The Safe Travels Voyager is designed for those who are looking for the highest benefit limits available to protect a trip. This plan covers trips up to $100,000 for trip cancellation and includes $250,000 per person of primary medical coverage. The plan covers pre-existing medical conditions when purchase of the plan is within 21 days of initial deposit, prior to the Final Payment, the full, non-refundable trip cost is insured, and you are medically able to travel on the policy effective date. Trip delay includes minimum coverage of $3,000 for potential accommodations/lodging expenses.

This plan offers several Optional Coverages including:

• Cancel for Any Reason (CFAR) provides reimbursement for 75% of the prepaid, non-refundable, forfeited payments you paid for your trip if you cancel your trip for any reason not otherwise covered by this policy. Must be purchased with the initial plan and within 21 days of initial deposit, prior to the Final Payment and the full, nonrefundable trip cost is insured. To use the CFAR option, you must cancel your trip in its entirety, at least 2 days prior to your scheduled departure date.
• Interruption for Any Reason (IFAR) if purchased provides reimbursement for 50% of the insured trip cost up to $50,000
• Additional $250,000 of accident and sickness medical coverage for trips 1-31 days in length
• Trip delay maximum upgrades to $4,000 or $7,000 maximum per person
• 24 Hour accidental death and dismemberment.

Provides reimbursement of insured, non-refundable, pre-paid trip costs if a trip is cancelled or interrupted for a covered reason*.

If You purchase this optional benefit, the following coverage is available to You. If You cancel Your Trip for any reason not otherwise covered by this Policy, the Company will reimburse You for the percentage of the pre-paid, forfeited, non-refundable Payments or Deposits You paid for Your Trip shown on the Schedule of Benefits provided: (a) Your premium is received within the Time Sensitive Period and prior to Final Payment; and (b) You insure all pre-paid Trip costs that are subject to cancellation penalties or restrictions, and also insure within twenty-one (21) days of the payment for those arrangements, the cost of any subsequent arrangements (or any other arrangements not made through Your travel agent) added prior to Your Trip; and (c) You cancel Your entire Trip two (2) days or more before Your Scheduled Departure Date.

If You purchase this optional benefit, the following coverage is available to You. If You interrupt Your Trip, forty-eight (48) hours or more after Your actual departure date, for any reason not otherwise covered by this Policy, the Company will reimburse You for the percentage of the a) unused portion of the pre-paid, forfeited, non-refundable Payments or Deposits You paid for Your Trip; b) the airfare paid less the value of applied credit from an unused travel ticket to return home limited to the cost of one-way economy airfare or similar quality as originally issued ticket by scheduled carrier, from the point of destination to the point of origin shown on the original travel tickets shown on the Schedule of Benefits; provided: (a) Your premium is received within the Time Sensitive Period and prior to Final Payment; and (b) You insure all Trip costs that are subject to cancellation penalties or restrictions, and also insure within twenty one (21) days of the payment for those arrangements the cost of any subsequent arrangements (or any other arrangements not made through Your travel agent) added prior to Your Trip.

Coverage if You are delayed, while coverage is in effect, en route to or from the Trip for six (6) or more hours due to a covered reason* Covered Trip Delay Expenses include: (a) Any pre-paid, unused, non-refundable land and water accommodations; (b) Any Reasonable Expenses incurred; (c) An Economy Fare from the point where You ended Your Trip to a destination where You can catch up to the Trip; (d) A one-way Economy Fare to return You to Your originally scheduled return destination trip is delayed for a covered reason*.

Provides reimbursement for lost and stolen personal items, as well as costs incurred due to baggage delay.

Coverage if you miss your Cruise or tour departure, scheduled during your trip, that results from the cancellation or a delay for between three (3) and six (6) hours of all regularly scheduled airline flights due a Documented weather condition preventing You from getting to the point of departure for your trip. Benefits are provided for additional transportation expenses to join your departed cruise or tour; Reasonable expenses up to the per day limit; pre-paid nonrefundable payments or deposits for the unused portion of your cruise. Coverage is secondary to any compensation provided by a Common Carrier. Coverage will not be provided to individuals who are able to meet their scheduled departures but cancel their cruise or tour due to inclement weather.

