What's the difference between international health insurance and travel insurance?

International health insurance policies are designed for people living or working overseas long term. It covers treatment for both emergency and routine health care, and provides expats with flexibility in terms of choice of doctor and treatment facility, with the ability to receive treatment anywhere within their region of cover. A travel insurance policy is designed to cover short trips abroad, such as a holiday or business trip. This kind of insurance only offers short term cover for medical emergencies, and some non-medical benefits like cover for lost luggage or trip disruptions.

Why do expats need insurance?

The quality of health care can vary widely around the world. The quality of care in your new country may not be to the standard you were used to back home. In addition, as a foreign national, you and your family may not be entitled to any free or subsidized health care. It is therefore important to consider an expat health plan to provide for the health care needs of you and your family.

What does expat health insurance cover?

A Cigna expat health insurance policy is flexible to your needs. Start with one of our core insurance policies - these provide protection for essential hospital stays and treatment. Surgeon and consultation fees and hospital accommodation are also covered. Inpatient, outpatient and cancer treatments are also provided for. In addition, our Gold and Platinum core policies offer levels of cover for routine maternity needs.

You can tailor your policy with a range of optional benefits. International Outpatient provides you with more extensive outpatient cover including doctor’s visits and prescriptions. In addition, you can add medical evacuation, health and well-being and vision and dental benefits to your policy.

Cigna Global Health Insurance Plan offers comprehensive Worldwide coverage. Customizable with three levels to choose from, four optional add-ons and flexible payment options.

How to create your plan : 1. Select your Core Plan: - You have three currencies to choose from: US Dollars, Euro or Sterling - Select your payment frequency: monthly, quarterly (3% discount) or annually (10% discount) - Select your deductible for the Core plan (this is the amount you must pay towards your cost of treatment until the deductible for the period of cover is reached). - Select your Core Plan: Silver, Gold or Platinum (All of core plans cover hospital stays and treatments. Surgeon and consultation fees and hospital accommodation are also covered.) - You can also change the Cost Share of the Core Plan. Cost Share is the percentage you must pay toward your cost of treatment. It is subject to Out of Pocket Maximum (OOP Max). OOP Max is the maximum amount of cost share you have to pay per period of coverage. 2. Add optional modules: - International Outpatient (Consultation with medical practitioner and specialist that does not require overnight hospital admission, diagnostic tests, prescribed drugs, durable medical equipment and more). - International Vision & Dental (Preventative, routine and major dental treatments; Routine eye examination and cost for glasses & lenses). - International Evacuation & Crisis Assistance Plus (Medical evacuation in the event of an emergency & global crisis response service). - International Health & Wellbeing (A broad range of tools to manage your day-to-day health and wellbeing, including preventative cancer screening, emotional support councillors and wellness coaches). 3. Manage your premium: You have the flexibility to adjust your premium by choosing a Deductible and/or Cost Share.  If you choose an annual or quarterly payment for your plan, your premium will be slightly lower than paying monthly.

Their price can escalate significantly at time of renewal and make your health plan unaffordable after the first year of enrollment.   • Out of Area Emergency Hospitalization - up to 21 days per trip to a maximum of 60 days per year when you travel outside of your selected area of coverage. When your country of nationality is within the selected area of coverage, temporary visits to your country are covered up to 180 days per policy year.

Cancellations and Refunds:

This policy is an annual contract. This means that, unless it is terminated earlier, the cover will end 1 year after the start date. This is not a short-term contract. You have a statutory right to cancel your policy within 14 days from the date you receive this policy and receive a full refund (unless there is a claim paid or a guaranteed of payment has been issued). If you want to terminate this policy after the 14 days free look period, you may do so at any time by giving Cigna at least 14 days’ notice in writing. Termination will take effect 14 days after the request. If the policy is terminated, before the end date, and there is a claim paid or a guarantee of payment has been issued during the period of cover, you will be liable for the remainder of any premiums in respect of the policy which are unpaid. If your annual premium is collected at intervals throughout the policy year, you will be responsible for making these payments for the remainder of the period of cover or alternatively, settle the outstanding premium amount.

