The Brazilian Butt Lift (BBL) is a popular procedure to enhance the buttocks into a pleasing shape , especially in the Miami and Fort Lauderdale, FL area. The conventional BBL removes fat through liposuction from the abdomen or thighs, and it is transferred to the buttocks, improving the patient’s overall body shape. The conventional BBL may not be an option for petite women with low body fat levels without an accompanying weight gain. For women who are petite or small-framed, there may not be the excess fat available for traditional liposuction and fat transfer. Also called a “skinny BBL,” a small BBL is for individuals with limited fat stores but want to enhance their buttocks without synthetic implants. The ideal candidates for most BBL procedures have a body mass index (BMI) of 25-30 and benefit from the liposuction to narrow their waist or flatten their tummy. Candidates for a small BBL may have a BMI of less than 25 (usually less than 21) and minimal fat on the abdomen, requiring specialized and creative liposuction options to harvest the needed fat for the procedure.

BBL for Women With Small Frames

A small or skinny BBL is designed for women with athletic builds, low BMI or small frames. While there is less fat to harvest, the amount of fat needed to create the desired result is also less. A Complete Guide to a Skinny BBL Procedure

Generally, Brazilian Butt Lift procedures and butt enhancement procedures are increasing in popularity as many patients wish to enhance their curves or achieve a more hourglass figure. However, not all patients who desire a BBL are suitable candidates to undergo the procedure. In some cases, these candidates may be suitable for what is referred to as a ‘Skinny BBL’ which we will explain in this blog.

What is a Skinny BBL?

Patients who are ‘skinny’ can often have less fat around their bodies.  Since a BBL procedure uses Liposuction to harvest fat deposits from another area of the patient’s body, this can be difficult for those patients as they do not have enough fat to be harvested. Therefore, patients who cannot have fat grafted from their bodies will not qualify for a traditional Brazilian Butt Lift procedure .

Despite this, some patients who are slim or have a low BMI may still be possible candidates to undergo a traditional BBL as even though they have a low weight or BMI, they have a high enough proportion of fat on their body to be harvested by liposuction for a fat transfer. Additionally, other slim candidates may only desire a small butt enhancement and so despite having a low amount of fat deposits on their body, they can be safely taken by liposuction and the fat graft will achieve the results they desire.

Conversely, other candidates may not have enough fat to be able to undergo this type of BBL procedure and instead be more suited to undergo a Skinny BBL. A Skinny BBL uses liposuction of more areas than a traditional BBL because one or two areas of the body will not have enough fat deposits to be used for the enhancement. This means that liposuction of multiple areas will need to occur to ensure that enough fat is gathered. Alternatively, in some cases, patients may be advised to gain some weight before the procedure so that these gained fat deposits can be used to enhance the size of the buttocks.

Some patients are informed that they are not suitable to undergo a BBL procedure after having a consultation with their plastic surgeon.  For patients who have been told that they are unsuitable as they are too slim or don’t have enough fat deposits to undergo a traditional BBL procedure, there are other options, they may be suitable for a Skinny BBL. Any potential Skinny BBL patients should book their consultation with a Board-Certified Plastic Surgeon who has expertise in performing a range of BBL procedures to ensure their suitability for this surgery. Some of the factors that can make a candidate suitable for a Skinny BBL include: 

  • Candidates who are thin or ‘skinny’
  • Patients who have a low BMI of less than 25
  • Patients who are ‘too skinny’ for a traditional BBL can often be suitable candidates for a Skinny BBL 
  • Are in good physical health
  • Do not smoke

However, some patients may not be suitable even for a Skinny BBL as they do not have enough fat deposits across their whole body to be harvested for their buttock enhancement. Additionally, some of these patients may not want to gain weight for the procedure or may physically struggle to do so. Therefore, some patients who are unsuitable for a traditional BBL may also be unsuitable for a Skinny BBL. This is why it is extremely important that all patients interested in a Skinny BBL book a consultation with at least one Board-Certified Plastic Surgeon to ensure that they are suitable for the procedure and can safely undergo liposuction. 

Some patients who are unsuitable question whether they can use donor fat to be grafted to their buttocks. However, this is very rarely a possible option as it does not tend to be safe and the fat also tends to be rejected by the body so it is not an effective procedure.

Candidates who are not suitable do have other options for buttocks enhancement as butt implants do not require a patient to have excess fat deposits across their body to be used for a fat transfer. Butt implants can be inserted and there are a variety of sizes and shapes of implants to ensure that patients can customize their butt enhancement procedure. Finally, it is possible to have injections to enhance the size of the buttocks so there are other treatment options patients can undergo to achieve their surgical goals.

BBL procedures are performed by carrying out Liposuction , this is where a cannula is inserted into an area of the body to vacuum out fat deposits. For a Skinny BBL, this will likely be performed on multiple areas of a patient’s body.  For a BBL, liposuction is often performed on the waist, hips, or back while for a Skinny BBL these areas are often all needed to gather enough fat deposits. Liposuction can be performed on any area across the body including the arms, neck, chin, cheeks, back, waist, hips, love handles, thighs, or lower legs. 