Provides reimbursement for emergency medical treatment if sickness or injury occurs while traveling.

Exclusion is waived if coverage is purchased within 21 days of initial trip deposit date, before the final trip payment, and the full, non-refundable trip cost is insured, and you are medically able to travel on the policy effective date.

You may cancel this plan for refund by giving notice of cancellation to us, during the free look period. Most states have a 10 day free look period beginning the date the policy was purchased, but the State Certificate ultimately determines the timeframe of the free look period, if any.

Said arbitration shall be administered by JAMS. The arbitral tribunal (“Tribunal”) shall be composed of one arbitrator, who shall be independent and impartial. If the parties fail to agree on the arbitrator within twenty (20) calendar days after the initiation of an arbitration hereunder, JAMS shall appoint the arbitrator. The arbitration shall be conducted in the English language. The decision of the arbitrator will be final and binding on the parties, subject to all rights of appeal allowed by JAMS. Judgment on any award(s) rendered by the arbitrator may be entered in any court having jurisdiction thereof. The arbitrator shall have the authority to determine arbitrability of any disputes arising out of or relating to this Agreement. Let Me Know When You Arrive Safely

We could also use a phrase like “text me when” instead of “let me know when.” If we want to be more specific about the manner of messaging, this phrase works just as well.

• This is especially true for flying out of a major city hub. If you’re flying out of a major city and flying internationally, then you may want to arrive at the airport 2-3 hours before the departure time.
• If possible, consider bringing only a personal and a carryon for your trip and avoid checking a bag. This will save you money since most airlines charge you to check bags (some charge you for carryons as well). It will also save you time since you do not have to worry about dropping your luggage off at the gate.

It is relatively easy to track your loved ones adventure with today’s technology. I use an app called Flight Radar 24 , which allows you to track the exact flight they’re on and pinpoint exactly where they are in the sky.

This is also great if you want to know if there will be any delays or exactly when they land.

You can use this app for other things as well, like exploring the map view and seeing every plane in the sky! It’s amazing to see how many planes are actually in the sky at once.

In general, it’s always good to stay in close contact with your loved ones, especially when they’re away from you! Letting them know you’re thinking of them with a simple text message goes a long way!

It sure was helpful when you said that you must do your best to do well throughout the flight since lights tend to dehydrate you. This is something that I will share with my parents since they mentioned that they are planning to rent a helicopter for a tour next week. Christopher Columbus returned from his first voyage in March 1493, having discovered the New World—although he didn’t know it. He still believed that he had found some uncharted islands near Japan or China and that further exploration was needed. His first voyage had been a bit of a fiasco, as he had lost one of the three ships entrusted to him and he did not bring back much in the way of gold or other valuable items. He did, however, bring back a group of Indigenous people he had enslaved on the island of Hispaniola, and he was able to convince the Spanish crown to finance the second voyage of discovery and colonization.

The second voyage was to be a large-scale colonization and exploration project. Columbus was given 17 ships and over 1,000 men. Included on this voyage, for the first time, were European domesticated animals such as pigs, horses, and cattle. Columbus’ orders were to expand the settlement on Hispaniola, convert the population of Indigenous people to Christianity, establish a trading post, and continue his explorations in search of China or Japan. The fleet set sail on October 13, 1493, and made excellent time, first sighting land on November 3.

The island first sighted was named Dominica by Columbus, a name it retains to this day. Columbus and some of his men visited the island, but it was inhabited by fierce Caribs and they did not stay very long. Moving on, they discovered and explored a number of small islands, including Guadalupe, Montserrat, Redondo, Antigua, and several others in the Leeward Islands and Lesser Antilles chains. He also visited Puerto Rico before making his way back to Hispaniola.