The easiest way to do this is via your secure online Customer Area. Claims forms and invoices should be submitted as soon as possible after any treatment.  If they are not submitted within 12 months of the date of treatment, the claim will not qualify for payment or reimbursement.

Exclusions:

The following exclusions apply to the International Medical Insurance plan and to all of the extra coverage options. Please also refer to the list of benefits detailed in the Customer Guide, including the notes section for any further restrictions and exclusions that apply, in addition to the General Exclusions. Please also refer to your Certificate of Insurance for any special exclusions that may apply. 1. Treatment which is provided by: a) a medical practitioner who is not recognised by the relevant authorities in the country where the treatment is received as having specialist knowledge of, or expertise in, the treatment of the disease, illness or injury being treated; b) a medical practitioner, therapist, hospital, clinic, or facility to whom Cigna have given written notice that we no longer recognise them as a treatment provider. Details of individuals, institutions and organisations to whom we have given such notice may be obtained by calling our Customer Care Team; or c) a medical practitioner, therapist, hospital, clinic, or facility which, in our reasonable opinion, is either not properly qualified or authorised to provide treatment, or is not competent to provide treatment. 2. Treatment for: a) a pre-existing condition; or b) any condition or symptoms which result from, or are related to, a pre-existing condition. Cigna will not pay for treatment for a pre-existing condition of which the policyholder was (or should reasonably have been) aware at the date cover commenced, and in respect of which we have not expressly agreed to provide cover. 3. Preventative treatment, including but not limited to health screening, routine health checks and vaccinations (unless that treatment is available under the International Medical Insurance plan or one of the options for which a beneficiary has cover). Under the International Medical Insurance plan, the limits of cover for preventative surgery in respect of congenital conditions will apply, other than for cancer. 4. Treatment which is provided by anyone who lives at the same address as the beneficiary, or who is a member of the beneficiary’s family. 5. Treatment which is necessary as a result of conflict or disaster including but not limited to: a) nuclear or chemical contamination; b) war, invasion, acts of terrorism, rebellion (whether or not war is declared), civil war, commotion, military coup or other usurpation of power, martial law, riot, or the act of any unlawfully constituted authority; c) any other conflict or disaster events; where the beneficiary has: i) put him or herself in danger by entering a known area of conflict (as identified by a Government in your country of nationality, for example the British Foreign and Commonwealth Office); ii) actively participated in the conflict; or iii) displayed a blatant disregard for their own safety. 6. Any treatment outside your selected area of coverage, unless the treatment can be covered under the ‘Out of Area Emergency Hospitalisation Cover’ conditions. 7.  Travel costs for treatment including any fares such as taxis or buses, unless otherwise specified, and expenses such as petrol or parking fees. 8.  Any expenses for ship to shore evacuations. 9.  Treatment in nature cure clinics, health spas, nursing homes, or other facilities which are not hospitals or recognised medical treatment providers. 10. Charges for residential stays in hospital which are arranged wholly or partly for domestic reasons or where treatment is not required or where the hospital has effectively become the place of domicile or permanent abode. 11.  Costs of hospital accommodation for a deluxe, executive or VIP suite. 12.  Any prosthetic device or appliance, including but not limited to hearing aids and spectacles (unless the International Vision & Dental module is selected) which is not medically necessary and/or does not fall within our definition of prosthetic device(s). 13. Incidental costs including newspapers, telephone calls, guests’ meals and hotel accommodation. 14. Costs or fees for filling in a claim form or other administration charges. 