Visitors who wish to stay longer are required to have a visa.
- Foreign nationals from Afghanistan, Bangladesh, India, Iran, Iraq, North Korea and other countries not in the list are required to have a visa even if the stay is less than 7 days.
- Holders of Brazilian and Israeli passports can enter without a visa and stay for a period not exceeding 59 days.

Medical Tourism in the Philippines
The Philippines is one of the top travel destinations in Southeast Asia, but it’s also fast becoming one of the top destinations in the world for medical tourists.
Ranked 8th in the world for being a top medical tourism destination, Philippines caters to about 250,000 foreign patients or clients each year. According to a 2015 list compiled by global non-profit association for medical tourism and international patient industry, the International Healthcare Research Center and Medical Tourism Association, the Philippines garnered the 8th spot based on providing the most suitable healthcare cost, culture, security, and quality in terms of medical healthcare.
As early as 2006, the Philippines had begun its push to promote medical tourism in the country thanks to a bill that was passed in the Philippine Congress. Three branches of the country, which are the DOH (Department of Health), DOT (Department of Tourism), and the DTI-BOI (Department of Trade and Industry – Board of Investments) worked together to create a roadmap that would put the country on the global map for medical tourism.
Their efforts have been fruitful with the country being one of the top destinations for medical tourism today.
Aside from the efforts of the government, as well as the private institutions, general factors that have contributed to the rise of medical tourism in the Philippines include:
- the Filipinos’ innate nature of being warm and hospitable;
- English as a second language of use in the country and the general public being able to speak the global language;
- the tropical climate that promotes healing and relaxation after procedures;
- the highly trained medical practitioners with many of them having had trained or studied internationally;
- state of the art medical equipment;
- and of course, the lower costs as compared to Western nations.
What are the Common Treatments Done?
Most foreign patients who go to the Philippines to do aesthetic and non-complicated procedures. The most common treatments done include procedures under the following:
- cosmetic surgery
- aesthetic surgery
- weight loss surgery
- rehabilitation
- stem cell therapy
- eye surgery and ophthalmology
- fertility treatments
- dermatology
How Much Do Medical Treatments Cost in the Philippines?
One of the most attractive features that draw foreigners to the Philippine health system is the significantly lower cost of procedures, as well as lower costs of accommodation as compared to other top medical tourism destinations.
Here are some examples of medical procedures cost in the US, and how they compare to the prices in the Philippines:
Liver Transplant – May cost between $290,000 to $310,000 in the US, only costs between $120,000 to $150,000 in the Philippines. That makes about 51% to 58% in savings.
Kidney Transplant – May cost between $200,000 to $250,000 in the US, only costs between $23,000 to $25,000 in the Philippines. That makes about 88% to 90% in savings.
Coronary Artery Bypass Surgery – May cost between $70,000 to $133,000 in the US, only costs between $11,500 to $17,500 in the Philippines. That makes about 83 to 86% in savings.
Breast Augmentation – This procedure may cost between $7,500 to $8,500 in the US, but breast augmentation in the Philippines only costs between $3,000 to $3,500. That makes about 58% to 60% in savings.
These are just some comparisons of cost between the country’s prices in terms of procedures. As you can see, significantly lesser costs are offered in the Philippines, making it an attractive alternative for many patients or clients who may not have the financial capacity to carry on such procedures in their home country.
Other patients or clients also opt to have their medical procedures done in the Philippines so they could travel at the same time. Along with low medical costs, the Philippines is a top travel destination with a tropical climate, which is perfect for anyone who wishes to heal and relax right after their surgery or procedures.
Malpractice and Liabilities
The country’s constitution has enough laws to ensure the safety and security of patients and clients at the hands of practitioners and hospitals. There are also adequate provisions in the Penal Code for medical malpractice. These laws and provisions make sure patients and medical clients are protected against negligence and incompetence from medical practitioners.