Columbus had wrecked one of his three ships the year of his first voyage. He had been forced to leave 39 of his men behind on Hispaniola, in a small settlement named La Navidad . Upon returning to the island, Columbus discovered that the men he left had raped Indigenous women and angered the population. Indigenous people had then attacked the settlement, slaughtering the Europeans to the last man. Columbus, consulting his Indigenous chieftain ally Guacanagarí, laid the blame on Caonabo, a rival chief. Columbus and his men attacked, routing Caonabo and capturing and enslaving many of the people.

Columbus founded the town of Isabella on the northern coast of Hispaniola, and spent the next five months or so getting the settlement established and exploring the island. Building a town in a steamy land with inadequate provisions is hard work, and many of the men became sick and died. It reached the point where a group of settlers, led by Bernal de Pisa, attempted to capture and make off with several ships and go back to Spain: Columbus learned of the revolt and punished the plotters. The settlement of Isabella remained but never thrived. It was abandoned in 1496 in favor of a new site, now Santo Domingo .

Columbus left the settlement of Isabella in the hands of his brother Diego in April, setting out to explore the region further. He reached Cuba (which he had discovered on his first voyage) on April 30 and explored it for several days before moving on to Jamaica on May 5. He spent the next few weeks exploring the treacherous shoals around Cuba and searching in vain for the mainland. Discouraged, he returned to Isabella on August 20, 1494.

Columbus had been appointed governor and Viceroy of the new lands by the Spanish crown, and for the next year and a half, he attempted to do his job. Unfortunately, Columbus was a good ship’s captain but a lousy administrator, and those colonists that still survived grew to hate him. The gold they had been promised never materialized and Columbus kept most of what little wealth was found for himself. Supplies began running out, and in March of 1496 Columbus returned to Spain to ask for more resources to keep the struggling colony alive.

Columbus brought back many enslaved Indigenous people with him. Columbus, who had once again promised gold and trade routes, did not want to return to Spain empty-handed. Queen Isabella , appalled, decreed that the New World Indigenous people were subjects of the Spanish crown and therefore could not be enslaved. However, the practice of enslaving Indigenous populations continued.

• Juan Ponce de León would become governor of Puerto Rico but was most famous for his journey to Florida in search of the Fountain of Youth .

Columbus’ second voyage marked the start of colonialism in the New World, the social importance of which cannot be overstated. By establishing a permanent foothold, Spain took the first steps toward its mighty empire of the centuries that followed, an empire that was built with New World gold and silver.

When Columbus brought back enslaved Indigenous peoples to Spain, he also caused the question of whether to practice enslavement in the New World to be aired openly, and Queen Isabella decided that her new subjects could not be enslaved. But although Isabella perhaps prevented a few instances of enslavement, the conquest and colonization of the New World was devastating and deadly for Indigenous peoples: their population dropped by approximately 80% between 1492 and the mid-17th century. The drop was caused mainly by the arrival of Old World diseases, but others died as a result of violent conflict or enslavement.

Many of those who sailed with Columbus on his second voyage went on to play very important roles in the trajectory of history in the New World. These first colonists had a significant amount of influence and power over the span of the next few decades.

Organizers of Chicago festivals, events go all out to keep people safe

CHICAGO (CBS) -- Organizers of festivals in every part of Chicago are trying to keep everyone safe this summer.

The Puerto Rican Festival, or Fiestas Patronales Puertorriqueñas , began Thursday in Humboldt Park. Music boomed within the festival grounds, but getting in to see it required waiting in a security line.

Organizers prioritized safety—with a fence around the perimeter and bag checks at the entrance.

"One of the biggest things that we are focusing on - and have been focusing on for the last couple of years—is making sure this is completely family-friendly. That's a priority. Safety is a priority," said Puerto Rican Fest host Mike Oquendo. "So everything - every decision that's made - is made with that in mind."

Organizers say 2,000 people preregistered to enter the festival grounds on Thursday. They expect attendance to grow throughout the four-day festival.

Safety concerns at Chicago area events are not merely theoretical. Last weekend, the Posen Park Fest carnival was cut short Saturday evening after a large fight.

A fight broke out around 6 p.m. Sunday at the festival and carnival  and continued for almost two hours. Police made several arrests in connection to the incident and shut down the carnival due to safety concerns.