15. Non-medical admissions or stays in hospital which include: a) treatment that could take place on a daypatient or outpatient basis; b) convalescence; c) admissions and stays for social or domestic reasons e.g. washing, dressing and bathing. 16. Life support treatment (such as mechanical ventilation) unless such treatment has a reasonable prospect of resulting in the beneficiary’s recovery, or restoring the beneficiary to his or her previous state of health. 17. Foetal surgery, i.e. treatment or surgery undertaken in the womb before birth, unless this is resulting from complications arising through maternity and shall be subject to the limits detailed in the Complications from Maternity benefit under the International Medical Insurance plan. 18.  Footcare by a Chiropodist or Podiatrist. 19. Treatment for, or in connection with, smoking cessation. 20.  Treatment that arises from, or is in any way connected with attempted suicide, or any injury or illness that the beneficiary inflicts upon him or herself. 21.  Developmental problems, treatment for personality and/or character disorders, including but not limited to: a) learning difficulties such as dyslexia; b) physical development problems such as short height; c) affective personality disorder; d) schizoid personality disorder; or e) histronic personality disorder. 22.  Disorders of the temporomandibular joint (TMJ). 23. Treatment for a related condition resulting from addictive conditions and disorders. 24. Treatment for a related condition resulting from any kind of substance or alcohol use or misuse. 25. Treatment needed because of, or relating to, male or female birth control, including but not limited to: a) surgical contraception, namely: - vasectomy, sterilisation or implants; b) non-surgical contraception, namely: -  pills or condoms; c) family planning, namely: - meeting a doctor to discuss becoming pregnant or contraception. 26. Treatment by way of the intentional termination of pregnancy, unless the pregnancy endangers a beneficiary’s life or mental stability. 27. Treatment for sexual dysfunction disorders (such as impotence) or other sexual problems regardless of the underlying cause. 28. Treatment which is intended to change the refraction of one or both eyes, including but not limited to laser treatment, refractive keratotomy and photorefractive keratectomy. Note that we will pay for treatment to correct or restore eyesight if it is needed as a result of a disease, illness or injury (such as cataracts or a detached retina). 29. Gender reassignment surgery, including elective procedures and any medical or psychological counselling in preparation for, or subsequent to, any such surgery, unless state or federal law requires such coverage. We will cover medically necessary behavioural health services, including but not limited to, counselling for gender dysphoria and related psychiatric conditions (such as anxiety and depression) and medically necessary hormonal therapy. 30. Treatment which is necessary because of, or is any way connected with, any injury or sickness suffered by a beneficiary as a result of: a) taking part in a sporting activity at a professional level; b) taking part in a hazardous sporting activity or hobby, including but not limited to off-piste winter sports, base jumping, tombstoning or cliff jumping, mountaineering or rock climbing, potholing, motorsports, equestrian sports (for instance horse racing or jumping, polo, or hunting), bull riding or bull running, parkour; c) solo scuba-diving; or d) scuba-diving at a depth of more than thirty (30) metres unless the beneficiary is appropriately qualified (namely PADI or equivalent) to scuba-dive at that depth. 31. Treatment which (in our reasonable opinion) is experimental, or has not been proven to be effective. This includes but is not limited to: a) treatment which is provided as part of a clinical trial; b) treatment which has not been approved by the relevant public health authority in the country in which it is received; or c) any drug or medicine which is prescribed for a purpose for which it has not been licensed or approved in the country in which it is prescribed. 32. Any form of cosmetic or reconstructive treatment and any complication thereof, the purpose of which is to alter or improve appearance even for psychological reasons, unless that treatment is medically necessary and is a direct result of an illness or an injury suffered by the beneficiary, or as a result of surgery. 33. Treatment that is in any way caused by, or necessary because of, a beneficiary carrying out an illegal act.

