Recent Trends in Community Health Center Patients, Services, and Financing

Key Takeaways

Community health centers are a national network of over 1,300 safety-net primary care providers, serving more than 30 million patients in 2022. They are located in medically underserved urban and rural communities and serve all patients regardless of their ability to pay. Health centers also played a major role in the nation’s response to the coronavirus pandemic, particularly for hard-to-reach populations. This brief analyzes the changes in health center patients, services, and financing from 2019 (pre-pandemic) through 2022 using the Uniform Data System (UDS), to which all health centers are required to report annually . Key takeaways include the following:

  • The health center patient population has returned to pre-pandemic levels after dipping during the first year of COVID and is up slightly from 29.8 million patients in 2019 to 30.5 million in 2022 (2%). But the number of children served by health centers decreased somewhat from 9.2 million to 8.8 million (4%) during the same period, with the greatest decline among children ages 0-5. There is evidence indicating that utilization of primary and preventive services among children on Medicaid remains below pre-pandemic levels, which may partially explain the drop in pediatric patients at health centers.
  • Health centers disproportionately served communities of color and low-income people—63% of patients were people of color and over 90% had income that was at or below 200% of the federal poverty level in 2022.
  • More people were returning to in-person care since the start of the pandemic, with over 105 million health center patient visits conducted in-person in 2022 as compared to 85.7 million in 2020, but telehealth was still utilized in 17% of all visits in 2022.
  • From 2019 to 2022, the number of visits for mental health and substance use disorder services increased 21%, with large increases in number of patients diagnosed with anxiety disorders (26% increase) and attention deficit disorders (21% increase). The number of patients receiving medication-assisted treatment (MAT) for opioid use disorder increased 36% from 2019 to 2022.
  • From 2019 to 2022, the share of health center patients who are uninsured dropped from 23% to 19%, while the share of patients covered by Medicaid increased from 49% to 51%, likely due to the Medicaid continuous enrollment provision that was in place from March 2020 through March 2023.
  • Medicaid was the largest revenue source for health centers, providing 42% of all revenue in 2022 while federal grant funding comprised 12% of total funding. Health center revenues increased by over $11 billion since 2019, primarily due to growth in Medicaid funding related to the increase in Medicaid patients and COVID-related funding.

The unwinding of the Medicaid continuous enrollment provision will likely impact health center patients, services, and financing starting in 2023 as people who are disenrolled from Medicaid enroll in other coverage or become uninsured. KFF tracking indicates that more than 20 million people have been disenrolled from Medicaid since the start of the unwinding. Early research conducted using health center claims data found that 17% of patients who were covered by Medicaid prior to the start of unwinding were uninsured when they sought care at a health center during the first six months of unwinding. Among those with the highest Medicaid disenrollment rates were patients with HIV/AIDS, mental health conditions, or SUDs. While Congress recently reauthorized increased federal grant funding for health centers through 2024, the increase may not fully offset any drop in Medicaid revenue during the unwinding. The expiration of pandemic-related funding could further stress health center finances.

Health Center Patients

The number of patients served by served by health centers increased in 2022, although the number of child patients has not fully rebounded to pre-pandemic levels. Following temporary site closures and social distancing guidance at the start of the coronavirus pandemic in 2020, the number of patients dipped, but in 2021, the number of patients began increasing again and that trend continued in 2022. Health centers served 30.5 million patients in 2022, an increase of 2% from 2019 (Figure 1). Although the number of adult patients increased, the number of child patients decreased by 4% and remains below the number of children served in 2019.

Health centers served fewer children ages five and under in 2022 compared to 2019, driving the overall drop in pediatric patients. From 2019 to 2022, the number of children 0-5 visiting health centers decreased 14% from 3.2 million to 2.7 million (Figure 1). The largest decline in the 0-5 age range was among children under age 1, which declined from 688,000 patients in 2019 to 471,000 in 2022 (a 32% drop). There was a smaller decrease in the number of patients ages 6-11 (3%), while children ages 12 and over saw a 5% increase over pre-pandemic patient numbers.

Most health center patients were people of color, and the vast majority were low-income. In 2022, people of color represented 63% of health center patients and Hispanic patients comprised the largest share of patients at 39% (Figure 3). Health centers also served many patients with limited English proficiency, with roughly one in four patients (26%) who were best served in a language other than English. Reflecting their role as safety-net providers, nine in ten health center patients had incomes that were at or below 200% of the federal poverty level (FPL) and two-thirds (66%) had incomes at or below 100% FPL in 2022 (Figure 3).

