeshare to buy so that we can do these trade-ups!

But I won’t get too ahead of myself in this introductory article. Instead, let’s take a look at the basics behind an Interval International Membership and exchanging your timeshare week. 

The Deposit First Feature

When you know you want to use your week for a vacation at another resort, go ahead and deposit your timeshare ownership into the system. Interval International gives you a very generous four-year window, allowing you to book an exchange two years before your week all the way to two years after the check-in day of that week.

With this type of “instant” exchange, you’re able to exchange into larger sizer units if they are available. So when searching it is possible that your studio unit will match with a 2 bedroom unit. 

Their Request First Feature

Another difference between an Interval International Membership and RCI is that II allows you to request an exchange first without depositing your vacation ownership week. This way, you don’t have to “give up” your home resort week until you see what you can get on exchange.

However, with this feature, you can only secure exchanges for the two years before your check-in date and only search for a unit size similar to yours (or less). A way around this is to request an exchange into a resort which only has 2 bedroom units.  Here’s how to place this type of request with your Interval International Membership.

More Important Details

If you’re thinking about getting an Interval International Membership, here’s a couple more things to know to help you make a decision about this part of the vacation ownership world.

Disney Vacation Club Now Exclusively Trades with Interval International

After many years of an exclusive agreement with RCI, Disney Vacation Club has returned to Interval International for timeshare exchange. So if you want to exchange your timeshare into the upscale, perk-filled Disney timeshares, then you have to do it with Interval International.

“For the past 30 years, Disney Vacation Club has been a leader in vacation ownership and has provided its Members and guests with exceptional experiences. We are delighted to welcome them back to the Interval network,” said  Jeanette Marbert , president of Interval International. 

Last-Minute Plans

For people who want to reserve a room on short notice, you can do that with Interval International’s Flexchange feature, which offers exchanges 59 days to 24 hours before the check-in date. On the flip side, you can also deposit your week with II in their Flex Deposit program.

You can deposit your week as late as 14 days before check-in as a way to still get value from your week, even if plans change . However, deposits made between 59 days and 14 days from the check-in date are considered “late deposits” and can only be used for exchanges 59 days in advance. 

Regional Restrictions on Timeshare Exchanges

Just like RCI, there are some regional restrictions that limit trades within certain cities. For example, if you own a timeshare in Orlando, you might be restricted from trading to another resort in Orlando. So if you’re planning on getting an Interval International membership as part of a way to travel hack, please look into regional restrictions first. 

What’s an Accommodation Certificate?

If you’ve seen this phrase thrown around but don’t know exactly what it is, Interval International offers these “bonus” weeks to people who have high-value or high-demand timeshare ownership weeks. In a system where a week is straight-up exchanged for another, you can see why owners of high-value weeks might not even deposit theirs. So as a way to get those weeks into the exchange pool, Interval International offers these extra weeks as a motivation. 

However, your exchange options with an Accommodation Certificate will be limited and are, in essence, the leftovers of what other people didn’t take 59 days from the check-in date.  That doesn’t mean there isn’t great stuff in there, but expectations need to be managed.

If you can travel at the last minute, during off-season or shoulder season, there are some scores to be had! These certificates are good for one year and aren’t free to use. The redemption costs are from $229 to $329 based on the size of the room. 

Their System has Trading Filters

Another big difference between RCI and Interval International Membership is that their system has trading filters baked into their website, restricting what you see when you’re searching. I know this sounds radical, but in their world of “ a week for a week,” they put some controls in place so that people weren’t dramatically trading down in quality of resorts. 

So if you have a Gold Crown or another high-quality week, the website won’t even show you lower-quality resorts.  So if you are open to widening your search, you’ll need to call in and have a representative walk you through all the options for an exchange. 

Getaways: Cash-Only Deals

If you’ve heard people talk about Interval International Getaways, these are cash-only last-minute deals on still available resort rooms. You could purchase an Interval International Membership for this feature alone! In fact, some people get an Interval International membership and never deposit their home resort week.