Cigna will not offer cover or pay claims when it is illegal for us to do so under applicable laws. Examples include but are not limited to, exchange controls, local licensing regulations or trade embargo.

In accordance with clause 19, Cigna will not cover any beneficiaries or pay claims in jurisdictions when doing so would violate applicable trade restrictions, including but not limited to: restrictions imposed by the United States Department of Treasury’s Office of Foreign Assets Control; the European Union Commission, or; the United Nations Security Council Sanctions Committees.

Cigna Global Individual Private Medical Insurance provides excellent health insurance overseas for the most discerning expatriates and globally mobile individuals. There are three coverage levels, various additions, and an array of deductible and cost share options. You can be sure to find a plan which meets both your exacting overseas health requirements and your budget. Coverage is available for expatriates, business people working overseas and international volunteers with NGOs. Also nonprofits, humanitarian organizations and the faith-based community.

The Cigna Global travel insurance plans have a unique way of operating. Let’s start from the beginning, which is the coverage that is provided at minimum – the Core! The Core coverage on these plans is for hospitalizations, inpatient psychiatric care, and cancer. Newborn care is also included for all three levels of coverage. Maternity benefits are included with Gold and Platinum after 12 months of coverage. Expenses from doctors visits, prescriptions or medications, and physiotherapy or physical therapy are not covered at the Core coverages.

You may add the following modules to your coverage. Each of these coverage levels provides additional benefits, which are tailored for maximum proficiency. This is a prominent reason that the Cigna Global plans are extraordinary health insurance overseas!

International Outpatient :

This module is for expenses from lab work, doctors visits, prescriptions and physical therapy. Also immunizations, as well as annual routine for children and well child tests. The higher the level of coverage, the lower the caps are on some benefits.

International Medical Evacuation :

This coverage is highly recommended for any type of international travel. International medical evacuation insurance is designed to provide transportation to the nearest adequate medical facility for necessary treatment. This is if there is no local treatment available for the immediate medical necessity. Adding this module will provider you with not only medical evacuation coverage, but also repatriation of mortal remains, compassionate visit, and more. All of the benefits are the same for every level of coverage.

Even though great hospitals may be in your destination, they may not be able to handle every type of medical condition. We never want any individual, or their families, to be stuck with a medical evacuation bill!

International Health and Wellbeing :

This is a wonderful addition to support overall health and wellbeing! It is a rich resource that gives you access for health screenings and check-ups, physical exams, counseling, dietetic consultations, and online education and programs. All of this can be included to help you get the most out of your overseas medical insurance.

International Vision and Dental :

General coverage for tests, preventative care, and some coverage for treatment and materials relating to Dental and Vision.

U.S. Coverage :

Having U.S. coverage is very important for those with travels to the U.S.A., and for U.S. citizens. Coverage up to six months per year is available on the Cigna Global international health and travel insurance. Even if a U.S. citizen doesn’t plan to come back to the U.S., if a serious medical condition occurs, they sometimes go back to the U.S. for care. Citizens of other countries traveling outside the U.S. will save money by not selecting this module.

In Summary:

For the Core and for the International Outpatient module, you have the option to choose your deductibles, as well as cost sharing. Cost sharing is where you can choose a sum of money to pay a percentage of after the deductible. The higher the amount and percentage you pay with the cost share, the lower the premium will be. On the other hand, the higher your deductible and cost share you have, the more you will pay before full benefits kick in. When you get quotes, you will see how each module affects the premium.

Ask the Expert: What you need to know about travel insurance before you go on a trip

Canadians looking to ditch the cold this winter for a sunny destination may want to be prepared in case something goes awry.

While most of the travel issues from the pandemic aftermath are mostly resolved, the industry is still working out the kinks and travel insurance can provide protection in case things don’t go as planned, according to Matt Hands, vice-president of insurance at Ratehub.ca.

“The travel industry is still in a state of flux. It's in a much better position today than it was a year ago, but flight delays will happen,” Hands said in a recent interview with Yahoo Finance Canada.

“There’s a lot of baggage loss that happens. So having that peace of mind in coverage, which isn't quite expensive to add on to a policy – the most expensive part is obviously travel medical — is just something to give peace of mind in case something goes wrong.”

Despite this peace of mind, not everyone buys a travel insurance package before their trip. A 2022 survey from insurance software firm Ancileo found about 68 per cent of Canadians planned to buy travel insurance, while Baby Boomers (73 per cent) were most likely to purchase travel insurance.

Still, there are some tricks to travel insurance that can ensure you’re covered in case your dream trip doesn’t go as smoothly as you envisioned. Yahoo Finance Canada spoke to Hands and Suzanne Morrow, senior vice-president at InsureMyTrip.ca, to discuss all things travel insurance.