Additionally, liposuction often needs to be more precise for a Skinny BBL as these patients have less fat so it is essential not to remove too much fat. Once these fat cells have been harvested they will need to be placed into a centrifuge so that the fat can be purified before it can be inserted into the buttock. 

After this, the fat that has been grafted will be injected into the buttocks in specific quantities across the buttocks to achieve the results, size, and shape the patient desires. Again, a Skinny BBL often requires more precise injections of fat to ensure that the buttocks remain proportional to the patient’s figure and the results are natural. 

Skinny BBL Results 

The results of any type of BBL can be long-lasting or even permanent as the fat deposits are permanently removed from the body so these results can be long-term. Additionally, the results of a fat graft can be long-lasting after the initial graft has settled. During recovery around half of the fat injected can be absorbed after the procedure and the rest of the grafted fat will remain.

Therefore, the results can be long-lasting but since the procedure revolves around removing and reinserting fat, this fat can be affected like any fat in the body. This means that if a BBL patient gains weight or loses weight this will affect the results

According to RealSelf , surgeons are split on whether the recovery from a Skinny BBL is the same as the recovery from a traditional BBL. Since liposuction is undertaken for both procedures the aftercare instructions will be the same for this including needing to wear compression garments. However, for the Skinny BBL, there will likely be many more areas where liposuction was performed and so it may be necessary for these patients to wear garments on more areas of the body. Also According to RealSelf , there tends to be less downtime after a Skinny BBL, and it is often only necessary that patients stay off their buttocks for two weeks rather than the usual six for traditional BBL procedures.

The reason that patients must avoid sitting or putting pressure on the buttocks is the same for both BBL procedures, that more fat will be absorbed if pressure is applied to the buttocks and so will negatively impact the BBL results.

This young patient is shown 2 months following a “skinny” Brazilian Butt Lift (BBL) with 360 Liposuction.  Oftentimes in patients who do not have much fat, a BBL can be more about “what you take out”, and less about what you “put in”.  With careful liposuction of the abdomen, flanks, and back in this patient, I tried to “snatch” the waist to accentuate her buttock contour and placed the harvested fat predominantly in the hip dips to optimize her hourglass figure.  As always, extra time was taken in the OR to filter, purify, and concentrate the suctioned fat prior to injecting it safely into the buttocks to maximize fat graft take

six things you need to know about recovering from a Brazilian butt lift

There's no shortage of curiosity or debate surrounding the Brazilian butt lift . This buzzworthy procedure, which transfers fat from one area of the body into the butt and hips for a more shapely hourglass silhouette, continues to shed its formerly infamous reputation, as education expands, spurring refined surgical techniques and fewer postop complications. In fact, it's safer than ever to perform and to receive a BBL, according to Oakland, California-based plastic surgeon and American Society of Plastic Surgeons member, Steven Williams, MD , who is a nationally recognized authority on the procedure.

"It's important to recognize that the initial reported dangers of BBL surgery have significantly reduced through surgeon education and training," says Dr. Williams. Compared to several years ago, the number of serious complications associated with this procedure have dramatically reduced. For the safest possible result, it's always important to consult a board-certified plastic surgeon."

While safer, the procedure also requires a very specific set of recovery dos and don'ts to support an optimal result, and what makes the recovery journey so notorious – and attracts so much curiosity and interest – is the patient's postop delicate dance of sitting, sleeping and just existing, without applying pressure to the sensitive area and subsequently, causing damage.

Every plastic surgery procedure comes with its own collection of recovery realities, and to satisfy our curiosity once and for all, we asked three board-certified plastic surgeons and ASPS members who perform BBLs regularly for their take on what exactly to expect once you've been lipo'ed and lifted.

Initial recovery lasts about five days

New York City-based plastic surgeon, Chris Funderburk, MD , who performs 100 to 150 BBL procedures each year, describes the recovery in three phases: Initial, resuming everyday life with restrictions and no restrictions.

"A lot of times during those first four to five days after surgery, the patient will just stay home," he says. "We encourage them to walk around their home or at most the little local neighborhood, but they just need to really relax and recover."

Compression garments are worn for at least one month

Many prospective patients don't realize a BBL procedure is actually a 2-in-1 surgery, starting with liposuction followed by fat grafting into the buttock area.

"The recovery is similar to other liposuction-type procedures," says Dr. Williams, who adds that many find the liposuctioned areas to be tender, but not overly sensitive. "Patients should expect to have a recovery period that consists of mild to moderate pain and most need some degree of pain medication for the first four to five days."

"I'm a very big fan of compression garments because they definitely help alleviate the swelling," says Dr. Funderburk. "I advise patients to wear them as much as possible, but basically 24/7, except for showering, the first month and at least 12 hours a day the second, usually they can at least do it at night."

Sitting is restricted

This crucial piece of the recovery puzzle affects every aspect of a patient's new postop life for several months, or else they're at risk for asymmetrical or quickly diminishing results.