They either use it or rent it and only use their Interval International Membership to book these cash-only Getaway deals. That’s how good some of these are. You have to be able to travel at the last minute and be really flexible on the destination. Still, you can score really upscale resort rooms in Hawaii, Mexico, the Caribbean, Europe, Florida, and California by learning and using this one feature of Interval International membership.

Using Interval for Cruise Exchange

Can someone who has done this tell me how this works. My question is does it cover entire basic cost of a cruise or just a portion of cruise cost? If a portion how is that figured? Thanks, Suzanne

Interactions Between Travel Vaccines & Drugs

Vaccine–Vaccine Interactions

Travel vaccines & drugs, antimalarial drugs, drugs used for travel to high elevations, hiv medications, herbal & nutritional supplements.

During pretravel consultations, travel health providers must consider potential interactions between vaccines and medications, including those already taken by the traveler. A study by S. Steinlauf et al. identified potential drug–drug interactions with travel-related medications in 45% of travelers taking medications for chronic conditions; 3.5% of these interactions were potentially serious.

Most common vaccines can be given safely and effectively at the same visit, at separate injection sites, without impairing antibody response or increasing rates of adverse reactions. However, certain vaccines, including pneumococcal and meningococcal vaccines and live virus vaccines, require appropriate spacing; further information about vaccine–vaccine interactions is found in Sec. 2, Ch. 3, Vaccination & Immunoprophylaxis—General Principles .

Live Attenuated Oral Typhoid & Cholera Vaccines

Live attenuated vaccines generally should be avoided in immunocompromised travelers, including those taking antimetabolites, calcineurin inhibitors, cytotoxic agents, immunomodulators, and high-dose steroids (see Table 3-04 ).

Chloroquine and atovaquone-proguanil at doses used for malaria chemoprophylaxis can be given concurrently with oral typhoid vaccine. Data from an older formulation of the CVD 103-HgR oral cholera vaccine suggest that the immune response to the vaccine might be diminished when given concomitantly with chloroquine. Administer live attenuated oral cholera vaccine ≥10 days before beginning antimalarial prophylaxis with chloroquine. A study in children using oral cholera vaccine suggested no decrease in immunogenicity when given with atovaquone-proguanil.

Antimicrobial Agents

Antimicrobial agents can be active against the vaccine strains in the oral typhoid and cholera vaccines and might prevent adequate immune response to these vaccines. Therefore, delay vaccination with oral typhoid vaccine by >72 hours and delay oral cholera vaccine by >14 days after administration of antimicrobial agents. Parenteral typhoid vaccine is an alternative to the oral typhoid vaccine for travelers who have recently received antibiotics.

Rabies Vaccine

Concomitant use of chloroquine can reduce the antibody response to intradermal rabies vaccine administered as a preexposure vaccination. Use the intramuscular route for people taking chloroquine concurrently. Intradermal administration of rabies vaccine is not currently approved for use in the United States (see Sec. 5, Part 2, Ch. 19, . . . perspectives: Rabies Immunization ).

Any time a new medication is prescribed, including antimalarial drugs, check for known or possible drug interactions (see Table 2-05 ) and inform the traveler of potential risks. Online clinical decision support tools (e.g., Micromedex) provide searchable databases of drug interactions.

Atovaquone-Proguanil

Antibiotics.

Rifabutin, rifampin, and tetracycline might reduce plasma concentrations of atovaquone and should not be used concurrently with atovaquone-proguanil.

  • Anticoagulants

Patients on warfarin might need to reduce their anticoagulant dose or monitor their prothrombin time more closely while taking atovaquone-proguanil, although coadministration of these drugs is not contraindicated. The use of novel oral anticoagulants, including dabigatran, rivaroxaban, and apixaban, is not expected to cause significant interactions, and their use has been suggested as an alternative for patients in need of anticoagulation.