But back in Humboldt Park, the show will go on, and the festival will adapt to whatever organizers face.

"The festival in the last couple of years post-pandemic has gone—Like other festivals in the city, like other festivals in the country – has gone through some challenges, and the community has worked through that," said Oquendo.

Navy Pier fireworks on July 3, 6 among major summer events

Meanwhile, the city acknowledged it will not hold a fireworks show on Lake Michigan on the Fourth, instead deferring to Navy Pier's regularly scheduled fireworks shows on Wednesdays and Saturdays. This is not a departure from the norm of recent years—the city has not held a fireworks show since 2010, and Navy Pier fireworks shows have been the only option since then.

The city once hosted a July 3 fireworks extravaganza the night before the 4th as part of the Taste of Chicago in Grant Park. However, this fireworks show has not been held since 2009, when Richard M. Daley was still mayor.

Today, even the Taste of Chicago itself is now held in September.

In 2008, a man was killed, and several others were injured as patrons flooded into the Loop from Grant Park after the July 3 fireworks show and gunshots rang out. The shooting caused mass hysteria in the streets of the Loop and prompted more stringent security measures at the Taste the following year.

The Taste of Chicago was still held over the July 4th weekend in 2010, but there was no July 3 fireworks show that year. It was replaced with three smaller shows—at Navy Pier, 63rd Street Beach on the South Side, and Montrose Beach on the North Side—but this plan only happened once.

Since 2011, the Navy Pier fireworks shows have been the only ones for the 4th. The Navy Pier fireworks shows have been held on July 4th in past years , but not in some time.

Last year, the Navy Pier fireworks show for Independence Day was on July 1, with no fireworks on the 4th. This year, the Navy Pier Independence Day fireworks show will be held on Wednesday, July 3.

There will also be fireworks at Navy Pier on Saturday, July 6.

Boeing spacecraft carrying two astronauts lifts off on historic maiden voyage

The third attempt was the charm for Boeing’s Starliner mission after launching its first crewed flight test Wednesday in a milestone that has been a decade in the making.

The new spacecraft’s maiden voyage with humans on board lifted off atop an Atlas V rocket at 10:52 a.m. ET from Cape Canaveral Space Force Station in Florida.

The historic event is streaming live on NASA’s website .

Veteran NASA astronauts Butch Wilmore and Suni Williams are riding aboard the Starliner capsule on a journey that takes them to the International Space Station.

Weather conditions were 90% favorable for a Wednesday morning launch, with the only concern being cumulus clouds, according to the US Space Force’s 45th Weather Squadron.

The mission, known as the Crew Flight Test, is the culmination of Boeing’s efforts to develop a spacecraft to rival SpaceX’s prolific Crew Dragon capsule and expand the United States’ options for ferrying astronauts to the space station under NASA’s Commercial Crew Program. The federal agency’s initiative aims to foster collaboration with private industry partners.

The flight marks only the sixth inaugural journey of a crewed spacecraft in US history, NASA Administrator Bill Nelson noted in a May news conference.

“It started with Mercury, then with Gemini, then with Apollo, the space shuttle, then (SpaceX’s) Dragon — and now Starliner,” Nelson said.

Williams will also make history as the first woman to fly aboard such a mission.

Historic flight has lofty goals

The astronauts will spend just over 24 hours traveling to the space station.

After docking around 12:15 p.m. ET Thursday, Williams and Wilmore are set to spend eight days living in the orbiting laboratory, joining the seven astronauts and cosmonauts already on board.

Aboard Starliner is a crucial pump needed to fix the space station’s urine processor assembly, which failed on May 29.

“That urine processor takes all of the crew’s urine and processes it in the first step of a water recovery system,” said Dana Weigel, manager for NASA’s International Space Station Program. “It then sends it downstream to a water processor which turns it into drinking water. The station’s really designed to be a closed loop.”

Now, the urine has to be stored onboard in containers, so Starliner’s anticipated arrival to the space station can’t come soon enough.