One may not think of the Philippines as the top destination for medical treatments but recent surveys and reports will make you think otherwise. The Healthcare System in the Philippines
There’s not much to say about the public healthcare system in the Philippines, except for the fact that it is indeed one of the lowest quality in the region. Its private healthcare system, however, is on a completely different level.
In fact, the country has a total of 5 internationally accredited hospitals, which are: The Medical City, Makati Medical Center, Asian Hospital and Medical Center, St. Luke’s Medical Center in Global City, and St. Luke’s Medical Center in Quezon City. All hospitals are located in Metro Manila, and all are certified as institutions for providing excellent health care by JCI, or the Joint Commission International. JCI is an accreditation and certification body with over 100 member countries worldwide, and it also happens to be the only accreditation body for patient safety and quality healthcare.

Philippines to boost medical tourism with strategic global partnership

The Philippine Department of Tourism (DOT) has recently joined forces with Dubai-based Agora Group, a global leader in the medical and wellness industry, to position the country as an emerging medical and wellness tourism destination in Asia.
Both will also partner for the first International Health and Wellness Tourism Congress (IHWTC) 2023, which will be held from June 8 to 9 in Dusseldorf, Germany. This two-day event is a high-level closed-door gathering of invitation-only tourism stakeholders mostly from the medical and wellness industry.
Tourism secretary Christina Garcia Frasco commented that the partnership will help strengthen the Philippines’ position as a health and wellness tourism hub in Asia, as well as globally. She also emphasised how special the Philippine medical healthcare workers are, primarily the Filipino diaspora.
“We are giving the Philippines a fighting chance at becoming a tourism powerhouse in Asia. We recognise that medical tourism and wellness tourism hold one of the keys to this endeavour, because we have the people, we have the facilities, and we are adjusting government policies to ensure that the climate for medical tourism to thrive will ensue,” she said.
Competing with other countries like Saudi Arabia and Qatar with prime healthcare facilities, Frasco acknowledged that while those countries already have the “wealth of medical facilities”, however, “the culture of care and compassion shown by Filipinos who are in these facilities give us the strategic advantage that people from the Middle East are already familiar with the level of care that Filipinos can give”.
In addition, 23 of the country’s health facilities are internationally accredited and recognised by the International Society for Quality in Health Care, and there are 63 private hospitals in close proximity to four international airports.
The DOT is targeting to train 100,000 tourism front-liners this year under its Filipino Brand of Service Excellence programme, and with the IHWTC 2023 later this year, this would open massive opportunities for the country’s health and wellness offerings and to market the Philippines, especially from the most coveted Middle East tourist market.

The emerging medical tourism industry in the philippines.
For the past decade, the travel and tourism industry continues to have a significant impact on the Philippines’ economic growth. According to the latest report by the World Travel and Tourism Council (WTTC), the industry has contributed a total of Php 1.43 trillion to the nation’s economy in the previous year. This is equivalent to about 10.6 percent of the country’s gross domestic product (GDP). The figure, creating another promising economic growth engine, is also forecasted to rise by 6.6 percent this year and an additional 5.4 percent increase by 2026.
As a tropical archipelago brimming in biodiversity and rich culture, destinations in the Philippines have been commonly visualized as beaches, underwater gardens, mountains, and historical and cultural sites. But apart from natural and cultural awes, the Philippines has become the prime destination for medical tourists. And our country has been gaining international popularity for the past few years.

Traditionally, people from underdeveloped countries have no choice but to seek medical treatments in highly developed countries due to the lack of availability in their country. But because of the globalization of medical technology, the present situation has reversed, especially when clients get the same–or in many instances, even better quality medical procedures and services at a relatively lower cost-condition.
When it comes to the medical tourism industry, the Philippines generated $66 million in 2013 but tremendously grew to $145 million in 2014, according to the 2014 study by Ian Youngman, a medical tourism author. Records also show that the country now serves approximately 80,000 to 250,000 patients annually.
If asked, would you believe if you were told that the Philippines actually belongs to the list of the top ten most popular countries in the global medical tourism industry?
According to the list of the International Healthcare Research Center and the Medical Tourism Association (MTA), the Philippines now ranks 8th among the top medical tourism destinations in the world—ranking even ahead of developed countries and economic superpowers like Japan and France.

