Expectations For Your Therapy Journey: What Does Progress Look Like?

Whether you’re considering participating in therapy or have just begun the counseling process, you may be wondering what to expect. Everyone’s journey with therapy can look different, and it may take varying amounts of time to start making progress and positive changes in your life. While therapy can be a lengthy procedure, the process is not all stress and hardship. It may be helpful to understand the phases of therapy you may progress through as you delve deeper into self-knowledge and self-healing. Keep reading to learn more about the therapeutic process and what you may be able to expect along the way.

Initiating the journey

Since therapy is a personal journey, it’s possible that your experience could be nonlinear or complex. When participating in therapy, it can be important to trust the process while maintaining open and honest communication with your therapist regarding your feelings about your treatment plan. The two of you may find a way to collaborate on creating a positive therapy journey together.

When you begin therapy, you and your therapist will often focus on building a stronger relationship. You may not feel comfortable starting the healing process right away, as your therapist is a stranger to you, and you might need some time before you are okay with being fully open and vulnerable with them. You may start by talking about your thoughts, feelings, and relationships in your life while your therapist listens and validates your experiences. This validation can help to create a sense of safety between the two of you, which then may lead you to have deeper, more closely held emotions that could lead to greater therapeutic insight.

At this phase in the journey, counseling may slightly reduce the mental health symptoms you have been experiencing. This shift is likely due to the benefits of simply talking to someone else about what you have been going through. However, it is important to note that the real gains are likely still yet to be made, and it can be crucial to stick with and trust the therapeutic process at this phase. 

In other cases, counseling may cause slightly worsening symptoms at the start of the process, as opening up to someone may reveal deep and intense emotions. This doesn’t mean therapy isn’t working to treat the symptoms of the mental illness; rather, you may just need more time to see its full effect.

Self-reflection

Once you and your therapist have developed a connection , you can start to approach a central question of your counseling journey: what challenges are you hoping to address? You may have started sessions with an idea of what you would like to work on, such as tackling relationship troubles, improving emotional hardships, building greater self-esteem, or deepening your relationship with yourself. Even if you already have a sense of what you’re hoping to gain from therapy, working through the process may help illuminate potential underlying concerns that are contributing to the challenges you are experiencing in your life.

On the other hand, you may have no tangible feeling of what you want from therapy. Maybe you started your journey because you don’t feel like you’re the best version of yourself that you could be, but you don’t know what that version looks like. Maybe you have a general sense of stagnation or frustration in your life, and are unsure how to address it. It is perfectly okay and understandable not to know what you hope to get out of therapy – that too can be part of the journey. Your therapist can help you identify your idea of progress in personal development and how you can start moving in that direction. 

Goals for your experience

This phase can be the most exciting component of the journey. By this point, it’s likely that you and your therapist have built a  strong therapeutic relationship and have identified a concern you want to work on – now you can develop a plan to address that concern and engage in personal growth.

You and your therapist can work as a team to create this plan. You may start by discussing your value systems and making sure you develop a plan that is authentic to you and supportive of your specific needs while you are tackling your goals. These goals may include finding a resolution to an ongoing conflict or challenge in your life or reducing symptoms of a mental health condition. It could also mean learning how to shift behavioral patterns, such as feeling empowered enough to stand up for yourself, integrating mindfulness into your daily life, prioritizing your own well-being, or something else. Having clearly stated goals can help you identify the progress you are making on your individual journey. 

Exploration can be the most difficult stage of this process. As you and your therapist embark on your treatment plan, you may uncover forgotten memories, emotional challenges, and hidden thoughts and feelings. You might begin to realize that certain belief systems are not serving you any longer and start to reassess your understanding of other people, including close relationships in your life. As you re-examine your past experiences, you might even learn that you have survived trauma. Exploring different parts of your mind and consciousness that you have previously been disconnected from can be complicated and potentially messy.

This phase of your therapy journey may be emotionally distressing, but it may be helpful to know that it is all part of making important self-progress. Integrating counseling lessons into your everyday life

A therapist can be a beneficial resource for working through challenging times, no matter what stage of life you’re in.

Journey Clinical raises $3M to allow psychotherapists to offer psychedelic therapies

Psychedelics companies are all the rage right now. Compass Pathways is working with the magic mushroom compound psilocybin to treat depression. It’s has raised $290 million in total. Atai Life Sciences — backed by PayPal co-founder Peter Thiel — brought in $258 million from its IPO. In the tech space, this has not gone unnoticed and the same business models that have been used in other platforms for health and wellness startups are coming to psychedelics.