The astronauts will test various aspects of Starliner’s capabilities, including the spacecraft’s thruster performance, how their spacesuits function within the capsule, and manual piloting in case the crew needs to override the spacecraft’s autopilot.

Williams and Wilmore will also test Starliner’s “safe haven” capability, designed to offer the space station crew a shelter if there is a problem, according to Steve Stich, manager of NASA’s Commercial Crew Program, during a May 31 news conference.

When it’s time to head home, the astronauts will return using the same Starliner capsule and parachute to a landing at one of several designated locations across the southwestern United States.

Years of development hang-ups, test flight problems and other costly setbacks have slowed Starliner’s path to the launchpad. Meanwhile, Boeing’s competitor under NASA’s Commercial Crew Program — SpaceX — has become the go-to transportation provider for the space agency’s astronauts.

This mission could be the final major milestone before NASA deems Boeing’s Starliner spacecraft ready for routine operations to deliver astronauts and cargo to the space station.

Weeks of troubleshooting

A number of issues caused the previous crewed launch attempts, on May 6 and June 1, to be scrubbed.

Two hours prior to the launch attempt on May 6, engineers identified an issue with a valve on the second stage, or upper portion, of the Atlas V rocket, which was built by United Launch Alliance, a joint venture between Boeing and Lockheed Martin. The entire stack, including the rocket and spacecraft, was rolled back from the launchpad for testing and repairs.

Teams also worked through a small helium leak within the spacecraft service module, a “ design vulnerability ” in the propulsion system and assessed the parachutes for the Starliner capsule.

Starliner was just 3 minutes and 50 seconds from liftoff Saturday afternoon, when an automatic hold was triggered by the ground launch sequencer, or the computer that launches the rocket.

United Launch Alliance technicians and engineers assessed the ground support equipment over the weekend, examining three large computers housed inside a shelter at the base of the launchpad. Each computer is the same, providing triple redundancy to ensure the safe launch of crewed missions.

“Imagine a large rack that is a big computer where the functions of the computer as a controller are broken up separately into individual cards or printed wire circuit boards,” said Tory Bruno, president and CEO of United Launch Alliance, during a Saturday news conference. “They’re all stand-alone, but together, it’s an integrated controller.”

The cards within the computers are responsible for different key systems that must occur before a launch, such as releasing bolts at the rocket’s base so it can lift off after ignition.

During the final four minutes before launch, all three computers must communicate and agree with one another. But during Saturday’s countdown, a card on one of the computers was six seconds slower in responding than the other two computers, indicating that something was not correct and triggering an automatic hold, according to Bruno.

Over the weekend, engineers evaluated the computers, their power supply and network communications between the computers. The team isolated the issue to a single ground power supply within one of the computers, which provides power to the computer cards responsible for key countdown events — including the replenishment valves for the rocket’s upper stage, which also caused an issue during the countdown, according to an update shared by NASA.

Starliner teams reported no signs of physical damage to the computer, which they removed and replaced with a spare. The other computers and their cards were also assessed, and all of them are performing normally as expected, according to the ULA team.

Billionaire Plans Dive to the Titanic in a Newly Designed Submersible

Larry Connor, 74, who made his wealth in real estate, said he’s building a new acrylic-hulled submersible that will be certified and rigorously tested to show that deep sea exploration is safe.

A real estate billionaire in Ohio is planning an underwater voyage to the site of the Titanic shipwreck, where a submersible imploded on its approach to the sea floor a year ago, killing all five passengers on board.

Shortly after the OceanGate disaster, Larry Connor, 74, a real estate investor and amateur adventurer, contacted the co-founder of Triton Submarines, Patrick Lahey, imploring him to build a submarine that could reach the depths of the Titanic safely and repeatedly, according to The Wall Street Journal.

The two men aim to explore and conduct scientific research at the site, located off the coast of Newfoundland, 12,500 feet under the sea, in a two-person submersible that Triton is designing in the summer of 2026.

“Ours is just not a trip to the Titanic,” Mr. Connor said in an interview on Tuesday. “It’s a research mission.”

“The other purpose is to demonstrate to people around the globe that you can build a revolutionary, first-of-its-kind sub and dive it safely and successfully to great depths,” he added.