MTA, a non-profit global association for the medical tourism and international patient industry, works with healthcare providers, governments, insurance companies, employers, and other buyers of healthcare–with a primary focus on providing the highest quality transparent healthcare.
Popular Medical Treatments in PH Among the most popular services offered in the Philippines are surgeries, therapies, dental services, cosmetic services, and other medical treatments.
In recent years, unknown to most Filipinos, people have already been traveling to the Philippines for medical treatment before the country even became a popular healthcare destination.
According to a recent study released by Transparency Market Research, the cost of treatment in developing medical tourism countries ranges from 20% to 30% less than the cost in the United States—allowing the patient to save up to 30% to upwards of 85% on medical services.
Because of the low cost of living in the Philippines compared to other developed nations, the country offers relatively less expensive medical rates for expatriates and tourists.
Also, the leading hospitals in the country offer state of the art medical equipment that are on par with top institutions in other nations like the US and Europe. In fact, five hospitals have been certified for excellent medical care by the Joint Commission International (JCI), one of the most prestigious healthcare accrediting bodies in the United States.
These medical institutions include: 01 Asian Hospital and Medical Center, Muntinlupa City 02 Makati Medical Center, Makati City 03 St. Luke’s Medical Center, Bonifacio Global City 04 St. Luke’s Medical Center, Quezon City 05 The Medical City, Pasig City
At the same time, 62 hospitals around the country are now internationally accredited according to the Department of Tourism (DOT), Department of Health (DOH) and Department of Trade and Industry – Board of Investments (DTI–BOI)’s latest report.
Also, foreign tourists do not have to worry about language barriers in the Philippines like other countries in Asia. English is widely spoken–not only by medical professionals but by almost 90 percent of the Filipino population. Aside from that, Filipinos are known for their warm hospitality and compassionate attitude. This kind of treatment comes free of charge and is now considered to be one of the country’s best innate assets.

The Challenge Even though the Philippine medical industry is still maturing, we are being recognized by the international community. This is definitely something to be proud of. However, this industry is mainly captured by the private sector. As Filipinos, our challenge lies here.
People from the lower class of the economy–even in the middle class–finds private hospitals too expensive so they turn to public medical facilities. But what’s so bad about the public medical facilities?
To be clear, public hospitals and health centers do not necessarily mean bad services. There are many jewel public hospitals such as the Heart, Lung and Kidney Centers and the Philippine Children’s Medical Center. Still, we cannot deny that public health facilities are generally perceived to provide lower quality health services than private ones.
Back in 2006, the Philippine Medical Tourism Program was launched through the Departments of Health, Tourism, Trade and Industry, and Foreign Affairs. This program aims to ensure that the Philippines is globally competitive through implementation of quality standards in both public and private sector.
To realize our full potential as the best medical destination in the World, the country must promote and create partnerships in the private and public health sectors. That way, more Filipino citizens can also appreciate, enjoy, and afford quality health care services—which they deserve to begin with.

Medical Tourism pushed by Marcos Administration with strategic global partnership – DOT Chief