Antiemetics

Metoclopramide can reduce bioavailability of atovaquone; unless no other antiemetics are available, this antiemetic should not be used to treat vomiting associated with the use of atovaquone at treatment doses.

Antihistamines

Travelers taking atovaquone-proguanil for malaria prophylaxis should avoid using cimetidine (an H2 receptor antagonist) because this medication interferes with proguanil metabolism.

Atovaquone-proguanil might interact with the antiretroviral protease inhibitors atazanavir, darunavir, indinavir, lopinavir, and ritonavir, or the nonnucleoside reverse transcriptase inhibitors (NNRTIs) efavirenz, etravirine, and nevirapine, resulting in decreased levels of atovaquone-proguanil. For travelers taking any of these medications, consider alternative malaria chemoprophylaxis .

Selective Serotonin Reuptake Inhibitors

Fluvoxamine interferes with the metabolism of proguanil; consider an alternative antimalarial prophylaxis to atovaquone-proguanil for travelers taking this selective serotonin reuptake inhibitor (SSRI).

Chloroquine

Antacids & Antidiarrheals

Chloroquine absorption might be reduced by antacids or kaolin; travelers should wait ≥4 hours between doses of these medications.

Chloroquine inhibits bioavailability of ampicillin, and travelers should wait ≥2 hours between doses of these medications. Chloroquine should not be coadministered with either clarithromycin or erythromycin; azithromycin is a suggested alternative . Chloroquine also reportedly decreases the bioavailability of ciprofloxacin.

Concomitant use of cimetidine and chloroquine should be avoided because cimetidine can inhibit the metabolism of chloroquine and increase drug levels.

CYP2D6 Enzyme Substrates

Chloroquine is a CYP2D6 enzyme inhibitor. Monitor patients taking chloroquine concomitantly with other substrates of this enzyme (e.g., flecainide, fluoxetine, metoprolol, paroxetine, propranolol) for side effects.

CYP3A4 Enzyme Inhibitors

CYP3A4 inhibitors (e.g., erythromycin, ketoconazole, ritonavir) can increase chloroquine levels; concomitant use should be avoided.

Chloroquine can increase digoxin levels; additional monitoring is warranted.

Immunosuppressants

Chloroquine decreases the bioavailability of methotrexate. Chloroquine also can cause increased levels of calcineurin inhibitors; use caution when prescribing chloroquine to travelers taking these agents.

QT-Prolonging Agents

Avoid prescribing chloroquine to anyone taking other QT-prolonging agents (e.g., amiodarone, lumefantrine, sotalol); when taken in combination, chloroquine might increase the risk for prolonged QTc interval. In addition, the antiretroviral rilpivirine has also been shown to prolong QTc, and clinicians should avoid coadministration with chloroquine.

Doxycycline

Antacids, Bismuth Subsalicylate, Iron

Absorption of tetracyclines might be impaired by aluminum-, calcium-, or magnesium-containing antacids, bismuth subsalicylate, and preparations containing iron; advise patients not to take these preparations within 3 hours of taking doxycycline.

Doxycycline can interfere with the bactericidal activity of penicillin; thus, in general, clinicians should not prescribe these drugs together. Coadministration of doxycycline with rifabutin or rifampin can lower doxycycline levels; monitor doxycycline efficacy closely or consider alternative therapy.

Patients on warfarin might need to reduce their anticoagulant dose while taking doxycycline because of its ability to depress plasma prothrombin activity.

Anticonvulsants

Barbiturates, carbamazepine, and phenytoin can decrease the half-life of doxycycline.

Antiretrovirals

Doxycycline has no known interaction with antiretroviral agents.

Concurrent use of doxycycline and calcineurin inhibitors or mTOR inhibitors (sirolimus) can cause increased levels of these immunosuppressant drugs.

Mefloquine can interact with several categories of drugs, including anticonvulsants, other antimalarial drugs, and drugs that alter cardiac conduction.