What kind of travel insurance plan do I need?

Both experts say a base travel insurance package is appropriate for a short road trip or weekend getaway in the U.S., but anything longer or further may need a more comprehensive plan.

“If you're doing any sort of risky activity, maybe you're going skiing overseas, or maybe you’re going paragliding … this is where it gets a little bit more complex,” Hands said. “You'll want to have a specific type of policy that covers more risky activities and medical coverage because standard policies won't do that.”

Additionally, pre-existing health conditions, the cost of the trip, the modes of transportation you will be using and your destination are things to consider before choosing the level of coverage you may need.

Morrow says any trip with a non-refundable portion that has already been paid is a candidate for insurance as a means to protect against cancellation or interruptions.

“If they're planning a trip and they've put money on the line, they may want to have coverage should something happen to cancel their trip,” she said

Read the fine print and check your credit card insurance

Many Canadians will have some level of travel insurance coverage through either their credit card plan or employer, but it’s important to check exactly what level of coverage these plans provide.

“With credit card (travel insurance), anything you want coverage for has to be purchased through your credit card, which makes sense,” Morrow said.

“If it's limited coverage for things that you're concerned about, you may want to look into buying something in the retail market.”

Morrow adds that many employers only offer emergency medical insurance through company benefits, meaning travellers will likely need to look elsewhere for trip cancellation insurance.

“Know what you have so that you can plan for what you may need to buy,” she said.

“The biggest mistake that we hear about is people assume that they are covered. They don't check, they don't verify, they just assume they're good.”

What snowbirds need to know

Snowbirds, or older Canadians who spend their winters in a warmer climate, should consider their own type of insurance package, Hands says. These packages offer multi-trip coverage over an extended stay and include robust coverage for medical expenses, he adds.

“They're all-inclusive, they include a lot of travel medical coverage, because they're designed with an older clientele in mind, so they're expecting them to need potentially hospital treatment, medical supplies, ambulance services, or physician services, or even as much as emergency dental work, or emergency return to Canada,” Hands said.

According to Statistics Canada , as many as 375,000 Canadians spend at least part of their winter in the U.S. or Mexico. The number of snowbirds may soon rise as well, as Canada’s population ages and U.S. officials consider loosening restrictions on snowbird travel.

Hands says snowbirds considering spending more than a month away from home should tell their home insurance provider about their travel plans.

“If you were to leave, and you had nobody checking on your house, or you didn't do the proper precautions in terms of pipes freezing someone and something happens, they can easily reject your claim,” he said.

Senators See Possible Conflicts of Interest in Health Care Pricing Tools

A data analytics firm that helps insurers collect big fees while leaving some patients with unpaid bills has been summoned to explain its business model.

The chairmen of two Senate committees overseeing health policy, concerned about companies “padding their own profits” at the expense of patients, are looking into the practices of a data analytics firm that works with big insurers to cut payments to medical providers.

The firm, MultiPlan, recommends what it says are fair payments for medical care, but the firm and the insurers can collect higher fees when payouts are lower. This business model could “result in an improper conflict of interest,” the chairmen of the two committees, Ron Wyden of Oregon and Bernie Sanders of Vermont, wrote in a letter to the firm’s chief executive that was released on Tuesday.

The senators called on MultiPlan to meet with the committees’ staffs to discuss an investigation last month by The New York Times that found the firm’s pricing tools could leave patients with unexpectedly large bills when they see doctors outside their health plans’ networks.

“Our committees are engaged in ongoing legislative work to put a stop to practices by plan service providers that drive up health care costs for consumers while padding their own profits,” the letter to Travis Dalton, the MultiPlan chief executive, said.

In a statement, MultiPlan said it was working with the Senate committees “to address their questions and explain the cost and complexity patients can face” when choosing high-priced care outside their networks. “We are committed to helping make health care transparent, fair and affordable for all,” the statement said.

The committees’ inquiry reflects growing scrutiny of the New York-based firm, which has largely remained out of the limelight even as it has staked out a dominant position in a lucrative corner of health care.