The Philippines is positioning the country as an emerging medical and wellness tourism destination in Asia with the Department of Tourism’s (DOT) recent partnership with a global leader in the medical and wellness industry.
The tourism chief made the announcement on Friday (May 5) as the DOT formally inked an agreement with Dubai-based Agora Group, the “pioneer and leader in providing business solutions and productivity in the health and wellness tourism”, to be a partner for the prestigious and high-level gathering of tourism stakeholders and medical professionals at the First International Health and Wellness Tourism Congress (IHWTC) 2023. The event is slated for June 8 to 9, 2023 in Dusseldorf, Germany.
The agreement was formalized by Secretary Christina Garcia Frasco and Agora Co-Founder and Chief Executive Officer (CEO) Hadi Malaeb, at one of the country’s leading medical facilities, The Medical City in Pasig.
In her remarks, the tourism chief emphasized that the event is a manifestation of how the DOT, under the Marcos Administration, conveys its “seriousness” in prioritizing medical tourism as a marketable product around the world. In a bid to strengthen the Philippines’ position as a health and wellness tourism hub in Asia, as well as globally.
“We are giving the Philippines a fighting chance at becoming a tourism powerhouse in Asia. And we recognize that medical tourism and wellness tourism hold one of the keys to this endeavor, because we have the people, we have the facilities, and we are adjusting government policies to ensure that the climate for medical tourism to thrive will ensue,” she said.
Philippines, a viable global destination for medical and wellness tourism
The Philippines’ intent to sponsor the global congress is anchored on the current strategic position and potentials as a “serious regional contender.” This, as the DOT is currently operating under the 7-point agenda embodied in the National Tourism Development Plan for 2023-2028, particularly, the diversification of product portfolio through multidimensional tourism, thus, the aggressive push for the development of a highly competitive health and wellness tourism product.
In fact, the Philippines has 23 health facilities internationally accredited and recognized by the International Society for Quality in Health Care (ISQua). In terms of ease of access to care, there are 63 private hospitals in close proximity to four international airports with almost no waiting time for appointments and treatment. These, apart from the Philippines’ cost of treatments that are competitively lower compared to Western countries and other countries within Asia.
The Philippines is also one of the biggest English-speaking countries in the world where hospitals employ English-speaking healthcare workers and are mostly recognized by foreign insurance companies.
Secretary Frasco then emphasized that the partnership with Agora is a welcome development for the industry as it conveys great confidence in the potential of the Philippines as a next emerging medical health and wellness destination in Asia.
With the Philippines’ natural wonders that have been recognized numerous times not only in Asia but in the global arena, the Secretary also added that traveling visitors can “heal after their procedure” on the beach, in the mountains, and they can also enjoy delicious Filipino food and the distinct brand of Filipino hospitality. In the case of medical tourism, the Philippines is also offering the signature Filipino “hilot” at various tourism-accredited resorts and spas in the country.
“We are also looking at our islands, including Boracay, as a potential destination for Filipino wellness as well as CALABARZON that have a host of wonderful offerings in terms of wellness,” the tourism chief added.
She also puts emphasis on how special the Philippine medical healthcare workers are, primarily the Filipino diaspora.
“Everywhere that I’ve been in the world since becoming Secretary of Tourism and I say this all the time. I’ve always heard a positive story about a Filipino worker abroad and mostly it’s about doctors, nurses, and caregivers, stories from foreigners who have felt the compassion, the care, the commitment to duty, and the willingness to go the extra mile to help those in need. That diaspora is very well replicated in all our medical personnel here in the Philippines, wherever you may find yourself in our over 7,000 islands and that is why we are very confident in pitching for the Philippines as the next emerging medical tourism and wellness destination in Asia,” she said.