The custom sub, which Mr. Connor plans to call “The Explorer — Return to the Titanic,” is still in the design phase and will be based on an existing submarine design that Mr. Lahey had worked on for years. It is listed on the Triton website as the Abyssal Explorer, an acrylic-hulled vessel than can reach depths of 13,000 feet, “the perfect submersible for repeated trips to the deep ocean.”

“Once subsurface, the submersible’s hydrodynamic shape — with wings folded — speeds the descent to 13,000 feet,” the company said on its website. “The journey takes less than two hours, significantly faster than previously possible.” It will be the first acrylic-hulled sub to reach such depths, Mr. Connor said, expanding the visibility of a deep-sea sub from small window portals and cameras to a 320-degree view.

“Frankly, the technology didn’t exist even six or eight years ago,” Mr. Connor said. “It’s only been with recent developments in the last five years that you could build this.”

An interview request with Mr. Lahey was referred to Mr. Connor’s spokesman, who said that only Mr. Connor would speak about the expedition.

Mr. Connor said his interest in venturing down to the Titanic in a new vessel stems from his broader interest in advancing ocean exploration, in this case by innovating the field's best tool — the submersible.

“The best way, in my limited experience, to explore the ocean, is in a submersible,” he said.

The final cost of the sub is still to be determined, but Mr. Connor said it would be in the millions.

Mr. Connor took great pains to contrast the submersible that he plans to use on his dive to the Titanic with that used in the deadly voyage to the sunken ship a year ago.

After the Titan disaster on June 18, 2023, criticism emerged from recreational and professional underwater explorers about the vessel’s cost-saving design choices .

The vessel disappeared under the dark waters of the North Atlantic, losing contact with its Canadian expedition ship on the surface, MV Polar Prince, about 400 miles south of St. John’s, Newfoundland, an hour and 45 minutes into its journey.

On board were Stockton Rush , 61, the founder and chief executive of OceanGate Expeditions, who was piloting the vessel; Hamish Harding , 58, a British businessman and explorer; Paul-Henri Nargeolet , 77, a French maritime expert; Shahzada Dawood , 48, a British Pakistani businessman; and his son, Suleman, 19.

Six days later, a multinational search ended with evidence of a catastrophic implosion that left no survivors.

Until the Titan disaster, no one had ever died while piloting or riding a submersible in the deep sea, a safety record that stood for nearly a century, despite explorers making many thousands of dives.

Mr. Connor contends that the OceanGate episode has hurt the submersible industry and tainted the public’s perceptions of attempts to innovate in the space.

“I’m concerned that people associate diving subs, especially new or different subs, with danger or tragedy,” Mr. Connor said.

Mr. Connor said he wanted to reassert the safety of well-made submersibles, certified (which the industry calls classified) by respected organizations that do rigorous testing. Mr. Connor says the sub would be certified, and take two and a half to three years to build.

OceanGate’s experimental Titan design was not certified, which Mr. Rush advertised as proof of the sub’s cutting-edge innovation even as industry experts expressed concern about the vessel’s safety.

Mr. Connor, on the other hand, said he had a reputation for never taking on “unacceptable risk.”

“If we can’t do it, what we call ‘s and s’ — safely and successfully — we’re just not going to do it,” Mr. Connor said. “We’re not thrill-seekers. We’re not big risk-takers.”

Mr. Connor is also a record-setting skydiver , astronaut and deep-sea explorer, who in 2021 joined Mr. Lahey on three deep dives in five days at the Mariana Trench in the western Pacific Ocean, some 200 miles off the coast of Guam. Their vessel, a Triton-built sub known as D.S.V. Limiting Factor, reached sea depths of about 35,000 feet, taller than Mount Everest.

In April 2022, Mr. Connor joined two other paying customers and a retired NASA astronaut on a SpaceX flight to the International Space Station, the first such mission manned only by private citizens and NASA’s first foray in space tourism.

During the eight-day mission, which cost Mr. Connor and the other two customers $55 million each, Mr. Connor and others conducted a series of research experiments in collaboration with the Mayo Clinic and other medical organizations.