Another senator, Amy Klobuchar of Minnesota, this month asked federal antitrust regulators to investigate whether insurers and MultiPlan were colluding to fix prices, and multiple health systems have sued the firm, accusing it of similar anticompetitive behavior.

Separately, the Department of Labor said Tuesday that it had “a number of open investigations” into the type of pricing services MultiPlan provides, but declined to name specific companies. The agency, the primary regulator of employer-based health insurance, stressed in a statement that companies were legally obligated to ensure the firms processing medical claims acted in their employees’ best interest.

The letter from Mr. Wyden, a Democrat, and Mr. Sanders, an independent, also steps up attention on employer-based health insurance, which is the most common way Americans get coverage and a major component of MultiPlan’s business.

As health care costs climb, some employers are looking more closely at what they pay insurance companies to administer their plans, but they are often frustrated by contracts that limit access to their own claims data. To address this, a bipartisan group of senators, including Mr. Sanders, introduced legislation in December that would require insurers to turn over this data .

“Most businesses do their best to manage the ever-increasing cost of their group health plan, but it should be easier,” Senator Mike Braun, an Indiana Republican and cosponsor of the bill, said in a statement.

A majority of employers choose to pay medical claims with their own money and use an insurer to administer their plans. This setup, known as “self-funding,” can be lucrative for insurers like UnitedHealthcare, Cigna and Aetna, as well as specialized firms like MultiPlan.

The insurers pitch MultiPlan’s tools as a way to save employers money when their employees see a medical provider outside the plan’s network. The bills for these out-of-network providers are subject to negotiation, and insurers often send the claims to MultiPlan, which recommends an amount to pay.

Both MultiPlan and insurers typically collect a fee from the employer based on the size of what they call the “savings” — the provider’s list price minus the recommended payment. Lower payouts can mean bigger fees. Meanwhile, patients can be stuck with the unpaid balance, The Times investigation found.

Companies are legally obligated to ensure the insurers act in employees’ best interest, and a closely watched lawsuit filed last year could force them to become more active monitors.

A worker at Johnson & Johnson sued the company, saying it had failed to adequately oversee the administrator of its drug benefits plan. By paying too much — in one instance, $10,000 for a drug that was available for as little as $28.40 — the company had allowed the administrator, the Cigna subsidiary Express Scripts, to profit at employees’ expense, the suit claimed.

In a statement, Johnson & Johnson called the claims “meritless” and said, “We are committed to our employees and seek to provide the best coverage.”

A small industry of consultants, lawyers and data analysts has arisen to help companies step up monitoring and negotiate better deals with the insurers administering their plans.

Kraft Heinz last year sued Aetna, claiming the insurer improperly paid claims and kept millions in undisclosed fees. Trustees for a union health plan in Massachusetts sued Blue Cross Blue Shield of Massachusetts in 2021, accusing the insurer of repeatedly overpaying claims and then charging a fee to correct the errors. And in January the Department of Labor sued Blue Cross and Blue Shield of Minnesota, claiming the company forced multiple employers to pay medical providers’ tax bills without disclosing the charges.

(Aetna declined to comment on the case but said it worked with employers “to facilitate access to quality, affordable and convenient health care.” Blue Cross and Blue Shield of Minnesota said the government’s allegations were “without merit” and “based on unsupported interpretations” of the law. A court dismissed the Massachusetts case.)

The success of the employers’ efforts sometimes hinges on an unsettled legal question: Does a company’s duty to act solely in its employees’ best interest extend to insurers and firms like MultiPlan? Courts have reached different conclusions.

MultiPlan has argued that the answer is no, and in March a federal judge in California agreed, dismissing the company from a lawsuit filed by medical providers. The case against the insurer, Cigna, was allowed to go forward.

In pitches to investors, MultiPlan has highlighted its murky legal obligations. Because the firm doesn’t provide insurance or pay claims, it noted in a public filing, “we generally are not directly regulated and face significantly lower levels of regulatory complexity.”