Further, she emphasized that under the Marcos Administration, the focus is to improve the essential pillars of tourism development such as the building of tourism infrastructure, enhancing connectivity, and digitalization of processes recognizing as well the need to further enhance the availability of medical tourism facilities across the country.
Apart from hard infrastructure, the DOT is also improving soft infrastructure by embarking on an ambitious goal to train 100,000 tourism frontliners this year under its Filipino Brand of Service Excellence (FBSE) program.
More opportunities for PH tourism workers
In a media interview, Secretary Frasco expounded that the partnership with Dubai-based Agora Group for the International Health and Wellness Tourism Congress would also open massive opportunities for the country’s health and wellness offerings and to market the Philippines, especially from the most coveted Middle East tourist market.
Asked specifically how the Philippines will attract more visitors for this aspect as it competes with other countries like Saudi Arabia and Qatar with prime healthcare facilities, the tourism chief recognized that those countries already have the “wealth of medical facilities.” But she said what gives the Philippines an advantage is “the culture of care and compassion shown by Filipinos who are in these facilities give us the strategic advantage that people from the Middle East are already familiar with the level of care that Filipinos can give.”
Pursuing Medical Tourism
In 2004, President Gloria Macapagal Arroyo through Executive Order No. 372 identified service industries which the Philippines has the potential to be globally competitive which included health and wellness, among others.
In pursuit of the objective of the DOT to diversify its portfolio, Secretary Frasco underscored the potential of medical tourism in ensuring the growth of the Philippine tourism industry.
“When I first came into the fold of the department, one of the things that we discussed was how we can engage with our partners in the medical tourism and wellness sector. The work towards that has been through collaboration with our medical tourism partners, our wellness providers, as well as our government agencies involved in ensuring that the policies that are in place to fashion this as a marketable product around the world are in such a manner that it would ensure the growth of this industry,” she added.
In its initial presentation to the DOT, Agora Group Dubai estimated the global health and wellness tourism to be worth $45.5 to $72 billion, with 14 to 16 million cross border patients and an average spend of $3,800 to $6,000 per visit.
International Health and Wellness Tourism Congress (IHWTC) 2023
The two-day IHWTC 2023 stands to be an important platform for promoting the health and wellness tourism offerings of the Philippines which is also a high-level closed-door gathering of tourism stakeholders mostly from the medical and wellness industry who will come only by invitation, and with an expectant 100 corporate buyers with a combined budget of $2 billion.
“The signing of this partnership with Agora is one that we welcome with great hope and optimism knowing that it’s been in the pipeline for 20 years,” she said, adding that this would also open up massive opportunities for the country’s health and wellness offerings and to market the Philippines especially from the most coveted Middle East tourist market.
For his part, Malaeb expressed his gratitude to the DOT for sponsoring the global event, citing that he himself has expressed the world-renowned Filipino brand of hospitality.
“Thank you all for the effort and the welcoming of hospitality that I have witnessed since I arrived here. Huge thank you to the Department of Tourism for being the title sponsor of the event,” Malaeb said.
“I am very proud to be a partner of the Department of Tourism of the Philippines in order to promote the destination as a medical and wellness destination to some of the best and largest buyers of that product and service in the world,” he added.
The Medical City President and CEO Dr. Eugenio Jose Ramos also thanked both parties for choosing the hospital as the venue for such momentous event.
“It a pleasure and honor that you have chosen The Medical City as the institution that would sponsor this Memorandum of Agreement signing between the Philippines, through the Secretary and Mr. Hadi,” he said.
The event was also witnessed by DOT OIC-Undersecretary for Tourism Development Verna Buensuceso, Office of Product and Market Development (OPMD) Director Paulo Benito Tugbang, and Agora Group Philippine Representative Angel Bognot.