"You're taking fat from an area of the body where it lives happily and has a nice blood supply, then you're taking it out and moving it somewhere new," explains Dr. Funderburk. "And then there's a period of time where that fat has to incorporate into the new area and obtain its own blood supply. During that period, the fat is very susceptible to pressure necrosis, or pressure that kills off the fat cells, so for the first six weeks, the patient has to be very careful about not putting undue or prolonged pressure on the buttocks."

This recovery guideline has the potential to make ordinary activities, like going to the bathroom, rather challenging.

"We tell patients something along the lines of, just go to the bathroom, it's fine," says Dr. Funderburk. "But try to keep it within the 10-minute window, which is about as long as you should be sitting at any given time for the first six weeks."

Of course, patients aren't expected to stand for six weeks, and there are clever workarounds. One of which is something Dr. Funderburk refers to as a "BBL pillow," which is a discreet, triangle-shaped pillow that will "offset the pressure onto their thighs" and is small enough to be carried to restaurants as patients begin to resume normal activities.

Patients return to work between days seven and 10, and if they're sitting at a desk all day, they need to be using the BBL pillow, or some type of cushioning and padding.

Sleeping requires a strategy

Rest is a critical aspect of any post-surgical recovery, and without it, you're more likely to feel groggy and prolong your recovery period overall. Patients are advised to sleep on their stomachs for the first night postop, and until given the clear from their surgeon.

And if you're a back sleeper exclusively? Surgeons have gotten creative with ways to keep BBL patients as comfortable as possible.

"For patients who absolutely cannot sleep on their tummies, we recommend a pool float that looks like an avocado," says Dr. Funderburk. "It has a hole for where the pit would be and a lot of patients find that nice to put on their bed, cover it with sheets and they have an opening for their buttocks."

Final results will evolve

"Most patients will see an immediate improvement in contour, both in the areas where liposuction was performed and the areas where fat was grafted," says Dr. Williams, who notes that there is a second phase of visible results.

"Most people refer to the second phase of recovery as one where the grafted fat is stable, but the final cosmetic results from the liposuction are still evolving," he explains. During this period, he says that the body is experiencing both tightening in the treated areas and swelling around the liposuction sites.

Around month three, the acute risk of losing fat from the buttock area subsides, says President of ASPS and Endowed Professor and Chair of Plastic Surgery at the University of Pittsburgh Medical Center, J. Peter Rubin, MD , and the region's size and shape will be a "pretty good indicator of what's going to stay there."

Dr. Williams notes that during months three to six, the shape "should further refine and the curve should become more apparent."

You may want a second BBL procedure

Even if a BBL patient did everything right, sometimes nature just takes its course.

"We know from a number of studies that the average fat take or the amount of the transferred fat that will survive is about 60%," says Dr. Funderburk. "And the remaining 40% is just naturally broken down and reabsorbed by the body, so while we want to do everything we can to maximize that fat take, we also intentionally overfill a bit to help compensate."

It's not uncommon for patients to return to their plastic surgeon for another BBL to smooth out any contour irregularities and asymmetry that may have formed during healing, while others return seeking a bigger butt, especially as the amount of fat that "takes" is highly variable and dependent upon a patient's compliance to the recovery guidelines.

What Is a Skinny BBL?

A Skinny BBL (Brazilian butt lift) is a procedure done to define the shape of the buttocks for patients who have a lean, or “skinny,” body type. Since patients with a lean body type have less body fat, buttock enhancement is subtler and more natural-looking.

The fat harvesting in a Skinny BBL procedure comes from liposuction in multiple areas. The areas to be harvested for fat grafting will be assessed and discussed during the consultation.

Am I a Good Candidate for a Skinny BBL?

Candidates for a Skinny BBL have a lean body type and are interested in augmentation the volume of their rear end. The best candidates are healthy individuals without any underlying conditions that affect the healing process.

Before considering a Skinny BBL, patients will need to avoid smoking and drinking alcohol before their procedure to prevent complications. They may also need to stop taking non-steroidal anti-inflammatory medications. Consult with your doctor to discuss alternatives.

Your Skinny BBL Consultation

The Procedure

A Skinny BBL procedure will begin with administering anesthesia. Once the anesthesia takes effect, Dr. Pakeman will begin creating very tiny incisions in the selected areas for liposuction.

After the fatty tissue is removed, it will then be prepared for buttocks enhancement. Finally, Dr. Pakeman will transfer the fat cells into the buttocks using precise injections to sculpt and lift.

The Recovery Process

The initial recovery period after a Skinny BBL is one week. During the first week, you will need to take time off work and restrict your movement. Have someone else drive you on the day of the procedure and accompany you during the first 24 hours.

A special pillow will be recommended to assist you when sitting. Please arrange enough pillows for support in your sleep area to help you sleep in the correct position.

You will need to avoid strenuous activities like heavy lifting, running, and sports during the recovery process. You can resume these activities after around six weeks.