Mefloquine can lower plasma levels of several anticonvulsant medications, including carbamazepine, phenobarbital, phenytoin, and valproic acid; avoid concurrent use of mefloquine with these agents.

Mefloquine is associated with increased toxicities of the antimalarial drug lumefantrine, which is available in the United States in fixed combination to treat people with uncomplicated Plasmodium falciparum malaria. The combination of mefloquine and lumefantrine can cause potentially fatal QTc interval prolongation. Lumefantrine should therefore be avoided or used with caution in patients taking mefloquine prophylaxis.

CYP3A4 Enzyme Inducers

CYP3A4 inducers include medications used to treat HIV or HIV-associated infections (e.g., efavirenz, etravirine, nevirapine, rifabutin) and tuberculosis (rifampin). St. John’s wort and glucocorticoids are also CYP3A4 inducers. All these drugs (rifabutin and rifampin, in particular) can decrease plasma concentrations of mefloquine, thereby reducing its efficacy as an antimalarial drug.

Potent CYP3A4 inhibitors (e.g., antiretroviral protease inhibitors, atazanavir, cobicistat [available in combination with elvitegravir], darunavir, lopinavir, ritonavir, saquinavir); azole antifungals (itraconazole, ketoconazole, posaconazole, voriconazole); macrolide antibiotics (azithromycin, clarithromycin, erythromycin); and SSRIs (fluoxetine, fluvoxamine, sertraline), can increase levels of mefloquine and thus increase the risk for QT prolongation.

Although no conclusive data are available regarding coadministration of mefloquine and other drugs that can affect cardiac conduction, avoid mefloquine use, or use it with caution, in patients taking antiarrhythmic or β-blocking agents, antihistamines (H1 receptor antagonists), calcium channel receptor antagonists, phenothiazines, SSRIs, or tricyclic antidepressants.

Concomitant use of mefloquine can cause increased levels of calcineurin inhibitors and mTOR inhibitors (cyclosporine A, sirolimus, tacrolimus).

Anti-Hepatitis C Virus Protease Inhibitors

Avoid concurrent use of mefloquine and direct-acting protease inhibitors (boceprevir and telaprevir) used to treat hepatitis C. Newer direct-acting protease inhibitors (grazoprevir, paritaprevir, simeprevir) are believed to be associated with fewer drug–drug interactions, but safety data are lacking; consider alternatives to mefloquine pending additional data.

Psychiatric Medications

Avoid prescribing mefloquine to travelers with a history of mood disorders or psychiatric disease; this information is included in the US Food and Drug Administration boxed warning for mefloquine.

Drugs Used to Treat Travelers’ Diarrhea

Antimicrobials commonly prescribed as treatment for travelers’ diarrhea have the potential for interacting with several different classes of drugs ( Table 2-06 ). As mentioned previously, online clinical decision support tools provide searchable databases that can help identify interactions with medications a person may already be taking.

Azithromycin

Increased anticoagulant effects have been noted when azithromycin is used with warfarin; monitor prothrombin time for people taking these drugs concomitantly.

Because additive QTc prolongation can occur when azithromycin is used with the antimalarial artemether, avoid concomitant therapy.

Drug interactions have been reported with the macrolide antibiotics, clarithromycin and erythromycin; antiretroviral protease inhibitors; and the NNRTIs, efavirenz and nevirapine. Concomitant use of azithromycin and these drugs can increase the risk of QTc prolongation, but a short treatment course is not contraindicated for those without an underlying cardiac abnormality. When azithromycin is used with the protease inhibitor nelfinavir, advise patients about possible drug interactions.

Concurrent use of macrolides with calcineurin inhibitors can cause increased levels of drugs belonging to this class of immunosuppressants.

Fluoroquinolones

Concurrent administration of ciprofloxacin and antacids that contain magnesium or aluminum hydroxide can reduce bioavailability of ciprofloxacin.