Medical tourism facilitators walk a tightrope between ethics and business

A new market of intermediary fixers for medical tourists say that they can lower the risks associated with seeking treatment overseas. Charu Bahri asks what these facilitators do—and who oversees them?
When Marie Gopaul,* a 71 year old Mauritian, needed a hip replacement she and her family had concerns about doing it locally. “Doctors told me that the outcomes of robotic hip replacement are the best,” says Marie’s son Louis.* “In Mauritius many hip replacement patients who are operated on conventionally struggle to walk postoperatively.”
Because robotic surgeries weren’t available in Mauritius the family decided to pay for Marie to have surgery abroad—becoming one of the hundreds of thousands of patients who choose to become medical tourists in a market projected to grow from US$24.14bn (£19bn; €22.24bn) in 2023 to $93.38bn by 2030. 1 They settled on India, and for assistance in choosing a doctor and a hospital Louis approached an online medical tourism facilitator recommended by his friends.
Medical tourism facilitators manage and coordinate all activities related to the care of patients and their treatments while travelling abroad, such as visa applications, hotel stays, and airport transfers, although patients normally pay separately for flights. 2 They function as intermediaries between medical tourists (some also work with domestic patients) and a wide network of hospitals.
Brokers or middlemen first emerged in the medical tourism market in India around 2010. They faced resistance from the leading Indian hospital chains, which tried to get them banned with assistance from the Federation of Indian Chambers of Commerce and Industry. 3 But the intermediaries proved steadfast, and eventually India’s leading health standards body, the National Accreditation Board for Hospitals and Healthcare Providers (NABH), officially recognised them under the title medical value travel facilitators.
The NABH introduced “empanelment,” 4 a form of accreditation where inspectors evaluate a facilitator’s services and grant empanelment status if they pass. But empanelment isn’t synonymous with regulation. There are still documented instances 5 6 of domestic patients filing a medical negligence complaint with the police or speaking out on social media against an empanelled facilitator. And, when treatments fail, the NABH has no role to play.
The absence of stricter regulation, as well as concerns over the veracity of information displayed on facilitators’ websites and whether they follow ethical guidelines, 7 has led some critics to warn that facilitators “highly emphasise benefits while downplaying the risks.” 8
Facilitators say that they help prospective patients understand their options. “It’s difficult to choose the right doctor when they all claim to be the best, especially when you don’t speak the local language,” says Louis, when asked why he didn’t approach a hospital directly. “What clinched it for me this time was the facilitator’s counsellor telling me that she would choose the same renowned orthopaedic doctor in Delhi for her own mother.”
Through facilitators patients can get access to treatment options at different price points, something a single hospital can’t deliver. Abhik Moitra, director at HBG Medical Assistance, a leading Indian facilitator, says, “We can offer a Whipple procedure [pancreaticoduodenectomy to remove cancerous tumours from the head of the pancreas] at $12 000 at a top hospital in a leading metro, or at $8000-$9000 in a smaller city.”
Facilitators in India have told The BMJ that they’re conscious of the hospitals they work with, preferring those accredited by the Joint Commission International and the NABH. They also look for hospitals that demonstrate a managerial commitment to international patients and those that have invested in cutting edge technology.
Beyond physical amenities, medical tourism facilitators can support patients’ more personal needs, such as language barriers and settling in. In emerging markets especially, they “ease exposure in unfamiliar settings by dealing with medical and tourism providers.” 9 Louis believes that medical tourists who don’t speak the local language “tend to get fleeced” without a service provider.
Benjamin Hunter, lecturer at the University of Glasgow and an expert on the global healthcare economy, says, “Desperate people, unfamiliar settings, and commercialised healthcare are a recipe for exploitation. Facilitators can ameliorate some of this—or exacerbate it.”
Critics argue that facilitatory service providers work within an absent or limited regulatory framework 10 that limits their responsibility if things go wrong. Treatment failures can burden the patient, as well as public health services in the patient’s home country. 11 For that reason the UK, among others, has drawn up guidelines for prospective medical tourists. 12
The UK’s NHS guidelines on going abroad for medical treatment 13 include a strong recommendation for patients to consider continuity of care and inquire about any compensation for negligence. This can vary by provider. Some Lithuanian hospitals, for instance, promise free further treatment if anything goes wrong, 14 whereas one Lithuanian private hospital, Nordorthopaedics, told The BMJ that only a part of the cost would be waived if the patient needed further intervention.