An earlier version of this article incorrectly described a submersible safety record prior to the Titan submersible disaster. No one had died on a submersible in the deep sea, but there had been deaths on submersibles in shallower water.

Paxlovid, effective in preventing severe COVID-19, didn’t appear to help long-COVID patients in this single-center study. But further research may show benefits with different doses or for people with specific symptoms.

In a clinical trial conducted by Stanford Medicine investigators and their colleagues, a 15-day course of Paxlovid — an antiviral drug combination targeting SARS-CoV-2, the virus that causes COVID-19 — proved safe as an extended-duration treatment but didn’t lessen select symptoms of the syndrome known as long COVID: the persistence, or reappearance, of COVID-related symptoms three months or more after an initial COVID-19 infection. 

The findings are described in a paper published June 7 in  JAMA Internal Medicine .

Paxlovid is a highly effective antiviral agent licensed for treatment of acute COVID-19. It’s approved by the Food and Drug Administration for the treatment of adults who are newly infected with SARS-CoV-2, have mild or moderate COVID symptoms, and are at high risk of complications because of age or various predisposing conditions. A five-day Paxlovid regimen has been shown to reduce recipients’ likelihood of hospitalization and death by more than 85%. 

While the Stanford trial did not show that Paxlovid reduced long COVID symptoms, it did show that taking the drug for more than two weeks was safe. 

“We’ve demonstrated the overall safety of a 15-day course of Paxlovid — that’s three times as long as it’s being taken for acute COVID,” said  Linda Geng , MD, PhD, clinical associate professor of primary care and population health, who was one of the trial’s two co-principal investigators. 

Geng is the paper’s lead author. Its senior author is the trial’s other co-principal investigator,  Upinder Singh , MD, professor and chief of infectious disease and geographic medicine and of microbiology and immunology.

Desperate people

“While there are now improved therapies and treatment practices for acute COVID, there’s nothing FDA-approved for long COVID, people continue to suffer and the numbers keep piling up,” Geng said.

An estimated 10% to 20% of SARS-CoV-2-infected people — tens of millions in the United States alone — develop long COVID. That estimate is fuzzy, because the definition of long COVID is ambiguous. More than 200 separate symptoms have been ascribed to the syndrome. These symptoms are often shared by other conditions, making a definitive diagnosis hard to pin down. It may well be that long COVID is a collection of different diseases set in motion by acute COVID but with different underlying mechanisms and effects and accordingly distinct methods required to alleviate them.

Indeed, numerous causes of long COVID have been posited. Among them are virus-induced changes in our resident gut bacteria; residual inflammatory overdrive; COVID-spurred autoimmunity; and the reawakening of other infectious viruses that have woven themselves into our genomes and remained dormant, only to emerge when our SARS-CoV-2-preoccupied or exhausted immune systems let their guard down.

The randomized, controlled, double-blinded phase 2 trial, known as STOP-PASC, is the first such test of Paxlovid treatment for long COVID. (STOP stands for Selective Trial of Paxlovid. PASC is an acronym for Post-Acute Sequelae of COVID, medical scientists’ formal name for long COVID.) The trial was designed to explore the so-called viral-reservoirhypothesis of long COVID: the possibility that while initial symptoms may have abated and measured viral counts gone to zero, the virus or viral debris persists in deep tissues, hiding out and perhaps replicating slowly there. 

“Some studies suggest that viral particles and molecular debris could be responsible for some long-COVID sufferers’ ongoing symptoms,” Singh said. “We figured if that’s the case, maybe treating them with Paxlovid could relieve some of these symptoms.”

There are anecdotal reports of Paxlovid providing relief for long-COVID symptoms. “We’ve observed long-COVID patients who’ve improved after taking Paxlovid for new infections,” said Geng, who co-directs Stanford’s long-COVID clinic.

Between November 2022 and September 2023, Geng, Singh and their colleagues enrolled and followed 155 trial participants who’d tested positive for SARS-CoV-2. All but two had been vaccinated. Demographically, they broadly reflected the San Francisco Bay Area’s ethnic distribution. Their median age was 43; about two-thirds were between 34 and 54 years old. 