The latest is Journey Clinical , based out of NYC, which has raised a $3 million seed round led by San Francisco VC firm Fifty Years . Also participating were Neo Kuma Ventures, Palo Santo, PsyMed Ventures, Lionheart Ventures, Christina Sass co-founder of Andela, ​​Edvard Engesæth, MD co-founder of Nurx and, Hans Gangeskar co-founder of Nurx.

Journey joins other startups in the space looking at psychedelic-assisted psychotherapy, where ketamine is used to treat depression, anxiety, PTSD, and trauma, known as ketamine-assisted psychotherapy (KAP). Miami-based startup NUE Life Health raised a $3.3 million seed round for the same purpose back in June. There is also Field Trip and Mindbloom playing in this space.

These startups are pushing at an open door on depression and anxiety. Pre-COVID-19, the National Center for Health Statistics estimated some 50 million Americans were fighting the afflictions. The pandemic has of course exacerbated this issue, with those figures doubling, by some estimates.

It’s still an early market. Journey says the market landscape for legal psychedelic therapies is very disparate, with over a million licensed mental health professionals lacking the infrastructure to offer these treatments as they lack access to prescribing clinicians. On the flip side, patients struggle to find psychotherapists who can prescribe psychedelics as treatment.

Journey says it has a “decentralized clinic model” that allows psychotherapists to offer legal psychedelic therapy treatments in their practice, starting with ketamine. The way it works is that Journey takes care of the pharmacology side, while psychotherapists that sign up to the platform take care of the psychotherapy of the patient. The treatment plans are then customized to meet the patient’s needs.

Jonathan Sabbagh, co-founder and CEO, was previously diagnosed with PTSD, but after discovering psychedelics, he went back to school to study clinical psychology, and went on to co-found Journey. He said: “We are on the verge of a paradigm shift in the field of mental health. Psychedelic-assisted psychotherapies are one of the most promising new means of treatment available; they will allow clinicians to tackle the growing global mental health crisis we are facing.”

Speaking to TechCrunch he added: “When we asked what was the main bottleneck for therapists to offer KAP to their patients, the #1 response was access to a prescribing doctor. Our alpha test group confirmed that guaranteeing access to a trained medical team and building a robust care management system would solve an essential bottleneck of mainstream adoption for KAP.”

Journey has two revenue streams. Psychotherapists pay them a $200 monthly membership fee which gives them access to a number of services including and access to the prescriber, an EHR (achieved through a white label), a KAP training (training materials created by a specialized training company), a profile on Journey’s directory, and a community of peers. Patients pay journey for medical services. They pay $250 for the intake consultation and $150 for follow-up consultations.

Ela Madej, Founding Partner at Fifty Years, said: “I dream of a world where those of us affected by trauma, anxiety, or depression don’t have to fall into learned helplessness. We’re lucky that powerful psychedelic treatments for the mind exist, but they need to be delivered responsibly, with proper screening, protocols, and follow-up. We’ve been incredibly impressed by Journey Clinical’s ambitious plan to empower psychotherapists to better treat their existing patients.”

The team also comprises Kyle Lapidus MD, Ph.D., who has over 20 years as a board-certified psychiatrist and has extensive experience working with ketamine; and Brigitte Gordon DNP a professor at Columbia University and also works for the Multidisciplinary Association for Psychedelic Studies (MAPS.)

Dude . . . the Facebook ad was actually legit!

Impressive organization and coordination of the group meetings from the company. Connecting with other people in a group sized like this is great and our therapist seems to have an awesome insight for when to linger on a topic and when to keep things moving. He's both a great facilitator and an insightful companion for the group.

I’m really enjoying my Journey! It’s great to have a safe space each week to just know I’m not alone. Sometimes the sessions are more involved, and sometimes we just play Discord games and vibe.

A great experience

I absolutely love Rachel (my journey guide) She is great, so friendly and is so supportive. I think it's just great that they offer this service because it's much more anxiety friendly than going into a psychologist office. I also love the group aspect because it's a chance to make new friends, play some new video games and for someone quiet like me, other people to lead the conversation where I can chime in with my experiences and everything. It's just so much more comfortable than any office I've been into.

They kicked me out of their discord bc…

They kicked me out of their discord bc I told them I was disabled and couldn't afford every single week. So, they refunded me and basically told me to come back when I can afford it. Do not recommend. Discrimination at its finest. Edit: they don't even deny that they kicked me out for this reason. Actually amazing. Screw yall if you think discrimination is okay and trying to justify it. Bye.

Low Pressure Safe Space

I just really enjoy the low pressure safe space that is created in these sessions. I feel like I can show up however I am feeling that day and I'll be welcomed without judgement. The topics we discuss and insights from the facilitator and fellow journeyers can be really helpful.