Likewise, using a facilitator in India doesn’t necessarily add any guarantees. Pankaj Chandna, cofounder of Vaidam.com, a leading facilitator in India, agrees that “a few patients need revision surgery” but says that his company “tries to support such patients as much as possible to alleviate their mental trauma and financial burden.”
Chandna’s company runs a programme called Vaidam Reconnect, “to stay in touch with patients and assign a case manager to those who need further assistance.” He says that recently a patient from Sierra Leone needed corrective surgery after an operation. “Our partner hospital waived the surgical fee and 70% of the medical charges,” says Chandna.
Heidi Kaspar of the Department of Health Professions at the Bern University of Applied Sciences in Switzerland has introduced the notion of “therapeutic itineraries” in medical tourism because she sees medical journeys as “circular movements” rather than “singular and straightforward trips from A to B.”
“Numerous uncertainties [and] inadequate diagnostics at home complicate treatment abroad, prolong the stays of patients, and require family caregivers to return home in order to arrange funding or to organise absence at home,” she says. “On top of this, aftercare and unpredictable illness trajectories when back home lead to replicate journeys.”
Conflicts of interest
Perhaps the biggest concerns about facilitators are their fairness and competence 15 when choosing appropriate doctors and hospitals—and the possibility of pushing unnecessary treatment 16 that’s not in the best interest of patients, just to make a sale.
Facilitators The BMJ spoke to in India said that they don’t charge patients, but they do earn a facilitation fee or marketing fee from the hospitals that treat their referred patients. These payment arrangements differ by hospital. “Some agreements cover outpatient consultations, others only hospital admissions,” says Moitra. Some agreements extend to covering marketing expenses, including outreach programmes where medical value travel facilitators can help advertise hospitals open to private referrals by arranging consultation programmes in partner hospitals in target countries.
Such arrangements stem from the “cut system” in the Indian healthcare sector that the government has yet to legislate against. 17 18 Hunter says, “Opaque commissions breed mistrust, but ultimately facilitators are only one component in a bigger system, and exploitation is not unique to medical travel: the problem is profit driven healthcare.”
Other countries in the region have stricter rules to combat this. Asia’s leading medical tourism destination is Singapore, and its health ministry doesn’t allow doctors and hospitals to pay for referrals, says Nakul Singh, who represents Mount Elizabeth Hospital in India and manages India operations at IHH Healthcare’s Singapore division. Singaporean hospitals open representative offices in overseas target countries to counsel patients, he adds.
Last year around half a million medical tourists entered India, and the central government launched a campaign, Heal in India, to boost medical tourism. Prasanna Saligram, a public health researcher with the People’s Health Movement India, believes that the number isn’t yet high enough to push the government for further regulations, particularly as it “has so many larger issues to address.” He points to the disparity between the cost of healthcare in public and private hospitals.
“Our health sector regulatory framework is very weak,” he explains. “The Indian healthcare story is about ‘globalised’ and ‘marginalised’ patients. Private hospitals thrive on the globalised Indian, high net worth individuals, and medical tourists are just the cherry on top, while poorer people struggle to access healthcare.”
Facilitator driven outreach
Santosh Shetty is chief executive officer of Kokilaben Dhirubhai Ambani Hospital in Mumbai. His hospital is one that works with facilitators, whose services he appreciates when they work within an organised framework and in the patient’s best interests.
“Facilitators with a strong network and presence in the target geographies can help get the necessary licences and permissions for events and can guide local patients,” says Shetty. “Their offices can sometimes also serve as a local centre, which is very reassuring to patients and can overcome language barriers.”
He says that facilitators referred 20-25% of his hospital’s 2600 international patients in 2023. “While we have a dedicated international patient department, our global reach is amplified through credible facilitators,” says Shetty.
“Credible” is the key word. While Louis was appreciative of the facilitator’s team (one of 26 companies empanelled with the NABH 4 ) and the surgeon, he wasn’t blind to gaps in the service. “Our stay in a one star hotel left a lot to be desired,” he says. “I know you get what you pay for, but we could have been better guided on that front.”
In the hospital, says Louis, his mother had a long wait before the first consultation despite having an appointment, and he was charged 65 rupees (about 60p) for every cup of tea he ordered while his mother was in the intensive care unit. “When you’re paying more than double the cost of treatment in your home country you expect better service,” he says. (The hip replacement cost him $6500, which compares with $2500 at a private clinic in Mauritius.)
Nevertheless, the family got what they wanted: Marie had her surgery in January 2024. “The doctor delivered exactly as he had promised,” says Louis. “He had said that Mother would be walking independently a fortnight after being operated on, and she was.”