On average, participants had been initially infected more than 16 months before enrolling in the trial. Yet, each participant reported moderate to severe cases of at least two out of six common “core” long-COVID symptoms: fatigue, brain fog, shortness of breath, body aches, and cardiovascular or gastrointestinal symptoms.

Half of the study participants were randomized to a 15-day course of Paxlovid; the others received a placebo along with low dosages of one of the two separate drugs that, combined, constitute Paxlovid. Both of these drugs are antiviral agents, but only one of the two, nirmatrelvir, has been found effective against SARS-CoV-2. The other drug, ritonavir, has no direct efficacy against this virus, but it ties up some of the same liver-resident enzymes that break down nirmatrelvir, boosting nirmatrelvir’s effective concentration in the body. 

Because ritonavir can cause an easily recognized side effect — a metallic taste in the mouth — the investigators added it to the placebo formulation so placebo recipients wouldn’t be able to distinguish it from the active drug, and vice versa.

Partway through the study, the data was submitted to an independent group for preliminary analysis. On determining that safety appeared to pose no problem but no benefit was yet evident, the researchers continued the trial but stopped recruiting new participants.

At 10 weeks, the prespecified time point for the final comparison, there was no statistically significant difference between the two groups in the study’s primary endpoint: a reduction in the severity of the six core symptoms. Nor was there any detectable divergence between the two groups in numerous secondary outcomes such as seated and standing blood pressure and heart rates, and performance on the one-minute “sit and stand” test (subjects are asked to sit in a chair, stand up and sit down again repeatedly as fast as they can for a minute).

The 15-day drug regimen was, however, safe. There was one serious, possibly trial-related adverse event (hepatitis) among the placebo recipients, while the three cases of serious adverse events experienced by Paxlovid recipients (blood loss anemia, forearm fracture and melanoma) were judged to be unrelated to the drug treatment. 

“One reasonable implication of our results is that Paxlovid can be given safely for a longer period of time,” Singh said, “for example, in instances where a newly infected patient is immunocompromised.”

The challenge of being first

The apparent absence of a clinical response in the recent trial doesn’t rule out the possibility that Paxlovid might be found to reduce long-COVID symptoms in some people, Singh said, remarking that too many questions remain unanswered: “Should we have tested patients with symptoms present after only seven or eight months instead of 16 or 17? Should we have treated them for a longer time? Were we even testing the right patients? Maybe only some symptoms will be responsive to antiviral treatment.” 

If long COVID is not a single entity, the required treatment could differ among people exhibiting different clusters of symptoms.

Even routine testing of participants for the initial or post-treatment presence of cryptic SARS-CoV-2 — which by definition would be tucked away out of reach of the immune system and, perhaps, simple blood tests — was a nonstarter, Singh said, because simple, noninvasive, cheap tests are nonexistent.

Intriguingly, on average both the intervention- as well as placebo-arm participants’ overall symptoms improved over the course of the study. That could be good news, indicating that long-COVID symptoms generally subside with time. On the other hand, there’s the possibility of a placebo effect: The extra attention and concern from nurses, doctors and other health-provider types, as well as the hope of relief from the drug, may nudge placebo recipients to sense subjective improvement — or even to experience objective improvement — in their condition.

Plus, the absence of validated objective biomarkers made it tough to tell if one person’s reported improvement was bigger than another person’s. 

Results from a number of chemical tests, immunological tests and wearables measurements taken throughout the trial may help the Stanford Medicine team find out whether some people benefited considerably more than others from Paxlovid and, if so, how they might be distinguished in advance of treatment or enrollment in a new trial. 

“We hope to report on our analyses of these measurements in four to six months,” Singh said. The results may inform other clinical trials, now underway or in planning stages, of Paxlovid and other drugs’ capacity to counter long-COVID symptoms. These trials are sponsored by the National Institutes of Health as well as by Pfizer, Inc., Paxlovid’s developer, manufacturer and marketer.