Health Center Services

Even as health center patients returned to in-person care in 2022, reliance on telehealth continued. Health centers provided a total of 126.9 million visits in 2022, including 21 million telehealth visits (Figure 4). Telehealth visits represented 17% of total visits, a substantially larger share than the 2019 baseline of less than 1%, but down from the high of 25% during the height of the pandemic in 2020. While in-person visits increased relative to 2020, they remained below pre-pandemic levels.

Mental health, substance use disorder (SUD), and medical services drove growth in health center visits from 2019 to 2022, while visits for dental and vision services remained below pre-pandemic levels. Overall, health center visits were 3% higher in 2022 compared to 2019. Visits for mental health and SUD services increased by 21%, with more modest growth in enabling services (9%) and medical services (4%) (Figure 5). Services that were largely provided in person, namely vision and dental, declined by 3% and 17%, respectively, over the same period.

Consistent with the increase in mental health and SUD visits, the number of health center patients diagnosed with certain mental health and SUD disorders increased in 2022. Compared to 2019, there was a notable increase in the number of patients experiencing anxiety disorders (26%) and attention deficit and behavior disorders (21%) in 2022 (Figure 6). In addition, the number of patients receiving medication-assisted treatment (MAT) for opioid use disorder increased 36% from 2019.

Health Center Revenue Sources

The share of health center patients who were uninsured continued to decline while the share enrolled in Medicaid surpassed 50%. From 2019 to 2022, the share of uninsured patients dropped four percentage points from 23% to 19% while the share of Medicaid patients increased from 49% to 51%, further solidifying Medicaid as the largest source of coverage for patients (Figure 7). Both of these changes are largely attributable to the Medicaid continuous enrollment provision , which temporarily halted Medicaid disenrollments from March 2020 through March 2023. After March 2023, states resumed disenrollments as part of the Medicaid unwinding .

Medicaid continued to be the largest source of health center revenue, though federal grants and COVID-related funding were also significant revenue sources in 2022. Medicaid accounted for 42% of health center revenue in 2022 while federal Section 330 grant funding, which supports health centers role as safety net providers, made up 12% of revenues at health centers nationally (Figure 8). Additionally, pandemic-related funding made up 8% of all health centers’ revenue in 2022 although this funding has since expired. Health center revenues increased by over $11 billion since 2019, primarily due to growth in Medicaid funding related to the increase in Medicaid patients and COVID-related funding.

Hi, my name is Nancy Baton. I am a retired high school principal and middle school principal. And I am truly enjoying my retirement.

I live with my husband, and I’ve been married to Bill for, it’ll be 38 years this summer. We have two children, grown children, and I have 5 grandsons.

When it came time for me to enroll in Medicare, I wanted the one with the least amount of hassles. And I had been told what a great health plan Aetna has. That’s why I chose Aetna and I’m very happy I did.

Every year Aetna offers the opportunity for a Healthy Home Visit. The nice part about the HHV is I don’t go anywhere. The nurse comes here to me. So I’m comfortable in my own environment. I don’t have to leave the house. And It’s just nice to have somebody come to you for a change.

It was a registered nurse and she spent about an hour and a half. Goes over my medications. Checks my blood pressure. And then said to me she thought I have a problem with circulation. So she did a test on me and my doctor called me and said you need to see a vascular surgeon. And I’m under his care. Now I never would have caught something that could be potentially very dangerous had that nurse not come to my home.

Having the support of Aetna and not having to worry about bills or paperwork or is this covered. I didn’t have any of that. All I do now is concentrate on my recovery.

Before your visit

Once you schedule your Healthy Home Visit through Signify Health, you’ll get a confirmation for your appointment. You’ll also get an email, text or call reminder 24 hours before your visit. If we don’t hear from you to set up your appointment, a member of the Signify Health team will reach out to help schedule your visit.

During your visit

On the day of your visit, a nurse will spend up to an hour with you in your home. They’ll answer health questions, address any concerns and more. This includes:

  • Checking vital signs and reflexes
  • Reviewing medical history
  • Going over medications
  • Checking breathing, eyes or feet
  • Perform preventive screenings 

After your visit

A summary of the visit will be shared directly with you and your doctor. Should anything with immediate concern come up, we help coordinate the care you need.

Complementary, Alternative, or Integrative Health: What’s In a Name?

We’ve all seen the words “complementary,” “alternative,” and “integrative,” but what do they really mean?

This fact sheet looks into these terms to help you understand them better and gives you a brief picture of the mission and role of the National Center for Complementary and Integrative Health (NCCIH) in this area of research. The terms “complementary,” “alternative,” and “integrative” are continually evolving, along with the field, but the descriptions of these terms below are how we at the National Institutes of Health currently define them.