What is medical tourism?
Medical tourism is when you go overseas for medical treatment.
Many Australians travel overseas for health care to save money. Some go for treatments that are unavailable in Australia.
The most common procedures Australians go overseas for are:
- cosmetic surgery
- dentistry
- heart surgery
- fertility treatments
- surrogacy
- gender affirming surgery
- stem-cell therapy
- cancer treatments.
What to think about before travelling for a medical procedure
Before you go, research the destination , doctor, hospital and procedure. This helps you reduce the risk of things going wrong.
Talk to your doctor in Australia about your plans. Ask for their advice. Get a health check at least 6 weeks before you go. Make sure you've got appropriate vaccinations .
Choosing a destination
Research the health system of your destination. The quality of care may not be what you're used to in Australia.
- Read your destination's travel advisory . See the 'Health' section for advice about local healthcare.
- Minimum healthcare standards in some countries can be low. Standards can differ between regions, hospitals, and medical professionals within a country.
- Learn about your legal rights in your destination. Find out if you can take legal action against the hospital and surgeon if things go wrong.
Choosing a hospital
Many hospitals overseas are a similar standard to hospitals in Australia. But quality and standards in some countries can be poor. Some may have low training standards for doctors and nurses. Others may have high rates of infection and complications.
- Find out if the hospital is accredited by the local authorities. Don't go to an unofficial medical facility.
- See if there are reports of antibiotic-resistant bacteria in the hospital. These infections may not be treatable.
- Compare the complication risks they give you to those mentioned in Australia. Be wary if a hospital overseas suggests much lower risks.
Choosing a doctor
Check if the surgeon who will do your procedure is qualified and accredited. Most countries have this information on an official website.
- Find out the reputation of the doctor and the experiences of past patients.
- In some countries, doctors and hospitals don't have to get professional insurance. Make sure your surgeon and hospital have indemnity and malpractice insurance.
Preparing for if things don't go to plan
There are always risks with medical procedures. You could suffer from complications, or you could die . Even if you're young, fit and healthy.
- Take someone with you. You'll need their support if things go wrong. They may need to make decisions for you.
- Make sure your will is up to date.
If your procedure goes wrong, you may end up with a new or worse problem. It could cost more to fix it. Make sure your travel insurance covers complications and medical evacuation.
The cost of medical treatment overseas
In some countries, elective procedures can cost much less than in Australia. But make sure you consider all possible costs.
- If you have a complication during or after the procedure, you may have to pay more to fix it. Make sure your estimate also covers this.
- Your travel insurance may not cover issues caused by your procedure. Check your policy.
You're responsible for the cost of your treatment overseas. You, your family or your travel insurer will have to pay if you need further care or a medical evacuation.
- Medicare doesn't cover you when you're overseas.
- Reciprocal health care agreements only cover some situations in some countries.
- You're not likely to get subsidised care through your destination's public health system.
The Australian government can't pay your bills or loan you money.
Medical tourism and insurance
Basic travel insurance policies rarely cover medical tourism. Get a specialised policy that covers the costs of your specific procedure. Some offer this as a paid extra on their policies.
Ask your insurer about:
- post-operative care
- complications
- medical evacuation.
Be open about your plans. You may void your policy if you don't tell them everything.
Know what you're covered for and what's excluded. Always read the Product Disclosure Statement (PDS).

Australian private health insurance for medical tourism
Some private health insurers in Australia have arrangements with hospitals overseas. They may give you the option to have a medical procedure overseas. Some may cover your travel expenses and medical fees. They may help organise your trip.
Tell them about your plans. Ask your insurer if they:
- will cover your procedure and travel costs
- have a list of hospitals and surgeons overseas that they endorse
- offer a travel insurance policy that covers you if things go wrong with your procedure.
Even if your health insurer won't cover you, always tell them about your plans. If you don't, you may void your insurance policy if you suffer a complication.

- Parents who frequently cross the border by land with a minor must always carry a letter of permission from the other parent.

Ports of entry in many countries have security measures to prevent international child abduction . If you are traveling alone with your child, you may be required to present documentation proving you are the parent or legal guardian. You may also need a letter of permission from the other parent for your child to travel.
If your child travels alone, depending on the country, they may be required to present a notarized letter from both parents or their legal guardian. If a minor is traveling abroad and is not accompanied by both parents or a legal guardian, contact the embassy or consulate of the country you will be visiting and ask about entry and exit requirements for that country.