An increase in the international normalized ratio (INR) has been reported when levofloxacin and warfarin are used concurrently.

Asthma Medication

Ciprofloxacin decreases clearance of theophylline and caffeine; clinicians should monitor theophylline levels when ciprofloxacin is used concurrently.

Immunosuppresants

Fluoroquinolones can increase levels of calcineurin inhibitors, and doses should be adjusted for renal function.

Sildenafil should not be used by patients taking ciprofloxacin; concomitant use is associated with increased rates of adverse effects. Ciprofloxacin and other fluoroquinolones should not be used in patients taking tizanidine.

Rifamycin SV

No clinical drug interactions have been studied. Because of minimal systemic rifamycin concentrations observed after the recommended dose, clinically relevant drug interactions are not expected.

Rifaximin is not absorbed in appreciable amounts by intact bowel, and no clinically significant drug interactions have been reported to date with rifaximin except for minor changes in INR when used concurrently with warfarin.

Before prescribing the carbonic anhydrase inhibitor, acetazolamide, to those planning high elevation travel, carefully review with them the complete list of medications they are already taking ( Table 2-07 ).

Acetazolamide

Acetaminophen & Diclofenac Sodium

Acetaminophen and diclofenac sodium form complex bonds with acetazolamide in the stomach’s acidic environment, impairing absorption. Neither agent should be taken within 30 minutes of acetazolamide. Patients taking acetazolamide also can experience decreased excretion of anticholinergics, dextroamphetamine, ephedrine, mecamylamine, mexiletine, and quinidine.

Acetazolamide should not be given to patients taking the anticonvulsant topiramate because concurrent use is associated with toxicity.

Barbiturates & Salicylates

Acetazolamide causes alkaline urine, which can increase the rate of excretion of barbiturates and salicylates and could cause salicylate toxicity, particularly in patients taking a high dose of aspirin.

  • Corticosteroids

Hypokalemia caused by corticosteroids could occur when used concurrently with acetazolamide.

Diabetes Medications

Use caution when concurrently administering metformin and acetazolamide because of increased risk for lactic acidosis.

Monitor cyclosporine, sirolimus, and tacrolimus more closely when given with acetazolamide.

Dexamethasone

Using dexamethasone to treat altitude illness can be lifesaving. Dexamethasone interacts with several classes of drugs, however, including: anticholinesterases, anticoagulants, digitalis preparations, hypoglycemic agents, isoniazid, macrolide antibiotics, oral contraceptives, and phenytoin.

Patients with HIV require additional consideration in the pretravel consultation (see Sec. 3, Ch. 1, Immunocompromised Travelers ). A study from Europe showed that ≤29% of HIV-positive travelers disclose their disease and medication status when seeking pretravel advice. Antiretroviral medications have multiple drug interactions, especially through their activation or inhibition of the CYP3A4 and CYP2D6 enzymes.

Several instances of antimalarial prophylaxis and treatment failure in patients taking protease inhibitors and both nucleoside and NNRTIs have been reported. By contrast, entry and integrase inhibitors are not a common cause of drug–drug interactions with commonly administered travel-related medications. Several potential interactions are listed above, and 2 excellent resources for HIV medication interactions can be found at  HIV Drug Interactions and HIV.gov . HIV preexposure prophylaxis with emtricitabine/tenofovir is not a contraindication for any of the commonly used travel-related medications.

Up to 30% of travelers take herbal or nutritional supplements. Many travelers consider them to be of no clinical relevance and might not disclose their use unless specifically asked during the pretravel consultation. Clinicians should give special attention to supplements that activate or inhibit CYP2D6 or CYP3A4 enzymes (e.g., ginseng, grapefruit extract, hypericum, St. John’s wort). Advise patients against coadministration of herbal and nutritional supplements with medications that are substrates for CYP2D6 or 3A4 enzymes, including chloroquine, macrolides, and mefloquine.