According to a 2012 national survey, many Americans—more than 30 percent of adults and about 12 percent of children—use health care approaches that are not typically part of conventional medical care or that may have origins outside of usual Western practice. When describing these approaches, people often use “alternative” and “complementary” interchangeably, but the two terms refer to different concepts:

  • If a non-mainstream approach is used  together with  conventional medicine, it’s considered “complementary.”
  • If a non-mainstream approach is used  in place of  conventional medicine, it’s considered “alternative.”

Most people who use non-mainstream approaches also use conventional health care.

In addition to the terms complementary and alternative, you may also hear the term “functional medicine.” This term sometimes refers to a concept similar to integrative health (described below), but it may also refer to an approach that more closely resembles  naturopathy  (a medical system that has evolved from a combination of traditional practices and health care approaches popular in Europe during the 19th century).

Integrative health brings conventional and complementary approaches together in a coordinated way. Integrative health also emphasizes multimodal interventions, which are two or more interventions such as conventional health care approaches (like medication, physical rehabilitation, psychotherapy), and complementary health approaches (like acupuncture, yoga, and probiotics) in various combinations, with an emphasis on treating the whole person rather than, for example, one organ system. Integrative health aims for well-coordinated care among different providers and institutions by bringing conventional and complementary approaches together to care for the whole person.

The use of integrative approaches to health and wellness has grown within care settings across the United States. Researchers are currently exploring the potential benefits of integrative health in a variety of situations, including pain management for military personnel and veterans, relief of symptoms in cancer patients and survivors, and programs to promote healthy behaviors.

Whole person health refers to helping individuals, families, communities, and populations improve and restore their health in multiple interconnected domains—biological, behavioral, social, environmental—rather than just treating disease. Research on whole person health includes expanding the understanding of the connections between these various aspects of health, including connections between organs and body systems.

  • An NCCIH-funded study is developing an innovative, collaborative treatment model involving chiropractors, primary care providers, and mental health providers for veterans with spine pain and related mental health conditions.

The goal is to help patients feel and function better and reduce their need for pain medicines that can have serious side effects.

Complementary approaches can be classified by their primary therapeutic input (how the therapy is taken in or delivered), which may be:

  • Nutritional (e.g., special diets, dietary supplements, herbs, and probiotics)
  • Psychological (e.g., mindfulness)
  • Physical (e.g., massage, spinal manipulation)
  • Combinations such as psychological and physical (e.g., yoga, tai chi, acupuncture, dance or art therapies) or psychological and nutritional (e.g., mindful eating)

Nutritional approaches include what NCCIH previously categorized as natural products, whereas psychological and/or physical approaches include what was referred to as mind and body practices.

These approaches include a variety of products, such as  herbs   (also known as botanicals),  vitamins and minerals , and  probiotics . They are widely marketed, readily available to consumers, and often sold as  dietary supplements .

According to the 2012 National Health Interview Survey (NHIS), which included a comprehensive survey on the use of complementary health approaches by Americans, 17.7 percent of American adults had used a dietary supplement other than vitamins and minerals in the past year. These products were the most popular complementary health approach in the survey. (See chart.) The most commonly used nonvitamin, nonmineral dietary supplement was fish oil.

Researchers have done large, rigorous studies on a few dietary supplements, but the results often showed that the products didn’t work for the conditions studied. Research on others is in progress. While there are indications that some may be helpful, more needs to be learned about the effects of these products in the human body, and about their  safety  and potential  interactions with medicines  and other natural products.

Complementary physical and/or psychological approaches include tai chi , yoga , acupuncture , massage therapy , spinal manipulation , art therapy, music therapy, dance, mindfulness-based stress reduction, and many others. These approaches are often administered or taught by a trained practitioner or teacher. The 2012 NHIS showed that yoga, chiropractic and osteopathic manipulation , and meditation are among the most popular complementary health approaches used by adults. According to the 2017 NHIS , the popularity of yoga has grown dramatically in recent years, from 9.5 percent of U.S. adults practicing yoga in 2012 to 14.3 percent in 2017. The 2017 NHIS also showed that the use of meditation increased more than threefold from 4.1 percent in 2012 to 14.2 percent in 2017.

Other psychological and physical approaches include relaxation techniques   (such as breathing exercises and guided imagery),  qigong ,  hypnotherapy , Feldenkrais method, Alexander technique, Pilates, Rolfing Structural Integration, and Trager psychophysical integration.