Wonderful Little Community

It’s been so great to have an outlet every week to talk about life, mental health struggles, and anything else with people who are going through something similar and who care. Our guide is really great and always offers a kind perspective. We are a group of 4 usually and it’s just enough time for everyone to share and have the other group member and guide offer advice and support. Also we play cute games together sometimes :)

Always making people feel welcome and…

Always making people feel welcome and not forgotten about. Theres a lot of things I forget I brought up previously or that we moved on from for a moment and then circled back around to as opposed to just ending the topic unresolved. Hero Journey Club has changed my life!

Hero Journey Club has been an amazing experience. I have always wanted to do group therapy and this has finally given me my chance. The group I am in is amazing and I feel I can talk about my feelings and thoughts in a non-judgmental place while gaming at the same time. As the weeks go on I am learning more about my group and I may have only been in the group for about a month and a half but it’s really changing and improving my life. The counselor really is so caring and compassionate and I like the format. I highly recommend this to anyone on the fence about therapy. I like the fact it’s just one easy flat rate per week and insurance is not involved making it much more accessible. Thank you, Hero Journey Club!

Connection and Reflection while Gaming

It's so nice to be able to connect with others. It's easier to open up when you see and hear that others are going through similar situations. Sometimes others bring up topics that you haven't even realized were issue in your own life and it helps things "click" Playing games also helps relax and make a cozy setting. It's nice to get to know each other better each session

Gentle coaching in a relaxed atmosphere.

The therapist and group that I was assigned to when I joined Hero Journey Club all share similar perspective and experience to me, which makes me feel welcome and heard every time I attend. I have never been one for one-on-one therapy because I find talking about my feelings a little uncomfortable, but it becomes much more bearable with a group to bounce my thoughts off of. Being able to indulge in a bit of gameplay while I unwind makes Hero Journey Club worth it each time.

My therapist ended up being a pedophile.

Edit: Something else that hasn’t been done by this company is a formal apology to all affected members that states they understand the trauma this has potentially left them. Multiple group members have expressed to each other that we are really torn up about this. They really can’t think that they’d vaguely touch on an arrest and us not dig to find out what it was. I hate that we were left with more trauma than what we came with. My therapist was arrested on two counts of possession of child pornography on May 28th, 2023. He started working for HJC in August of 2023. Why this wasn’t caught, I don’t know. I am disgusted that I spent 8 months of my life telling him deeply personal things. I’m not sure HJC vets their guides appropriately.

The charges do not involve any of their work with Hero Journey Club, however as soon as we learned of this matter we took immediate action: we terminated their role with us, contacted our members they were working with, and conducted an exhaustive audit of their company communications, which showed no evidence that any wrongdoing occurred through our platform. All Hero Journey Club Guides undergo extensive background checks conducted by an industry-leading provider to the full extent allowed by law, and we actively monitor to ensure there are no violations against their license or other credentials. Unfortunately, existing background check processes do not report on arrests prior to convictions in some states. Given this, we are working to strengthen our vetting processes to further ensure the safety and wellbeing of our community members. I wasn't sure what to expect at first...

I wasn't sure what to expect at first, but after a few months, I can say that not only does this work but group therapy + gaming is something special. Having a group to be vulnerable with while also all being able to relate because of our shared hobby of gaming has been insanely amazing! So far I have been enjoying what this company offers me in regards to social ability but also excited to see what the future holds as well! Being vulnerable in a group setting isn't easy but something about this culture makes it a little less scary. Please keep up the amazing work!!

The Hero’s Journey (Campbell, 1949) is a common narrative structure throughout ancient mythology and modern storytelling which describes the transformation of a hero through a journey of trials and represents the experience of psychological healing throughout various cultures. Because of the ancient and common nature of this structure and the psychological importance of personal narrative and storying even from an early age (Boyd, 2018; Klees, 2016), this thesis hypothesizes that a client’s experience throughout psychotherapy will follow the stages of the Hero’s Journey and that invoking this structure within the therapy room, particularly for drama therapists, will empower clients toward change and transformation. This thesis lays out the three stages of the Hero’s Journey (departure, initiation, and return) and key subsections of each stage and draws parallels with client’s progression through drama therapy and dramatic reality. The literature on psychotherapy and narrative supports the empowering structure of the Hero’s Journey and framing client’s experiences in therapy as heroic. For drama therapists, exploring the history of the Hero’s Journey grounds the work of drama therapy in the past and promotes spiritual integration into the therapy room. Though some work has been done to integrate the Hero’s Journey with certain drama therapy interventions (Klees, 2016; Minerson, 2017) the work has been limited in scope. This thesis aims to draw parallels throughout all drama therapy processes in therapy with the Hero’s Journey and to encourage therapists to promote empowerment and transformation in their clients through this narrative structure.