Research findings suggest that several psychological and physical approaches, alone or in combination, are helpful for a variety of conditions. A few examples include the following:

  • Acupuncture  may help ease types of pain that are often chronic, such as low-back pain, neck pain, and osteoarthritis/knee pain. Acupuncture may also help reduce the frequency of tension headaches and prevent migraine headaches.
  • Meditation  may help reduce blood pressure, symptoms of anxiety and depression, and symptoms of irritable bowel syndrome and flare-ups in people with ulcerative colitis. Meditation may also benefit people with insomnia.
  • Tai chi  appears to help improve balance and stability, reduce back pain and pain from knee osteoarthritis, and improve quality of life in people with heart disease, cancer, and other chronic illnesses.
  • Yoga  may benefit people’s general wellness by relieving stress, supporting good health habits, and improving mental/emotional health, sleep, and balance. Yoga may also help with low-back pain and neck pain, anxiety or depressive symptoms associated with difficult life situations, quitting smoking, and quality of life for people with chronic diseases.

The amount of research on psychological and physical approaches varies widely depending on the practice. For example, researchers have done many studies on acupuncture, yoga, spinal manipulation, and meditation, but there have been fewer studies on some other approaches.

Some complementary approaches may not neatly fit into either of these groups—for example, the practices of traditional healers, Ayurvedic medicine , traditional Chinese medicine , homeopathy , naturopathy , and functional medicine.

CHANGE TO THE ALL KIDS 1095-B PROCESS

Most people do not need the 1095-B form to file their taxes this year. Therefore, ALL Kids will not mail a 1095-B form this year. If you still need a 1095-B form, please call ALL Kids Customer Service at 1-888-373-5437 and request one.

Be Sure to Pay Your Premium Before You Renew

To make your renewal process faster, your premium for the previous coverage year must be paid before coverage will be renewed. Premiums can be paid online on the ALL Kids website, and by clicking the "Pay My Premium" link attached below. It will take at least 24 hours for the payment to process, so please wait at least 24 hours after you have paid your premium to submit your renewal. If the payment has not had time to process in the system, the renewal will be denied for non-payment of the premium.

ALL Kids NEVER asks members or applicants for premiums/money or sensitive personal information such as social security numbers by phone or text and will not threaten legal action by phone or text.

Influenza A(H5N2) in Mexico

As of 5 June 2024, a fatal human case of avian influenza A(H5N2) has been reported in Mexico. This is the first laboratory-confirmed human case of influenza A(H5N2) virus reported globally and the first human avian H5 virus infection reported in Mexico.

Dengue in Honduras

On 31 May 2024, the Government of Honduras declared a national health emergency due to an increase in dengue hospital admissions. To date 23,037 dengue cases and 16 confirmed deaths have been reported nationally.

Dengue in Burkina Faso

As of 28 April 2024, a total of 174,356 dengue cases, and 780 deaths, have been reported since the beginning of 2023. DENV-1, DENV-2 and DENV-3 viruses have been identified in the country.

Dengue in Cape Verde

As of 14 April 2024, a total of 1,127 dengue cases (with no deaths), have been reported. DENV-1 and DENV-3 viruses have been detected. Currently DENV-3 is the most common type with DENV-1 only identified in the municipality of Praia.

Dengue in Argentina

As of 18 May 2024, a total of 675,806 dengue cases have been reported in 2024. Of the three circulating dengue virus serotypes identified DENV-2 and DENV-1 account for more than 99.9% of cases with a few cases of DENV-3 also detected.

Dengue in Mauritius

As of 28 April 2024, a total of 7,177 dengue cases and 15 deaths have been reported since the initial outbreak started in December 2023. Currently only DENV-2 serotype has been identified.

Chikungunya in Maldives

As of 4 May 2024, a total of 240 chikungunya cases have been reported in 2024, with the majority of cases (80%) in the capital Male.

Dengue in Samoa

As of 20 May 2024, the Samoan authorities have reported a total of 388 laboratory confirmed dengue cases since November 2023.

Oropouche virus disease in Cuba

As of 27 May 2024, the Cuban authorities have reported that human Oropouche cases have been confirmed in Cuba. Travellers are advised to avoid insect bites.

cVDPV2 in Ethiopia

As of 22 May 2024, six cVDPV2 cases were reported in Ethiopia; one each in Tigray and Afar and two each in Gambella and Amhara. There was one cVDPV2 case reported in 2023 and 5 cases in 2024.

15 men brought to military enlistment office after mass brawl in Moscow Oblast

Local security forces brought 15 men to a military enlistment office after a mass brawl at a warehouse of the Russian Wildberries company in Elektrostal, Moscow Oblast on Feb. 8, Russian Telegram channel Shot reported .

29 people were also taken to police stations. Among the arrested were citizens of Kyrgyzstan.

A mass brawl involving over 100 employees and security personnel broke out at the Wildberries warehouse in Elektrostal on Dec. 8.
